Abstract (German)
Die allogene SZT bleibt v.a. für Patienten mit malignen hämatologischen Erkrankungen oftmals die einzige kurative Therapieoption. Dabei sind reife T-Zellen des Spenders für die Bekämpfung der Neoplasie von zentraler Bedeutung (GVL-Effekt). Allerdings können alloreaktive Spender-T-Zellen zeitgleich auch gesundes Spendergewebe angreifen und zerstören (GVHD). Neben den klassischen Zielgeweben wird auch das supportive Stroma primärer lymphatischer Organe (Thymus und Knochenmark) immunologisch geschädigt. Dies führt sowohl beim Menschen als auch in Mausmodell zu einer verzögerten Erholung des adaptiven Immunsystems, wobei die B-Zell-Rekonstitution in besonderem Maße kompromittiert ist. Wir konnten zeigen, dass eine MHC-differente (C57BL/6 → BALB/c, H-2b → H-2d) KMT alleine bereits zu einem deutlichen Rückgang oligopotenter (CLP 1) und zu einem geringeren Anteil auch B-Zell-restringierter (CLP 2), lymphatischer Vorläuferzellen im KM führt. Dies ist auf einen gesteigerten Umsatz im Rahmen der normalen Rekonstitution nach KMT zurückzuführen, da in der GVHD-freien Kontrollgruppe sogar ein erhöhter B-Zellgehalt im KM zu finden war. Wurde allerdings zusätzlich eine GVHD ausgelöst, so sank der CLP 2-Zellgehalt bei gleichzeitig vorliegender B-Zell-Aplasie noch tiefer ab. Dies bestätigt unsere Vermutung, dass eine Knochenmark-GVHD die B-Zell-Lymphopoese vor Stufe der CLP 2 durch Schädigung der B-Zell-Nische blockiert.
Treg spielen sowohl unter physiologischen Bedingungen als auch nach KMT eine zentrale Rolle bei der Aufrechterhaltung der Immunhomöostase. Sie können das Stroma lymphatischer Organe vor einer Schädigung durch eine akute GVHD schützen und ermöglichen dadurch eine vollständige B-Zell-Rekonstitution. Andererseits führt das Fehlen von Treg, z.B. in FoxP3-defizienten Scurfy Mäusen, zu einer deutlich veränderten B-Zell-Entwicklung. Mithilfe eines Treg-Ablationsmodells untersuchten wir deshalb, welche Rolle Treg für die B-Zell-Lymphopoese ohne eine KMT spielen. Unter Verwendung des Foxp3-DTR Modells konnten wir zeigen, dass die Inflammation nach Treg-Depletion auch unabhängig von der KMT zur Störung der B-Zell-Lymphopoese im KM führt. Dies ist vermutlich auf eine inflammatorische Zerstörung nischenspezifischer KM-Stromazellen durch autoreaktive Effektor-T-Zellen zurückzuführen (vergleichbar mit der Inflammation durch alloreaktive T-Zellen in der GVHD).
Um zu untersuchen, ob die Schädigung des supportiven Stromas lymphatischer Organe oder die Störung der Immunhämostase auch Teil des GVL-Effekts gegen
B-Zell-Neoplasien sind, wurde ein haploidentes GVHD/GVL-Transplantationsmodell (BALB/c → CB6F1 oder C57BL/6 → CB6F1) entwickelt. Direkte GVL-Effekte über MHC-Differenzen sind nur in der Konstellation C57BL/6 → CB6F1 (H-2b → H-2bd) möglich, da die eingesetzten B-Zell-Lymphome A20-luc/yfp/neo und Bcl1-gfp/luc aus BALB/c (H-2d) Mäusen stammen. Bei der Konstellation BALB/c → CB6F1 (H-2d →
H-2bd) kann eine Bekämpfung der Tumorzellen nur über indirekte GVL-Effekte erklärt werden. Wir konnten zeigen, dass in der Konstellation BALB/c → CB6F1 eine
GVHD-Induktion am Tag der KMT die B-Zell-Regeneration verhindert und das Tumorzellwachstum für beide B-Zell-Lymphome deutlich verzögert.
Bei fortgeschrittener Erkrankung scheinen diese indirekten anti-B-Zell-Effekte aber nicht therapeutisch wirksam zu sein, da eine verzögerte DLI-Therapie an Tag 16 nach haploidenter KMT nur bei MHC-differenten Spender-T-Zellen wirksam war, während Spender-T-Zellen mit Lymphom-identem MHC unwirksam blieben.

Allogeneic SCT often remains the only curative therapy option, especially for patients with malignant hematologic diseases. In this context, mature donor T-cells are of central importance in combating neoplasia (GVL-effect). However, alloreactive donor T-cells can simultaneously attack and destroy healthy donor tissue (GVHD). In addition to the classic target tissues, the supportive stroma of primary lymphoid organs (thymus and bone marrow (BM)) is also immunologically damaged. This leads to delayed recovery of the adaptive immune system both in humans and in mouse models, with B-cell reconstitution being particularly compromised. We demonstrated that MHC-mismatched (C57BL/6 → BALB/c, H-2b → H-2d) bone marrow transplantation (BMT) alone leads to a marked decrease in oligopotent (CLP 1) and to a lower proportion also of B-cell-restricted (CLP 2) lymphoid progenitor cells in the BM. This can be attributed to increased turnover during normal reconstitution after BMT, as there was even an increased B-cell count in the BM in the GVHD-free control group. However, if GVHD was additionally induced, CLP 2 cell count decreased even more significantly in the presence of concomitant B-cell aplasia. This confirms our assumption that BM-GVHD blocks B-cell lymphopoiesis before CLP 2 stage by damaging the B-cell niche.
Treg play a central role in maintaining immune homeostasis both under physiological conditions and after BMT. They can protect the stroma of lymphoid organs from damage by acute GVHD, thereby allowing complete B-cell reconstitution. On the other hand, the absence of Treg, e.g. in FoxP3-deficient Scurfy mice, results in markedly altered B cell development. Using a Treg ablation model, we therefore investigated the role of Treg in B-cell lymphopoiesis in the absence of BMT. Using the Foxp3-DTR model, we demonstrated that inflammation following Treg depletion also leads to disruption of B-cell lymphopoiesis in the BM independent of BMT. This is presumably due to inflammatory destruction of niche-specific BM stromal cells by autoreactive effector T-cells (comparable to the inflammation by alloreactive T-cells in GVHD).
To investigate whether damage to the supportive stroma of lymphoid organs or disruption of immune hemostasis are also part of the GVL-effect against
B-cell neoplasms, a haploidentical GVHD/GVL transplantation model (BALB/c → CB6F1 or C57BL/6 → CB6F1) was developed. Direct GVL-effects via MHC differences are only possible in the constellation C57BL/6 → CB6F1 (H-2b → H-2bd), because the B-cell lymphomas A20-luc/yfp/neo and Bcl1-gfp/luc used were derived from BALB/c (H-2d) mice. In the constellation of BALB/c → CB6F1 (H-2d → H-2bd), tumor cell targeting can only be explained by indirect GVL-effects. We were able to show that in the constellation BALB/c → CB6F1 GVHD induction on the day of BMT prevents
B-cell regeneration and significantly delays tumor cell growth for both B-cell lymphomas. However, in advanced disease, these indirect anti-B-cell effects do not appear to be therapeutically effective, as delayed DLI therapy on day 16 after haploidentical BMT was effective only for MHC-different donor T-cells, whereas donor T-cells with lymphoma-ident MHC remained ineffective.
