Abstract (German)
Die Stammzellnische im Knochenmark spielt eine zentrale Rolle in der lebenslangen Regulation der Blutbildung. Als eine der wichtigsten Zellpopulationen in der Nische konnten multipotente mesenchymale Stromazellen (MSZ) identifiziert werden. Durch die direkte Interaktion mit benachbarten Zellen und die Sekretion löslicher Signalmoleküle leisten MSZ einen wichtigen Beitrag in der Aufrechterhaltung der Homöostase im Knochenmark. Veränderungen in den MSZ können aber auch die Entartung hämatopoetischer Zellen fördern und so potentiell zur Entstehung maligner Knochenmarkserkrankungen beitragen. Im Vorfeld dieser Arbeit konnte Stanniocalcin 1 (STC-1) mithilfe einer Genexpressionsanalyse als Gen mit der stärksten Überexpression in den MSZ von AML-Patienten verglichen mit den MSZ hämatologisch gesunder Kontrollen bestimmt werden. Anknüpfend daran hatte die vorliegende Dissertation zum Ziel, die Überexpression von STC-1 in leukämischen MSZ zu verifizieren und mögliche Auswirkungen von STC-1 auf das Wachstum von AML-Zellen zu analysieren.
Dafür wurden zunächst MSZ aus dem Knochenmark von AML-Patienten und hämatologisch gesunden Spendern isoliert und in vitro expandiert. Um sicherzustellen, dass es sich um MSZ handelt, wurden die Zellen hinsichtlich des Oberflächenexpressionsprofils sowie des Differenzierungspotentials in Fett-, Knorpel- und Knochenzellen untersucht. Anschließend wurde die STC-1 Expression in den MSZ auf mRNA-, Protein- und humoraler Ebene analysiert, um mögliche Unterschiede zwischen den leukämischen und gesunden MSZ darstellen zu können. Zusätzlich wurden immunhistochemische Färbungen zur Untersuchung der STC-1 Expression in Knochenmarksschnitten von AML-Patienten und hämatologisch gesunden Spendern sowie in MSZ-Cytospinpräparaten durchgeführt. Im letzten Teil der Arbeit erfolgten in vitro Kulturen von AML-Zellen mit rekombinantem STC-1 sowie Ko-Kulturen von AML-Zellen und MSZ, wobei in einem Teil der Kulturen zusätzlich die Blockade von STC-1 durch die Gabe eines Antikörpers vorgenommen wurde. Anschließend erfolgte die Untersuchung der AML-Zellen hinsichtlich Apoptoserate und Zellproliferation.
Dabei konnten wir in den immunhistochemischen Untersuchungen eine deutlich verstärkte Expression von STC-1 im Knochenmark von AML-Patienten nachweisen und durch die immunhistochemische Färbung von Cytospins wurde bewiesen, dass STC-1 spezifisch von den MSZ exprimiert und gebildet wird. Bezüglich der mRNA- und Proteinexpression von STC-1 konnten in dem dieser Arbeit zugrundeliegenden Kollektiv von MSZ-Proben keine signifikanten Unterschiede zwischen den MSZ von AML-Patienten und den Kontrollen aus dem Knochenmark von hämatologisch gesunden Patienten beobachtet werden, es zeigte sich jedoch ein Trend zugunsten einer Überexpression von STC-1 in den MSZ von AML-Patienten. Bei der aktiven Sezernierung von STC-1 in das Kulturmedium hingegen wurde ein signifikant höherer Wert der AML-MSZ im Vergleich zu den MSZ knochenmarksgesunder Spender gefunden. Die Behandlung mit rekombinantem STC-1 in der AML-Primärkultur erzielte insgesamt keine signifikante Beeinflussung des Tumorzellwachstums, ebenso wirkte sich auch die Blockade von STC-1 in der Ko-Kultur mit MSZ nicht signifikant auf Proliferation und Apoptose der AML-Zellen aus.
Bezüglich der Rolle von STC-1 in der Tumorentstehung und -progression finden sich in der Literatur zum Teil Ergebnisse, die nicht vollständig mit den Ergebnissen dieser Dissertation übereinstimmen. So wurde eine von STC-1 vermittelte Reduktion der Apoptoserate99 sowie eine durch die Ko-Kultur mit MSZ gesteigerte Proliferationsrate von Tumorzellen beschrieben100, was in den Experimenten dieser Arbeit nicht beobachtet wurde. Diese Befunde sind jedoch kritisch zu betrachten, da in den entsprechenden Studien stets mit permanenten Zelllinien gearbeitet wurde, während in der vorliegenden Arbeit ausschließlich primäre Zellen zur Verwendung kamen. Daneben unterschieden sich die Versuchsbedingungen in der vorliegenden Arbeit zum Teil erheblich von denjenigen der bisher existierenden Arbeiten auf diesem Themengebiet.
