In der hier vorliegenden Studie wurde an 11 550 Patienten mit Rektumkarzinom im UICC Stadium III, deren Daten von Januar 2002 bis Dezember 2011 in klinischen Krebsregistern Deutschlands erfasst wurden, die prognostische Bedeutung des Lymphknotenquotienten im Hinblick auf das Gesamtüberleben und rezidivfreie Überleben evaluiert. Abstract (German)
In der hier vorliegenden Studie wurde an 11 550 Patienten mit Rektumkarzinom im UICC Stadium III, deren Daten von Januar 2002 bis Dezember 2011 in klinischen Krebsregistern Deutschlands erfasst wurden, die prognostische Bedeutung des Lymphknotenquotienten im Hinblick auf das Gesamtüberleben und rezidivfreie Überleben evaluiert. Der Lymphknotenquotient definiert das Verhältnis von tumorinfiltrierten zu insgesamt entnommenen Lymphknoten.
Bei 9 870 (85,5%) Patienten ergab sich, entsprechend der Empfehlung der International Union Against Cancer und vom American Joint Committee on Cancer, eine adäquate Lymphadenektomie mit Entnahme von mindestens 12 Lymphknoten. Nur bei 1 680 (14,5%) wurden weniger als die geforderten 12 Lymphknoten reseziert, oder es fand sich keine Angabe zur exakten Anzahl der entnommenen Lymphknoten. Die berechneten Cut-Off-Werte des Lymphknotenquotienten ergaben vier prognostisch bedeutsame Gruppen (Gruppe eins: <0,100; Gruppe zwei: 0,100 - 0,199; Gruppe drei: 0,200 - 0,399; Gruppe vier: >0,400), anhand derer die Patienten entsprechend ihres Lymphknotenquotienten eingeteilt wurden.
Die multivariablen Analysen, in welchen der Lymphknotenquotient in Abhängigkeit verschiedenerer klinischer und histopathologischer Angaben gesetzt wurde, ergab eine statistisch signifikante positive Assoziation zwischen hohem Lymphknotenquotienten und höherem Grading, bzw. größeren Tumoren. Bezüglich der Patientencharakteristika Alter, Geschlecht und Lokalisation des Tumors (Rektumdrittel) ergab sich hingegen kein statistisch signifikanter Unterschied.
In der neunjährigen Nachbeobachtungszeit sind 4 243 (36,7%) Patienten verstorben. Für das komplette Auswertekollektiv von 11 550 Patienten ergab sich ein medianes Gesamtüberleben von 5,2 Jahren. In den Überlebensanalysen nach Kaplan-Meier ergab sich für das Gesamtüberleben mit steigendem Lymphknotenquotienten ein statistisch signifikant geringeres Überleben... Auch nach Risikoadjustierung mit Hilfe der multivariablen Cox-Regression unter Einbeziehen weiterer Prognosefaktoren wie Diagnosealter, Grading des Tumors und Tumorgröße, ergab sich eine deutliche und signifikante Zunahme der Sterberate mit zunehmendem Lymphknotenquotienten. Gegenüber der Gruppe mit einem Lymphknotenquotienten 0,100 betrug die Hazard Ratio (HR) für die Gruppe mit Lymphknotenquotient 0,100-0,199 1,204 (95%-KI 1,104-1,314), für die Gruppe 0,200-0,399 betrug sie 1,327 (95%-KI 1,218-1,446) und für die Gruppe 0,400 2,051 (95%-KI 1,884-2,233)..
Die Rezidivhäufigkeit innerhalb der neunjährigen Nachbeobachtungszeit ergab, dass 3 037 (26,3%) Patienten ein Rezidiv entwickelten, wobei es sich in 2 579 (22,3%) Fällen um ein Fernmetastasenrezidiv handelte, in 833 (7,2%) Fällen um ein lokoregionäres Rezidiv und 511 (4,43%). Patienten entwickelten sowohl das eine, wie auch das andere. Je größer der Lymphknotenquotient, umso größer war auch das Risiko für ein Rezidiv. Die kumulative Rezidivrate stieg mit steigendem Lymphknotenquotienten, was sich auch aufgeteilt in lokoregionäres Rezidiv und Fernmetastasenrezidiv (p<0,001), und dem rezidivfreien Überleben bestätigte.
