Introduction and Objectives
The incidence of permanent pacemaker implantation (PPI) and new conduction abnormalities (CA) with the ACURATE neo (SYMETIS SA, Eclubens, Switzerland) has not been studied in detail. Thus, we aimed to analyze predictors of PPI and new CA with this valve, evaluating patient and device related factors, such as implantation depth and device to annulus ratio (DAR).
Methods
Two analyses of a multicentre population were performed: new PPI in all patients without prior pacemaker (n=283), and PPI/new-CA in patients without prior CA or pacemaker (n=232).
Results
New PPI was required in 9.9% of patients, which had a higher BMI, higher rate of right bundle branch block (RBBB) and bradycardia. Neither prosthesis implantation depth nor DAR differed in patients with PPI compared to those without (7.1±2.0 vs. 7.1±1.5mm; p=0.8 and 42±8.3 vs. 42±10.0%; p=0.8). In the multivariable analysis neither DAR (OR 1.010, 95% CI [0.967-1.055]; p=0.7), nor implantation depth (OR 0.972, 95% CI [0.743-1.272]; p=0.8) predicted need for PPI. Only, high BMI, bradycardia and RBBB persisted as independent predictors.
PPI/new-onset CA occurred in 22.8% of patients and was associated with a higher logistic EuroScore. Neither implantation depth nor DAR differed in patients with PPI/new-CA compared to those without (7.3±1.9 vs. 7.1±1.5mm; p=0.6 and 41.0±7.9 vs. 42.2±10.1%; p=0.4).
The only predictor of PPI/new-CA was a higher logistic EuroScore (OR 1.039, 95% CI [1.008-1.071]; p=0.013).
Conclusions
Rates of new PPI and new-onset CA were low with the ACURATE neo. These are mainly influenced by patient characteristics and not by device-depending factors, such as DAR or implantation depth.

Einleitung und Ziele
Die Inzidenz von Schrittmacherimplantation (SMI) und neue Reizleitungsstörungen (RLS) mit der ACURAE neo (SYMETIS SA, Eclubens, Schweiz) Transkatheter-Klappenprothesen (TKP) ist noch nicht im Detail analysiert worden. Ziel dieser Arbeit ist die Analyse von Prädiktoren von SMI und neuen RLS mit dieser TKP mit Fokus auf Patienten Charakteristika sowie Implantationstechnik wie Implantationstiefe und Prothesen zu Anulus Verhältnis (PAV).
Methoden
Es erfolgten zwei Analysen einer multizentrischen Population: neue SMI wurde in allen Patienten ohne vorherigen Schrittmacher (n=283) festgestellt, der kombinierte Endpunkt SMI/neue RLS wurden in Patienten ohne vorherigen Schrittmacher und ohne vorbestehende Reizleitungsstörung (n=232) untersucht.
Ergebnisse
Eine neue SMI wurde in 9.9% der Patienten notwendig, diese zeigten einen höheren BMI, höhere Raten von kompletten Rechtsschenkelblock (RSB) und Bradykardie. Weder Implantationstiefe noch PAV unterschieden sich in Patienten mit SMI im Vergleich zu Patienten ohne SMI (7.1±2.0 vs. 7.1±1.5mm; p=0.8 und 42±8.3 vs. 42±10.0%; p=0.8). In einer multivariaten Analyse konnten weder Implantationstiefe (OR 0.972, 95% KI [0.743-1.272]; p=0.8) noch das PAV (OR 1.010, 95% KI [0.967-1.055]; p=0.7) eine SMI vorhersagen. Nur hoher BMI, vorbestehende Bradykardie und RSB konnten als unabhängige Prädiktoren für eine SMI identifiziert werden.

SMI/neue RLS traten in 22,8% der Fälle auf und präsentierten einen höheren logistischen EuroScore.
Weder Implantationstiefe (7.3±1.9 vs. 7.1±1.5mm; p=0.6) noch das PAV (41.0±7.9 vs. 42.2±10.1%; p=0.4) unterschieden sich in Patienten mit SMI/neue RLS im Vergleich zu Patienten ohne den kombinierten Endpunkt.
In einer multivariaten Analyse blieb nur der logistische EuroScore (OR, 1.039; 95% KI [1.008-1.071]; p = 0,013) als unabhängiger Prädiktor für SMI/neue RLS bestehen.
Zusammenfassung
Schrittmacherimplantation und neu aufgetretene Reizleitungsstörungen mit der ACURATE neo TKP sind niedrig. In einer umfassenden Analyse haben wir festgestellt, dass der Bedarf an SMI und das Auftreten neuer RLS hauptsächlich von patientenbezogenen Eigenschaften und nicht von Faktoren wie dem PAV oder der Implantationstiefe beeinflusst wird.
