Abstract (German)

Nach einer erfolgten Lebertransplantation bei Kindern besteht ein Zusammenhang zischen der Organfunktion des Transplantates und der Menge an exogen zugeführter Glukosemenge, um einen vorgegebenen Blutzuckerkorridor von 120-200 mg/dl (=6,7-11.1 mmol/L) zu halten. Unsere Studie zeigte, dass das ein oder mehrmalige Überschreiten des BZ-Mittelwertes pro Tag > 200 mg/dl an einem oder mehreren Tage innerhalb einer Woche nach OP nicht mit einer erhöhten Rate an Komplikationen assoziiert ist. Ein hoher BZ-Mittelwert von Tag 1 bis inclusive Tag 7 post pLTx war assoziiet mit einer längeren Beatmungsdauer, längeren Liegezeit auf Intensivstation und einer erhöhten Letalität, Es stellt sich die Frage,ob ein hoher BZ-Wert der Grund für die aufgetretenen Komplikationen ist, oder nicht eher viel mehr ein Parameter für die Schwere der aktuellen Erkrankung des Patienten. Die Insulintherapie zur Einstellung des BZs auf Werte von 120-220 mg/dl war sicher, es kam zu keinen Hypoglykämien.

Postoperative glucose intake to keep BSL within the targeted range of 120 to 200 mg/dl (6.7-11.1 mmol/L) is linked to graft function. Translation of the abstract (English)

Postoperative glucose intake to keep BSL within the targeted range of 120 to 200 mg/dl (6.7-11.1 mmol/L) is linked to graft function. In our study population, short term hyperglycemia was not associated with complications. High postoperativ mean 7-day BSL was associated with increased morbidity and 6-month posttransplant mortality. With hyperglycemia being associated with poor glucose utilization, it is likely that the illness itself and poorgraft function caused this increase, with hyperglycemia as a marker. In our cohort, administration of insulin for treatment of very high BSL in the early pahse after pLTx was safe.