Der Lymphknotenquotient ermöglicht eine sensitive Beurteilung bezüglich der Qualität der Lymphadenektomie und gleichzeitig die Einteilung der Patienten in prognostisch bedeutsame Risikogruppen. Er sollte deshalb standardmäßig Eingang finden in die Dokumentation von Patienten mit Rektumkarzinom.
Die Validierung dieser Ergebnisse sollte unter Berücksichtigung weiterer Therapiemodalitäten, wie insbesondere der neoadjuvanten Therapie, welche in den Datensätze für diese Studie nicht ausreichend vorhanden war, erfolgen. Der Lymphknotenquotient sollte routinemäßig in den histopathologischen Befund zum Rektumkarzinom aufgenommen werden, da er eine prognosebestimmende Einteilung der Patienten in Gruppen erlaubt und zukünftig die Therapieentscheidung mit beeinflussen kann.

This study 11550 patients with colorectal cancer in UICC stage III, whose data were collected from January 2002 to December 2011 from clinical cancer registries in Germany, the prognostic significance of the lymph node ratio (LNR) in terms of overall survival (OS) and recurrence-free survival (RFS) was evaluated. The LNR is the ratio of infested to examined lymph nodes (LN).
In 9870 (85,5%) patients an adequate lymphadenectomy with at least twelve examined lymph nodes was performed, according to the recommendations of the International Union Against Cancer and the American Joint Committee on Cancer. Only in 1680 patients (14,5%) less than the required 12 LN were resected, or there was no information about the exact number of LN examined. The calculated cut-off values of the LNR resulted in 4 prognostically significant groups (group 1: <0,100; group 2: 0,100-0,199; group 3: 0,200- 0,399; group 4: >0,400) by which the patients were classified according to their LNR.
In the multivariable analyses, in which the LNR was set as a function of various clinical and histopathological data, a statistically significant positive association was found between high LNR and higher grade or larger tumors. There was no statistically significant difference in patient characteristics such as age, gender and tumor location.
In the nine year follow-up period 4243 (36,7%) patients died. For the entire group of 11550 patients a median OS of 5.2 years was found. The survival analyses according to Kaplan-Meier showed a statistically significant decrease in survival with increasing LNR. Even after risk adjustment using the multivariable Cox regression, taking into account other prognostic factors such as age of diagnosis, tumor grading and tumor size, there was a clear and significant increase in the survival rate with increasing LNR. Compared to the group with an LNR <0.100, the HR for the group with LNR 0.100-0.199 was 1.204 (95%-KI 1.104-1.314) , for the group 0.200-0.399 it was 1.327 (95%-KI 1.218-1.1446) and for the group >0.400 2.051 (95%-KI 1.884-2.233).
The recurrence frequency within the nine-year follow-up period showed that 3,037 (26.3%) patients developed a recurrence, of which 2,579 (22.3%) were distant metastatic recurrences, 833 (7.2%) were locoregional recurrences and 511 (4.43%) patients developed both one and the other. The higher the LNR, the higher the risk of recurrences was, which was shown by the hazard ratio (HR). The cumulative recurrence rate increased with increasing LNR, which was also confirmed in locoregional and distant metastatic recurrence (p<0.001) and for recurrence-free survival.
The LNR allows a sensitive assessment of the quality of the lymphadenectomy and at the same time the classification of patients into prognostically significant risk groups. It should therefore be included as standard in the documentation of patients with rectal cancer.
The validation of these results should take into account other therapeutic modalities, in particular neoadjuvant therapy, which was not sufficiently available in the data sets for this study. The LNR should be routinely included in the histopathological findings for rectal cancer, as it allows a prognosis-determining division into groups and may influence the therapy decision in the future.
