Abstract (German)
Ziel: Ziel der vorliegenden Arbeit war, die Auswirkungen von Etomidate auf das Outcome von polytraumatisierten Patienten zu untersuchen.
Methoden: Es handelt sich um eine retrospektive Studie am Universitätsklinikum Regensburg. Es wurden im Zeitraum vom 01.09.2007 bis zum 31.12.2014 am Universitätsklinikum Regensburg prospektiv Daten zur Versorgung und zum Outcome von Polytraumapatienten erhoben. Nach Auswahl der Studienpopulation wurden zwei Gruppen gebildet (Patienten mit Etomidate vs. Patienten ohne Etomidate) und die beiden Gruppen hinsichtlich demographischer Zusammensetzung, Verletzungsmuster, bestimmter anatomischer und physiologischer Parameter, der Therapie präklinisch, im Schockraum sowie im weiteren Krankenhausaufenthalt, verglichen. Anschließend fand ein Vergleich der beiden Gruppen bezüglich möglicher Komplikationen und Outcomeparameter statt.
Ergebnisse: Die Studienpopulation umfasst 387 Patienten, hierbei 271 in der Kontrollgruppe und 116 in der Etomidategruppe. Bezüglich der demographischen Faktoren, des Verletzungsmusters, der anatomischen und physiologischen Scores bestand kein Unterschied zwischen den Patienten. Auch bei den Komplikationen und dem Outcome zeigte sich kein Unterschied. Die Länge des Krankenhausaufenthaltes (21,65 +/-16,68 vs. 22,5 +/-18,1, p=0,795), die Beatmungsdauer (in Tagen, 7,66 +/- 8,15 vs. 9,57 +/- 11,81, p=0,214) und die Verweildauer auf der Intensivstation (13,15 +/- 12,19 vs. 12,72 +/- 12,97, p=0,566) unterschieden sich nicht. Die Rate an Pneumonien war in den beiden Gruppen ebenfalls gleich (43,3% vs. 40,0%, p=0,502), ebenso bei der Sepsis (9,6% vs 12,4%, p=0,464). Die Mortalität lag bei 18,5 % respektive 19,0% (p=0,910). Auffällig ist eine niedrige SMR (0,72 vs. 0,74).
Schlussfolgerung: In der vorliegenden Studie konnte kein Effekt von Etomidate auf das Outcome von polytraumatisierten Patienten nachgewiesen werden.

Purpose: The objective of this study is to examine the effect of etomidate on the outcome of multiple injured patients.
Methods: The presented study is a retrospective study at the university hospital of Regensburg. The data about the treatment and outcome of severely injured patients were prospectively collected at the university hospital of Regensburg from the 1st of September 2007 until the 31st of December 2014. After the selection of the study sample, two groups were formed (patients with Etomidate vs. patients without Etomidate) and compared for demographics, injury pattern, certain anatomic and physiological parameters, pre-hospital, and in-hospital treatment. The two groups were then compared for complications and outcome.
Results: The study population included 387 patients; 271 in the control group, and 116 in the etomidate group. No difference was found in the patients’ demographics, injury pattern, and in the anatomical and physiological scores. Neither the complications or the outcome showed any difference. The length of stay in hospital (21,65 +/-16,68 vs. 22,5 +/-18,1, p=0,795), the length of intubation (in days 7,66 +/- 8,15 vs. 9,57 +/- 11,81, p=0,214) and the length of ICU stay (13,15 +/- 12,19 vs. 12,72 +/- 12,97, p=0,566) did not differ significantly. The rate of pneumonia (43,3% vs. 40,0%, p=0,502) showed no difference as well as the incidence of sepsis in the groups (9,6% vs 12,4%, p=0,464). The mortality was 18,5 % respectively 19,0% with a p=0,910. A remarkably low SMR of 0,72 vs. 0,74 was found.
Conclusion: No effect of etomidate on the outcome of multiple injured patients could be found in this study.
