Sprache als komplexes System zum Austausch und zur Übertragung von Informationen basiert meist auf der Aktivierung unilateraler Hirnareale der linken Hemisphäre. Nach tumorbedingter Schädigung dieser Regionen reagiert das Gehirn gemäß zahlreicher Studienergebnisse mit Reorganisationsprozessen. Es gibt Hinweise in der Literatur, dass diese vornehmlich in der kontralateralen Hemisphäre liegen und je nach Tumorlokalisation zu kompensatorischen oder dysfunktionalen Prozessen führen (Thiel et al. 2006). Daneben scheinen auf neurokognitiver Ebene molekulare Marker einen Einfluss auf Sprachprozesse zu nehmen (Wefel et al. 2016). Ziel der vorliegenden Arbeit war es, mittels funktioneller Magnetresonanztomographie den Einfluss von Tumorlokalisation und molekularen Markern auf neuroplastische Prozesse zu analysieren.
Hierzu wurde der regionale Blutfluss des Gehirns als Marker der neuronalen Aktivität mit Hilfe der fMRT untersucht. Während der fMRT Messung bearbeiteten 62 Patienten und 34 Kontrollprobanden ein Sprachparadigma und eine Kontrollbedingung. Die Daten wurden im Anschluss mit der Software Statistical Parametric Mapping ausgewertet und in Gruppenanalysen mittels ein- und zweiseitiger T-Tests verglichen. Hierzu wurden die Patienten basierend auf der Tumorlokalisation in eine frontale, eine anterior-temporale und eine anterior-posteriore Gruppe unterteilt. Anschließend wurden Gruppen mit folgenden molekularen Markern verglichen: Mutation der Isocitratdehydrogenase 1 (IDH1), Loss of Heterozygosity (LOH) von 1p/19q und Methylierung der Methylguanin-Methyltransferase (MGMT).
Die Ergebnisse der vorliegenden Arbeit zeigten bei posterioren Hirntumorpatienten in der rechten Hemisphäre signifikant mehr Aktivität als bei den Gesunden. Darüber hinaus fanden sich Umstrukturierungsprozesse innerhalb läsionsferner Areale der linken Hemisphäre. Diese kontralateralen und periläsionalen Aktivitätsverschiebungen lagen vornehmlich im Gyrus angularis und supramarginalis.
Auch bei neuroplastischen Prozessen bezüglich der Isocitratdehydrogenase 1 spielt die Tumorlokalisation eine enorme Rolle. So fanden sich bei posterioren Tumorpatienten mit IDH1-Mutation sowohl kontralateral, als auch periläsional vermehrte Aktivierungen im Kontrast zur Gruppe mit IDH1-Wildtyp. Demgegenüber wurden beim Vergleich der Patienten mit Tumor im frontalen Sprachareal mit und ohne IDH1-Mutation vermehrte Erregungen beim IDH1-negativen Kollektiv errechnet.
Beim Vergleich der Patienten bezüglich der molekularen Marker LOH und MGMT wurden Mehraktivierungen jeweils derjenigen ohne Mutation visualisiert. In der Analyse hinsichtlich der MGMT-Methylierung lagen die detektierten neuroplastischen Veränderungen ausschließlich im Gyrus angularis. Die besondere Stellung dieses Hirnareals für neuronale Reorganisationsprozesse wurde durch die Korrelationsanalyse unterstrichen, welche im linken Gyrus angularis eine Abnahme kortikaler Aktivität bei stärkerer Methylierung zeigte.
Die Ergebnisse dieser Untersuchung liefern Hinweise, dass das Gehirn auf infiltratives Tumorwachstum mit Umstrukturierungsprozessen in periläsionalen und kontralateralen Arealen reagiert. Diese Aktivitätsverlagerungen sind sowohl von der Tumorlokalisation als auch der Ausprägung molekularer Marker abhängig.

Translation of the abstract (English)

Lesions caused by brain tumors in eloquent brain areas may trigger functional reorganization of language processing. A common finding is enhanced language-associated activation in contralesional homolog brain structures. There are indications in literature that contralesional activation might reflect a compensatory or dysfunctional process depending on the tumor localization (Thiel et al. 2006). The reasons for this are still unclear but certainly must be caused by processes in the microstructure. Moreover, there is the finding that patients with isocitrate dehydrogenase 1 (IDH1) wild type show reduced neurocognitive function compared with those with IDH1 mutation (Wefel et al. 2016). This might also be caused by brain activation patterns. Therefore, this study investigates a possible correlation of molecular markers and neural plasticity.
Functional magnetic resonance imaging (fMRI) measures brain activity by detecting regional cerebral blood flow changes. For the presented study, a sample of 62 patients and 34 healthy controls were tested on fMRI using a language paradigm. FMRI data was analyzed using the specific software SPM and different groups were analyzed via one and two-tailed T-tests. Therefore the patients were divided based on the location of the tumor, resulting in a frontal, an anterior-temporal and a posterior-temporal group. The groups were further divided by the presence of the following molecular markers: isocitrate dehydrogenase 1 (IDH1) mutation, loss of heterozygosity (LOH) 1p/19q and methylguanin methyltransferase (MGMT) methylation. Those markers are known to be involved in speech processes on a neural level.
The results of the present work show significant more activation in the right hemisphere in the posterior group in comparison to healthy controls. Moreover increased activation was found also in the left hemisphere. The contralateral and perilesional activity shift was principally located in the gyrus angularis and supramarginalis.
The localization of the tumor also plays an immense role on the neural plasticity concerning IDH1. More activation was found in the posterior group with IDH1 mutation in comparison to the subgroup without this mutation. In contrast, for the frontal group more activation was found in the subgroup without IDH1 mutation in comparison to the subgroup with this mutation.
Moreover, more neural activation could be seen in patients without LOH or MGMT in comparison to those with these mutations. The activations were only found in the gyrus angularis. The MGMT- methylation and the neural activation show also a negative dose-response relationship in the left gyrus angularis.
In summary, the findings suggest that the brain reacts on destructive tumor growth with neural plasticity in perilesional and contralateral areas. It is also suggested that the neural plasticity depends on the location of the tumor and the expression of molecular markers.
