Abstract (German)
Der diagnostische Zusatznutzen von Antikörper gegen mutiertes citrulliniertes Vimentin
(Anti-MCV-AK) und Antikörper gegen cyclische citrullinierte Peptide (Anti-CCP-AK), vor allem bei Rheumafaktor (RF)-negativen Patienten, ist in der Rheumatologie mittlerweile gut etabliert. So wurden diese beiden Immunparameter 2010 auch in die EULAR-Klassifikationskriterien für die Rheumatoide Arthritis (RA) mit aufgenommen. Das Verhalten dieser Antikörper im Kurs der Erkrankung und ihr Wert im individuellen Follow-up sind allerdings noch nicht ausreichend geklärt.
Ziel dieser Studie war es zu untersuchen, ob die Höhe der Titer von Anti-CCP-AK, Anti-MCV-AK und Rheumafaktor bei Patienten mit Rheumatoider Arthritis im Krankheitsverlauf mit der Krankheitsaktivität korreliert. Retrospektiv wurden die Titer für Anti-CCP-AK, Anti-MCV-AK, Rheumafaktor, CRP, BSG und der Disease Activity Score 28 (DAS28), als Marker für die Krankheitsaktivität, von 78 Patienten zu 6 Zeitpunkten innerhalb von zwei Jahren erfasst und ausgewertet.
Nach Feststellung der Tendenzen im zeitlichen Verlauf wurden jeweils für Anti-MCV-AK, Anti-CCP-AK, RF, CRP und BSG Phi-Korrelationen mit dem DAS28 errechnet. Dabei ergab sich eine mittelstarke, signifikante Korrelation für CRP, BSG und RF, eine leichte, signifikante Korrelation für Anti-MCV-AK und keine signifikante Korrelation für Anti-CCP-AK.
Bei weiterführenden Untersuchungen von Patienten mit guter Korrelation und solchen mit schlechter bis gar keiner Korrelation wurde versucht anhand des Mann-Whitney U Tests und Chi-Quadrat-Tests Unterschiede zu definieren. Dies gelang vor allem im Bereich der Entzündung, nämlich bei RF, CRP, BSG, DAS28, Visuelle Analogskala (VAS), Prednisolon-Einnahme und -Tagesdosis.
Wegen dem Einfluss auf die RA-spezifischen Antikörper wurden Patienten, die mit Rituximab behandelt wurden, aus der ursprünglichen Studienpopulation ausgeschlossen und gesondert betrachtet. Hier waren die Ergebnisse hinsichtlich CRP, BSG, RF und Anti-MCV-AK vergleichbar mit der Hauptgruppe. Auffällig war eine relativ starke, signifikante Korrelation von Anti-CCP-AK. Dies könnte aber mit der Wirkung des Medikaments erklärt werden. Signifikante Unterschiede zwischen gut und schlecht korrelierenden Personen wurden für Anti-MCV-AK im Bereich DMARD- und Prednisolon-Einnahme gefunden, für Anti-CCP-AK bei DAS28, VAS und Prednisolon-Tagesdosis.
Des Weiteren fand sich tendenziell bei mehr Entzündung eine stärkere Korrelation der ACPAs mit dem DAS28.
Zusammengefasst lässt sich feststellen, dass Anti-MCV-AK besser mit der Krankheitsaktivität korrelieren, als Anti-CCP-AK. Stärker als die beiden noch korreliert RF mit DAS28. Da die gefundene Korrelation in dieser Kohorte aber nur relativ schwach ist, ist nicht davon auszugehen, dass sich die wiederholte Abnahme der Antikörper generell im RA-Monitoring lohnen wird.

The additional diagnostic benefit of antibodies against mutated citrullinated vimentin (anti-MCV) and antibodies against cyclic citrullinated peptides (anti-CCP), especially in rheumatoid factor (RF)-negative patients, is well established. Therefor these two immune parameters were included in the EULAR classification criteria for rheumatoid arthritis (RA) in 2010. However, the evolvement of these antibodies in the course of the disease and their value in the individual follow-up have not yet been sufficiently assessed. The aim of this study was to determine whether the level of titers of anti-CCP, anti-MCV and rheumatoid factor in patients with rheumatoid arthritis correlates with disease activity during the course of the disease.
Retrospectively, titers for anti-CCP, anti-MCV, rheumatoid factor, CRP, BSG and Disease Activity Score 28 (DAS28), as marker for disease activity, of 78 patients were collected and analyzed at six points during two years.
After determining the trends over time, anti-MCV, anti-CCP, RF, CRP and BSG Phi correlations with DAS28 were calculated. This resulted in a medium strong, significant correlation with CRP, BSG and RF, a slight, significant correlation with anti-MCV-AK and no significant correlation with anti-CCP-AK.
In further studies of patients with good correlation and those with poor or no correlation, the Mann-Whitney U test and chi-square tests were used to determine differences. This was particularly successful in the area of inflammation, namely RF, CRP, BSG, DAS28, visual analogue scale (VAS), prednisolone intake and daily dosage of prednisolone.
Patients treated with rituximab were excluded from the original study population and considered separately due to the influence of the medication on RA-specific antibodies. The results regarding CRP, BSG, RF and anti-MCV were comparable with the main group, but a relatively strong, significant correlation was found with anti-CCP. However, this could be explained by the effect of the drug. Significant differences between good and poor correlating individuals were found for anti-MCV in the DMARD and prednisolone intake, for anti-CCP in DAS28, VAS and daily dosage of prednisolone.
Furthermore, with more inflammation, a stronger correlation of anti-MCV and anti-CCP with DAS28 was found.
In summary, anti-MCV correlates better with disease activity than anti-CCP. RF correlates more strongly with DAS28 than both of them. But since the correlation found in this cohort is relatively weak, it cannot be assumed that repeated measurements of anti-MCV and anti-CCP antibodies are suited for monitoring the course of disease in RA-patients.
