Ziel der vorliegenden Arbeit ist es, Kausalitäten und Prävalenzen anatomischer Limitationen bei für das TAVI-Verfahren gescreenten Patienten zu untersuchen. Ziel der vorliegenden Arbeit ist es, Kausalitäten und Prävalenzen anatomischer Limitationen bei für das TAVI-Verfahren gescreenten Patienten zu untersuchen. Bis dato existieren keine vergleichbaren publizierten Arbeiten.Die Kenntnis der zugrundeliegenden anatomischen Grundlagen und deren Prävalenzen ist essenziell, um die Entwicklung zukünftiger TAVI-Prothesen sinnvoll planen zu können, und potenziell bestehende Lücken in der Versorgung zu schließen. Insgesamt wurden 2260 Patienten mit durchgeführtem TAVI-Screening ermittelt. 71 Patienten mit vorbestehender biologischer Aortenklappenprothese, bei welchen eine sogenannte „Valve- in-Valve“-Prozedur mit Implantation der TAVI-Prothese in die degenerierte Bioprothese geplant war, wurden aus der weiteren Analyse ausgeschlossen.Im verbleibenden Kollektiv (n=2189 Patienten) wurde in 2130 Fällen eine für das TAVI- Verfahren geeignete Anatomie gesehen. 59 Fälle (2,7%) wiesen anatomische Kontraindikationen für das TAVI-Verfahren auf, und wurden entsprechend alternativ behandelt. Hierbei war in der Mehrzahl der Fälle (n=51, 86,4%) eine konventionelle Operation möglich, bei den als inoperabel eingeschätzten Pateinten erfolgte in drei Fällen (5,1%) eine Ballonvalvuloplastie, und in fünf Fällen (8,5%) eine konservative Therapie.

The aim of the present work is to investigate the causalities and prevalence of anatomical limitations in patients screened for the TAVI procedure. To date, there are no comparable published works.Knowledge of the underlying anatomical principles and their prevalence is essential in order to be able to plan the development of future TAVI prostheses in a meaningful way and to close any potential gaps in the supply. A total of 2260 patients with TAVI screening were identified. 71 patients with a pre-existing biological aortic valve prosthesis, for whom a so-called “valve-in-valve” procedure with implantation of the TAVI prosthesis in the degenerated bioprosthesis was planned, were excluded from the further analysis.In the remaining collective (n = 2189 patients), an anatomy suitable for the TAVI procedure was seen in 2130 cases. 59 cases (2.7%) had anatomical contraindications for the TAVI procedure and were treated alternatively accordingly. In the majority of cases (n = 51, 86.4%) a conventional operation was possible, in the patients assessed as inoperable, balloon valvuloplasty was performed in three cases (5.1%), and in five cases (8.5%) ) conservative therapy.
