Bei Patienten mit Diabetes mellitus Typ 2 (DM2) ist eine schlafbezogene Atmungsstörung (SBAS) eine häufige Komorbidität. Zugleich ist ein DM2 eng mit dem metabolischen Syndrom (MS) vergesellschaftet. Das Ziel der vorliegenden Arbeit war es, zu untersuchen, ob eine unabhängige Assoziation zwischen einer SBAS und dem MS sowie dessen Komponenten in einem Kollektiv aus Patienten mit DM2 vorliegt.
Methoden: 679 Teilnehmer der DIACORE-SDB Sub-Studie, einer Kohortenstudie aus Patienten mit DM2, wurden auf das Vorliegen eines MS untersucht. Nach den Kriterien des National Cholesterol Education Program (NCEP) lag ein MS vor, wenn mindestens drei der folgenden fünf Kriterien erfüllt waren: Taillenumfang >102 cm (Männer) / >88 cm (Frauen), Nüchterntriglyzeride >150 mg/dl, High-Density-Lipoprotein (HDL)-Cholesterin <40 mg/dl (Männer) / <50 mg/dl (Frauen), Blutdruck ≥130/85 mmHg und eine Nüchternglukose ≥110 mg/dl. Der Apnoe-Hypopnoe-Index (AHI) wurde im Rahmen eines ambulanten Monitorings für eine SBAS erhoben. Anhand des AHI erfolgte die Definition der Schweregrade einer SBAS (AHI<15,0: keine/milde SBAS; AHI 15,0-29,9: moderate SBAS; AHI ≥30,0: schwere SBAS).
Ergebnisse: 228 (34%) der 679 Patienten (mittleres Alter 66 Jahre, mittlerer Body-Mass-Index [BMI] 31.2 kg/m², mittlerer AHI 14/h) wiesen eine SBAS auf. Die Häufigkeit eines MS stieg mit zunehmenden Schweregrad einer SBAS signifikant an (keine/milde SBAS vs. moderate SBAS vs. schwere SBAS: 72% vs. 79% vs. 85 %, p=0,038). In einer logistischen Regressionsanalyse zeigte sich nach Adjustierung für Geschlecht, Alter, Adipositas (BMI ≥30kg/m²) und den HOMA-Index eine signifikante Assoziation zwischen dem AHI und der Präsenz eines MS (OR[95%CI]=1,039[1,011; 1,069]; p=0,007). Zudem stellte sich eine signifikante und unabhängige Assoziation zwischen dem AHI und den NCEP-Kriterien viszerale Adipositas (OR[95%CI]=1,031[1,006; 1,056]; p=0,014), Hypertonie (OR[95% CI]=1,049[1,000; 1,100]; p=0,048) und Hypertriglyzeridämie (OR[95% CI]=1,018[1,002; 1,035]; p=0,029) dar.
Fazit: In einem Kollektiv aus Patienten mit DM2 war eine SBAS signifikant und unabhängig von bekannten Modulatoren mit einem MS assoziiert.

Metabolic syndrome (MS) and sleep-disordered breathing (SDB) are highly prevalent in patients with diabetes mellitus type 2 (DM2). The present study examined whether there is an independent association between SDB and MS in a sample of outpatients with DM2.
Methods: MS was determined in 679 patients of the DIACORE-SDB sub-study, a study of outpatients with DM2. According to the National Cholesterol Education Program (NCEP) criteria, MS is defined by at least three of the following five criteria: waist circumference of >102 cm (men) / >88 cm (women), blood pressure of ≥130/85 mmHg, a fasting triglyceride level of >150 mg/dl, high density lipoprotein (HDL) of <40 mg/dl (men) / <50 mg/dl (women) and a fasting glucose level of ≥110 mg/dl. The apnea-hypopnea-index (AHI) was assessed with an ambulatory monitoring device and used to define the severity of SDB (AHI <15.0: no/mild SDB; AHI 15.0-29.9: moderate SDB; AHI ≥30.0: severe SDB).
Results: 228 (34%) of the 679 participants (mean age 66 years, mean body-mass-index [BMI] 31.2 kg/m², mean AHI 14/hour) had SDB. MS was significantly more frequent in patients with more severe SDB (no/mild SDB vs. moderate SDB vs. severe SDB: 72% vs. 79% vs. 85 %, p=0.038). Logistic regression analysis adjusted for sex, age, obesity (BMI ≥30kg/m²) and the HOMA-index showed a significant association between the AHI and the presence of MS (OR [95% CI] =1.039[1.011; 1.069]; p=0.007). Furthermore, the AHI was significantly and independently associated with several components of MS: elevated waist circumference (OR[95%CI]=1.031[1.006;1.056]; p=0.014), hypertension (OR[95% CI]=1.049[1.000;1.100]; p=0.048) and hypertriglyceridemia (OR[95%CI]=1.018[1.002;1.035];p=0.029).
Conclusion: SDB is significantly and independently associated with MS in outpatients with DM2.
