In Deutschland erkranken jedes Jahr mehr als 7000 Frauen am Eierstockkrebs, welches den zweithäufigsten Genitalkrebs der Frau darstellt. Das Ovarialkarzinom hat eine sehr hohe Mortalitätsrate und stellt in der gynäkologischen Onkologie eine große Herausforderung dar. Die Primärdiagnose erfolgt oft erst in fortgeschrittenen Stadien, da effektive Screeningmethoden fehlen. Dadurch verschlechtert sich die Prognose und Mortalität dieser Erkrankung. Auch wenn auf dem Gebiet der primären Therapie des Ovarialkarzinoms in den letzten Jahren Fortschritte verzeichnet werden konnten, rezidivieren etwa zwei Drittel der am Ovarialkarzinom leidenden Patientinnen in den ersten fünf Jahren. Die Rezidive des klassischen Ovarialkarzinoms sind sowohl durch eine schlechte Prognose als auch ein schlechtes Gesamtüberleben gekennzeichnet. Umso wichtiger ist die Verbesserung der operativen und medikamentösen Rezidivtherapie, um sowohl die Lebensdauer nach einem Rezidiv zu erhöhen, als auch die Lebensqualität und die Prognose zu verbessern.
Dieser Untersuchung liegen folgende Hypothesen zugrunde: Patienten mit rezidiviertem Ovarialkarzinom - insbesondere platinsensitive - profitieren von einer leitlinienkonformen Behandlung. Neben der Therapie gibt es wichtige Prognosefaktoren, die das Überleben und das Rezidivrisiko mit beeinflussen. In dieser Arbeit wird untersucht, wie die Leitlinienempfehlungen in der Versorgung von Patientinnen mit rezidiviertem Ovarialkarzinom umgesetzt sind. Weiterhin wird das Gesamt- und rezidivfreie Überleben in Abhängigkeit von der Rezidivtherapie und weiteren Einflussfaktoren beleuchtet.
Die vorliegende Arbeit basiert auf einem Datensatz von ursprünglich 1628 Patientinnen mit der Diagnose bösartige Neubildungen des Ovars (ICD-10-Code: C56) aus dem Bezirk Oberpfalz mit dem Diagnosezeitraum 1998-2013, die am Tumorzentrum Regensburg registriert und dokumentiert wurden. In die Betrachtung eingeschlossen sind nach Histologie 1338 Patienten mit klassischen Ovarialkarzinom, von denen nach Ausschluss derjenigen bei denen die Dokumentation unzureichend ist (n=121) noch 1217 Patienten mit primären Ovarialkarzinom in die Betrachtung eingeschlossen sind. In diesem Kollektiv von 1217 Patientinnen mit klassischen Ovarialkarzinom sind 300 Rezidive aufgetreten. Die Analysen des kumulativen Gesamtüberlebens, der kumulativen Rezidivraten und des rezidivfreien Überlebens werden mit der Kaplan-Meier-Methode umgesetzt. Eine Unterteilung erfolgt bei den Rezidivraten in lokale, lokoregionäre und Fernmetastasenrezidive. Die Ergebnisse der Kaplan-Meier Analysen werden jeweils in 3-, 5- und 10-Jahres-Überlebens- bzw. Rezidivwahrscheinlichkeiten sowie der medianen Überlebenszeit angegeben. Um Unterschiede zwischen den Patientenuntergruppen herauszufinden, wird mittels Log-Rank-Test nach Cox-Mantel auf Signifikanz geprüft. P-Werte ≤ 0,05 werden als signifikant und p-Werte < 0,001 als hochsignifikant bewertet. Zusätzlich werden univariable und multivariable Cox-Regressionsanalysen durchgeführt. In den multivariablen Analysen werden sowohl biologische als auch therapeutische Einflussfaktoren gleichzeitig auf ihre Auswirkung auf die Überlebenswahrscheinlichkeit oder die Rezidivrate untersucht. Als Maßzahl dieser Analysen wird die Hazard Ratio (HR) zum Vergleich der Überlebenszeiten und als Schätzer der durch die Einflussgröße bedingten Sterberisiken angegeben. Die Darstellung der Ergebnisse erfolgt mit 95%-Konfidenzintervall und p-Wert.