Inwieweit die Überexpression von STC-1 in MSZ in die Entstehung und Progression der AML involviert ist, konnte in der vorliegenden Dissertation nicht abschließend geklärt werden. Mithilfe einer erneuten Genchip-Analyse zur Untersuchung des Transkriptoms von MSZ aus dem Knochenmark von AML-Patienten und hämatologisch gesunden Spendern konnten wir jedoch belegen, dass Einflüsse des in der vorliegenden Dissertation verwendeten Nährmediums (NH-Medium) eine mögliche Ursache für die geringen Unterschiede in den Untersuchungen der Expression von STC-1 in den MSZ von AML-Patienten und gesunden Kontrollen sowie die fehlenden Auswirkungen von STC-1 in der Ko-Kultur mit MSZ darstellen. Da sich bei einer im Vorfeld durchgeführten Genchipanalyse von MSZ, die in dem Nährmedium DMEM kultiviert worden waren, eine deutliche Überexpression von STC-1 bei AML-Patienten zeigte, wäre neben einer Erhöhung der Stichprobengröße die Wiederholung der Experimente dieser Dissertation mit dem Nährmedium DMEM ein sinnvoller Ansatz, um die Rolle des von den MSZ sezernierten Proteins STC-1 in der Pathogenese der AML weiter abklären zu können.

The bone marrow stem cell niche plays a central role in the regulation of haematopoiesis. Multipotent mesenchymal stromal cells (MSC) were identified as an important cell population in the niche. By direct cell-cell contact and the secretion of soluble factors MSC are essential for the regulation and maintenance of haematopoiesis in the bone marrow. On the other hand, there is also data suggesting that alterations in MSC may be able to induce malignant transformation of hematopoietic cells and therefore contribute to the initiation of bone marrow diseases such as acute myeloid leukemia (AML).
In a microarray assay of MSC cultured in standard DMEM media we identified Stanniocalcin 1 (STC-1) as the gene with the highest overexpression in leukemic MSC compared to MSC of healthy bone marrow donors. Based on that, the aim of this dissertation was to verify the overexpression of STC-1 in leukemic MSC and to analyse possible effects of STC-1 on the proliferation and apoptosis of AML blasts.
After isolation and expansion of MSC from AML patients and healthy bone marrow donors we proved the ISCT-criteria for MSC: We showed that the MSC are plastic-adherent when maintained in standard culture conditions, express CD105, CD29, CD73, CD44 and CD90, lack expression of CD45 and CD34 surface molecules and are able to differentiate to chondrocytes, osteoblasts and adipocytes. STC-1 expression in leukemic and healthy MSCs was assayed by quantitative real-time polymerase chain reaction (qRT-PCR), western blot and enzyme-linked Immunosorbent Assay (ELISA). To compare STC-1 expression in the bone marrow of AML patients and healthy patients bone marrow biopsies were examined by immunohistochemistry. Similarly, the intracellular distribution of STC-1 in the MSC was visualized through immunohistochemical staining of MSC-cytospins with an anti-STC-1 antibody. Subsequently, apoptosis and proliferation of AML blasts was analysed by performing in vitro monocultures of AML blasts with recombinant human STC-1 (rhSTC-1) and cocultures of AML blasts and MSC from both healthy bone marrow donors and AML patients with and without an additional anti-STC-1 antibody.
Significantly higher levels of STC-1 were detected in the bone marrow biopsies of AML patients. Also, secreted STC-1 levels were significantly higher in leukemic MSCs. Surprisingly, western blot and qRT-PCR assays of MSC cell lysates indicated that the levels of STC-1 expression are not significantly different in leukemic MSC and MSC from healthy bone marrow donors. Also, rhSTC-1 did not change proliferation and apoptosis of AML blasts and in the coculture of MSC and primary AML cells the addition of an STC-1 antibody had no effects on proliferation and apoptosis of the AML blasts. Due to this surprising results, a second microarray assay of leukemic and healthy MSC was performed, this time MSC cultured in NH cell culture media, the MSC-specific culture media we used for all experiments except the initial microarray analyse, were examined. In contrast to the results of the initial microarray assay there was no significant overexpression of STC-1 in the leukemic MSC.
In the literature there is conflicting data concerning the role of STC-1 in tumour initiation and progression. On the one hand it has been shown that STC-1 can decrease apoptosis and enhance proliferation of malignant cells in the coculture with MSC, on the other hand there are studies proving that STC-1 can increase apoptosis of tumour cells. The results of this dissertation could not clarify definitely if and to what extent STC-1 is involved in the initiation and progression of AML. The discrepancy between our results and existing data concerning the role of STC-1 may be explained in part by a different experimental set-up. For example, in the mentioned studies cells from permanent tumour-cell lines were incubated under hypoxia or incubated with chemotherapeutic agents before culture whereas in our experiments we used primary, untreated AML cells. Also there might be specific factors in the cell culture media which could explain discrepancies in the expression levels of STC-1 and the lacking effects of STC-1 on proliferation and apoptosis of AML cells.
Since the initially performed microarray analysis of MSC cultured in standard DMEM cell culture media indicated a significant overexpression of STC-1 in leukemic MSC it then could be a reasonable approach to repeat the experiments of this dissertation with MSC cultured in DMEM cell culture media.
