Patienten mit schwerer respiratorischer Insuffizienz werden häufiger mit veno-venöser extrakorporaler Membranoxygenierung (vv-ECMO) behandelt. Bei diesen Patienten besteht aufgrund komplexer, multifaktorieller Gerinnungsstörungen, die aus der extrakorporalen Zirkulation resultieren, ein erhöhtes Risiko für Blutungskomplikationen. Diese Studie umfasste die retrospektive Analyse von prospektiv erhobenen Daten zur Evaluation der Inzidenz und Konsequenz thorakaler Blutungskomplikationen sowie deren Auswirkungen auf die Morbidität und Letalität aller Patienten, die zwischen Dezember 2010 und Dezember 2016 eine vv-ECMO benötigten. Thorakale Blutungskomplikationen wurden bei 9,6% der Patienten beobachtet und stellten damit die häufigste Blutungskomplikation während der vv-ECMO-Behandlung dar. Bei fast 60% dieser Patienten war eine operative Therapie erforderlich. Da die Mortalität bei diesen Patienten signifikant erhöht ist, sollte die vv-ECMO nur in erfahrenen Zentren mit thoraxchirurgischer Expertise erfolgen.

Patients with severe respiratory insufficiency are more frequently treated with veno-venous extracorporeal membrane oxygenation (vv-ECMO). These patients are at increased risk of bleeding complications due to complex, multifactorial coagulation disorders resulting from extracorporeal circulation. This study involved the retrospective analysis of prospectively collected data to evaluate the incidence and consequence of thoracic bleeding complications and their impact on the morbidity and mortality of all patients requiring vv-ECMO between December 2010 and December 2016. Thoracic bleeding complications were observed in 9.6% of patients, representing the most common bleeding complication during vv-ECMO treatment. Surgical therapy was required in almost 60% of these patients. As mortality is significantly increased in these patients, vv-ECMO should only be performed in experienced centers with thoracic surgery expertise.
