Abstract (German)
Ziel dieser Arbeit war die Untersuchung der Effekte einer Behandlung mit dem TNFR2 (Tumornekrosefaktor-Rezeptor 2)-selektiven Agonisten TNF80 in unterschiedlichen Entzündungszuständen. Zur Induktion einer chronischen Entzündung diente das BCG (Bacille Calmette-Guérin)-Mausmodell von Sade-Feldman et al. (Sade-Feldman et al. 2013). Um eine akute Entzündungsreaktion auszulösen, verabreichten wir BCG bzw. LPS (Lipopolysaccharid) einmalig als Challenge. Als Messgrößen dienten Gewicht, Zellzahl und Zellverteilung in der Milz bzw. im Knochenmark, die in vitro-T-Zell-Suppressivität sowie die Konzentration verschiedener Zytokine in Serum und Zellkulturüberständen .
Die wiederholte Injektion von BCG führte zu einer Notfallmyelopoese mit Expansion myeloider Zellen im Knochenmark und in der Milz. Gleichzeitig kam es zu einer Reduktion der B- und T-Zellen. Diese Entzündungsreaktion war weitestgehend unabhängig von der TNFR2-Expression der einzelnen Zellen. Ob die Verminderung der Lymphozyten auf Verdrängung, direkter Suppression durch MDSCs (myeloide Suppressorzellen) oder Migration in die Peripherie beruht, könnte Gegenstand weiterer Studien sein.
Eine zweimalige Behandlung naiver Mäuse mit TNF80 führte ähnlich einer alleinigen BCG-Challenge zu einer akuten Entzündungsreaktion mit Zunahme von Milzgewicht und -zellzahl. Diese Behandlung führte zu einer Toleranz gegenüber weiteren Entzündungsstimuli durch eine BCG-Challenge. Im chronischen Entzündungsmodell führte die TNF80-Behandlung zu einer relativen Expansion der Treg-Fraktion, ohne Einfluss auf Milzgewicht und -zellzahl zu nehmen. Da in einer anderen Studie mehrfache Applikationen von TNF80 nötig waren, um Treg zu vermehren (Schmid et al. 2017), ist anzunehmen, dass die entzündliche Umgebung die Treg empfänglicher gegenüber einer TNFR2-Aktivierung macht.
Wir zeigten außerdem, dass eine ein- oder zweimalige Behandlung mit TNF80 in akut oder chronisch entzündeten Mäusen zu einer Reduktion der Serum-TNF-Spiegel führt. Diese Toleranz gegenüber einem akuten Entzündungsstimulus, in diesem Fall gegenüber einer LPS-Injektion, bezeichneten wir als TNF80-Toleranz. Gleichzeitig kam es durch die Behandlung zu einer Reduktion des immunsuppressiven IL-10 (Interleukin 10) im Serum. Ob IL-10 dennoch an der Toleranzentwicklung beteiligt ist, ließe sich beispielsweise durch die zusätzliche Applikation von anti-IL-10 mAb untersuchen.
Die in vitro-Suppressivität der Treg wurde im chronischen Entzündungsmodell durch eine einmalige Behandlung mit TNF80 gesteigert. Eine zweimalige Behandlung führte zwar zur Expansion von Treg, diese zeigten sich jedoch weniger suppressiv als die nicht behandelter Mäuse. Die im Vergleich zu naiven Mäusen erhöhten IFN-ɣ (Interferon ɣ)-Konzentrationen in den Überständen der Treg chronisch entzündeter Mäuse konnten auch durch die einmalige TNF80-Behandlung nicht gesenkt werden. Allerdings führte die einmalige Behandlung hier zu einer deutlichen Steigerung der IL-10-Produktion im Überstand.
Die in vivo- Ergebnisse der Durchflusszytometrie und der Zytokinbestimmung in Serum steht teilweise im Widerspruch mit den in vitro-Ergebnissen. Um eine genauere Aussage bezüglich der Wirkung von TNF80 auf die Treg-Suppressivität sowie die Zytokinspiegel im Serum treffen zu können, wäre eine Untersuchung im in vivo-Suppressionsassay sinnvoll.
Zusammenfassend stellt die Behandlung von Pathologien mit chronischer Entzündung durch TNF80 einen interessanten Ansatz dar. Aufgrund der komplexen Wirkung in Abhängigkeit von der Applikationsfrequenz sowie des Entzündungszustandes sind jedoch ausführliche weitere Studien notwendig, um die Effekte einer Behandlung ausreichend abschätzen zu können.

The aim of this dissertation was the examination of the effects of treatment with a TNFR2 (tumor necrosis factor receptor 2) specific agonist (TNF80) in different states of inflammation in a mouse model. We achieved chronic inflammation through the BCG (Bacillus Calmette Guérrin) model presented by Sade-Feldman et al. (Sade-Feldman et al. 2013). In order to induce acute inflammation we administered BCG and LPS (lipopolysaccharide) once, respectively, as a challenge. We used weight, cell count and relative cell distribution in the spleen and in the bone marrow as well as in vitro T cell suppressive power and the concentration of a number of cytokines in the serum and supernatants of cell culture as indicators of the inflammatory status.
Repeated injection of BCG led to emergency myelopoesis with an expansion of myeloid cells in the bone marrow and in the spleen, accompanied by a reduction in B and T cells. This inflammatory reaction was mostly independent of TNFR2 expression on the respective cells. Wether the reduction in lymphocytes was due to extrusion, direct suppression through MDSCs (myeloid derived suppressor cells) or migration to the periphery could be subject to further studies.
Two courses of treatment of naive mice with TNF80 led to an acute inflammatory reaction, similar to a sole BCG-challenge, with an increase in spleen weight and cell count. This treament induced tolerance towards further inflammatory stimulation by a BCG-challenge. In the model for chronic inflammation the treatment with TNF80 prompted a relative expansion of Treg but did not have any effect on spleen weight and cell count. An earlier study showed that repeated applications of TNF80 were required to increase Treg numbers (Schmid et al. 2017), so can be argued that the inflammatory environment increases Treg sensitivity towards an activation of TNFR2.
Furthermore, we showed that one or two courses of treatment with TNF80 in acute or chronic inflammation led to a reduction in serum TNF levels. We called this tolerance towards an acute inflammatory stimulatus – in this case towards an injection of LPS – TNF80 tolerance. Simultaneously, the treatment caused a reduction of the immunosuppressive IL-10 (interleukin 10) in the serum. In vitro suppressive power of Treg cells of chronically inflamed mice could be increased by a single treatment with TNF80. Two courses of treatment led to an expansion of Treg cells, however, these Treg cells were less suppressive than those of mice which had not undergone treatment. Chronically inflamed mice showed higher concentrations of IFN-ɣ (interferone ɣ) in the supernatants of their Treg cells. Treatment with TNF80 had no impact on IFN- ɣ levels in the supernatants, however, a single treatment induced a significant increase in IL-10 levels.
The in vivo results we obtained by flow cytometry and cytokine measurements in the serum partially contradict the findings of the in vitro experiments. An analysis in an in vivo suppressive assay would be interesting in order to better understand the effects of TNF80 on Treg suppressive power as well as cytokine levels in the serum.
In summary, the treatment of pathologies with chronic inflammation with TNF80 is an interesting approach. Extensive further research is needed in order to understand the expectable consequences of the treatment because of the complex effects depending on the frequency of application and the inflammatory status.