Für das beschriebene Patientenkollektiv von 300 rezidivierten Patientinnen mit klassischen Ovarialkarzinom ergab sich ein medianes Gesamtüberleben von 21,6 Monaten mit einem 2-Jahres-Gesamtüberleben von 47,5% und einem 5-Jahresüberleben von 16,25%. Die kumulative 5-Jahres-Rezidivrate betrug 38%, das Risiko ein Rezidiv zu entwickeln steigt mit dem Alter, ein fortgeschrittenes Tumorstadium bei Erstdiagnose sowie eine geringe Differenzierung. In der Kaplan-Meier-Analyse stellte sich in der Rezidiv-Situation das Alter, eine platin- und taxolbasierte Chemotherapie sowie eine kombinierte Therapie bestehend aus Operation gefolgt von einer platinbasierten Chemotherapie als signifikante Prognosefaktoren für das Gesamtüberleben dar.
Der Überlebensvorteil der operierten und mit adjuvanter Chemotherapie behandelten Patienten konnte in der multivariablen Cox-Regressionsanalyse bestätigt werden (HR:0,162; 95%-KI: 0,094-0,279; p<0,001). Selbst eine alleinige Chemotherapie wirkt sich günstig auf das Gesamtüberleben aus (HR:0,430; 95%-KI: 0,256-0,723; p=0,001). Die besten Überlebenszeiten im Rezidiv zeigten im Vergleich zu Nicht-Behandelten in der multivariablen Cox-Regressions-Analyse die Operierten und adjuvant mit Platin und Taxol behandelten Patienten mit einer medianen Gesamtüberlebenszeit von 53,4 Monaten (HR:0,118; 95%-KI: 0,058-0,242; p<0,001). Jedoch auch eine operative Therapie gefolgt von einer alleinigen insbesondere platinhaltigen Chemotherapie wirkt sich günstig auf das Gesamtüberleben aus (medianes OS für Platin mono 34,2 Monate, p<0,001 vs medianes OS 29,6 Monate, p=0,001 für andere Chemo). Unter den im Rezidiv Nicht-Operierten Patienten wirkt sich in der multivariablen Analyse im Rezidiv eine platin- und taxolhaltige Kombinationstherapie am günstigsten auf die Überlebenszeit aus im Vergleich zur Referenzgruppe und denjenigen, die eine Monotherapie mit Platin erhalten haben (medianes OS 23,6 Monate HR:0,326; 95%-KI: 0,158-0,673; p=0,002). Auch eine alleinige Chemotherapie wirkte sich günstig auf das Gesamtüberleben aus im Vergleich zur Referenzgruppe (die keine Therapie erhalten haben), wobei eine platinbasierte Monotherapie einer anderen therapeutischen Substanz überlegen ist (Platin mono vs andere Chemo 16,8 Monate vs 10,8 Monate HR:0,351; 95%-KI: 0,184-0,669; p=0,001).
Wichtiger Teilschritt in der Therapie des rezidivierten Ovarialkarzinom ist eine platin- und taxolhaltige Kombinationschemotherapie, welche einen signifikanten Überlebensvorteil mit sich bringt. Auch eine operative Therapie im Rezidiv gefolgt von einer platin- und taxolhaltigen Chemotherapie erbrachte einen signifikanten Überlebensvorteil. Es werden weitere Studien notwendig sein, um den Nutzen einer operativen Therapie für ein selektives Patientengut zu validieren.

In Germany more than 7000 women develop ovarian cancer every year, which is the second most common genital cancer in women. Ovarian cancer has a very high mortality rate and represents a major challenge in gynecological oncology. The primary diagnosis is often only made at an advanced stage, as there are no effective screening methods. This worsens the prognosis and mortality of this disease. Even if progress has been made in the field of primary therapy for ovarian cancer in recent years, about two-thirds of ovarian cancer patients will relapse in the first five years. The recurrences of classic ovarian cancer are characterized by both a poor prognosis and poor overall survival. This makes it all the more important to improve surgical and medicinal relapse therapy in order to both increase the lifespan after a relapse and to improve the quality of life and the prognosis.
This study is based on the following hypotheses: Patients with relapsed ovarian cancer - especially those sensitive to platinum - benefit from treatment in accordance with the guidelines. In addition to therapy, there are important prognostic factors that influence survival and the risk of recurrence. This thesis examines how the guideline recommendations are implemented in the care of patients with relapsed ovarian cancer. Furthermore, the overall and relapse-free survival is examined depending on the relapse therapy and other influencing factors.
The present work is based on a dataset of originally 1628 patients with the diagnosis of malignant neoplasms of the ovary (ICD-10 code: C56) from the Upper Palatinate district with the diagnosis period 1998-2013, who were registered and documented at the Regensburg Tumor Center. According to histology, 1338 patients with classic ovarian cancer are included in the analysis, of which, after excluding those for whom the documentation is insufficient (n = 121), 1217 patients with primary ovarian cancer are included in the analysis. In this collective of 1217 patients with classic ovarian cancer, 300 recurrences occurred. The analyzes of cumulative overall survival, cumulative recurrence rates and recurrence-free survival are implemented using the Kaplan-Meier method. The recurrence rates are subdivided into local, locoregional and distant metastasis recurrences. The results of the Kaplan-Meier analyzes are given in terms of 3-, 5- and 10-year survival or recurrence probabilities as well as the median survival time. In order to find out differences between the patient subgroups, the Cox-Mantel log-rank test is used to test for significance. P values ≤ 0.05 are rated as significant and p values <0.001 as highly significant. In addition, univariable and multivariable Cox regression analyzes are carried out. In the multivariable analyzes, both biological and therapeutic influencing factors are examined simultaneously with regard to their effect on the probability of survival or the rate of recurrence. The hazard ratio (HR) for comparing survival times and as an estimate of the risk of death caused by the influencing factor is given as a measure of these analyzes. The results are presented with a 95% confidence interval and p-value.
For the patient collective described of 300 relapsed patients with classic ovarian cancer, the median overall survival was 21.6 months with a 2-year overall survival of 47.5% and a 5-year survival of 16.25%. The cumulative 5-year recurrence rate was 38%, the risk of developing a recurrence increases with age, an advanced tumor stage at initial diagnosis and low differentiation. In the Kaplan-Meier analysis, age, platinum- and taxol-based chemotherapy, and combined therapy consisting of surgery followed by platinum-based chemotherapy were significant prognostic factors for overall survival in the relapse situation.
The survival benefit of the operated patients and those treated with adjuvant chemotherapy was confirmed in the multivariable Cox regression analysis (HR: 0.162; 95% CI: 0.094-0.279; p <0.001). Even chemotherapy alone has a positive effect on overall survival (HR: 0.430; 95% CI: 0.256-0.723; p = 0.001). In the multivariable Cox regression analysis, the best survival times in relapse compared to untreated patients were found in the operated patients and patients treated adjuvantly with platinum and taxol with a median overall survival time of 53.4 months (HR: 0.118; 95% CI: 0.058 -0.242; p <0.001). However, surgical therapy followed by chemotherapy alone, especially platinum-based chemotherapy, has a positive effect on overall survival (median OS for platinum mono 34.2 months, p <0.001 vs median OS 29.6 months, p = 0.001 for other chemotherapy). In the multivariable analysis of patients who did not operate in relapse, a combination therapy containing platinum and taxol had the most favorable effect on survival time in relapse compared to the reference group and those who received monotherapy with platinum (median OS 23.6 months HR : 0.326; 95% CI: 0.158-0.673; p = 0.002). Chemotherapy alone also had a positive effect on overall survival compared to the reference group (who did not receive any therapy), whereas platinum-based monotherapy is superior to another therapeutic substance (platinum mono vs other chemo 16.8 months vs 10.8 months HR: 0.351; 95% CI: 0.184-0.669; p = 0.001).
An important step in the treatment of relapsed ovarian cancer is a combination chemotherapy containing platinum and taxol, which has a significant survival benefit. Surgical therapy in the event of a relapse followed by chemotherapy containing platinum and taxol also produced a significant survival benefit. Further studies will be necessary to validate the benefit of surgical therapy for a selective patient population.
