Einleitung: Die vorliegende Arbeit untersucht die Inzidenz sowie potenzielle Risikofaktoren internistischer und chirurgischer Komplikationen nach elektiven neurochirurgischen Eingriffen an der lumbalen Wirbelsäule. Abstract (German)
Einleitung: Die vorliegende Arbeit untersucht die Inzidenz sowie potenzielle Risikofaktoren internistischer und chirurgischer Komplikationen nach elektiven neurochirurgischen Eingriffen an der lumbalen Wirbelsäule.
Material und Methoden: Hierzu wurde ein Patientenkollektiv bestehend aus 1.226 Patienten betrachtet, welche im Zeitraum von Januar 2009 bis einschließlich Oktober 2014 in der Klinik für Neurochirurgie des Universitätsklinikums Regensburg aufgrund eines Bandscheibenprolaps, einer Spinalkanalstenose oder einer Instabilität der LWS erstmals elektiv operiert wurden. Als Referenzgruppe dienten die Daten aller im Zeitraum vom 05.02.2014 bis 16.10.2014 mit oben genannten Indikationen operierten Patienten, welche einen unkomplizierten intra- und postoperativen Verlauf zeigten. Die untersuchten Prädiktoren Alter, Geschlecht, Vorerkrankungen, Indikation und Lokalisation sowie die Art und der Verlauf postoperativer Komplikationen konnten Stationsakten und Entlassberichten entnommen werden. Zur Erhebung intraoperativer Daten wie der Dauer des Eingriffs dienten OP- und Anästhesieprotokolle. Um statistisch verwertbare Stichprobengrößen zu generieren wurden sowohl Vorerkrankungen als auch Komplikationen in Gruppen zusammengefasst.
Ergebnisse: Der Altersdurchschnitt der Patienten in der Studiengruppe betrug 67,4 (SD = 12,2) Jahre und war signifikant höher als in der Kontrollgruppe (M = 58,6, SD = 13,9, p<0,001). Bezüglich der Geschlechterverteilung ergab sich keine statistische Signifikanz. Die Diagnose Bandscheibenvorfall war mit 59,2% die häufigste Indikation in der Kontrollgruppe, in der Studiengruppe überwog hingegen mit 48,4% die Diagnose Instabilität, die damit signifikant häufiger auftrat (p<0,001). Am häufigsten wurde das Segment L4/5 operiert. Multisegmentale Eingriffe wurden bei 24,7% der Patienten in der Studiengruppe durchgeführt, in der Kontrollgruppe waren es 11,7% (p=0,017). Eine oder mehrere Vorerkrankungen wurden bei 37,6% der Patienten der Studiengruppe dokumentiert, in der Kontrollgruppe wiesen 22,3% der Patienten Komorbiditäten auf (p=0,019). Kardiale Vorerkrankungen waren mit 23,7% in der Studien- und 10,7% in der Kontrollgruppe am häufigsten, gefolgt von Diabetes mellitus.
Als allgemeine Risikofaktoren für das Auftreten internistischer oder chirurgischer Komplikationen konnten in der univariaten Analyse hohes Patientenalter (p =0,001), lange OP- Dauer (p <0,001), multisegmentale Eingriffe (p=0,017), die Indikation Instabilität (p <0,001) sowie das Vorhandensein einer oder mehrerer Vorerkrankungen im Allgemeinen (p=0,019) und kardialer Vorerkrankungen (p=0,015) im Speziellen identifiziert werden. Diese Ergebnisse stehen in Einklang mit der vorhandenen Literatur. In der multivariaten Analyse verblieben Alter und die Indikation Instabilität als signifikante Risikofaktoren. Spezielle Komplikationen, bei welchen ein Zusammenhang mit dem Patientenalter zu erkennen war, waren Bluttransfusionen, Nachblutungen, Wundheilungsstörungen, kardiale Komplikationen und internistische Infektionen. Bezüglich des Geschlechts hatten lediglich weibliche Patientinnen ein erhöhtes Risiko intra- oder postoperativer Bluttransfusionen. Die Dauer der Operation spielte für das Auftreten einer Materialfehllage, Verletzungen der Dura sowie Pneumonien und Harnwegsinfekte eine Rolle, außerdem kam es auch hier zu einer vermehrten Notwendigkeit von Bluttransfusionen. Multisegmentale Eingriffe korrelierten mit Bluttransfusionen, Nachblutungen, Wundheilungsstörungen und internistischen Infektionen. Die Indikation Instabilität war signifikant häufig mit Wundheilungsstörungen, internistischen Infektionen und nephrologischen Komplikationen vergesellschaftet.
Hinsichtlich spezieller Vorerkrankungen fanden sich einige statistisch signifikante Ergebnisse. Für kardiale Komplikationen fand sich starker Zusammenhang mit kardialen Komorbiditäten und Diabetes mellitus. Infektionen und nephrologische Komplikationen traten vermehrt bei Patienten mit nephrologischen Vorerkrankungen auf. Ein in der Literatur häufig beschriebener Zusammenhang zwischen malignen Vorerkrankungen und Thrombembolien konnte in der vorliegenden Studie ebenfalls nachgewiesen werden. Für die Vorerkrankung Gerinnungsstörung konnten aufgrund der niedrigen Stichprobenzahl keine Aussagen getroffen werden.
Patienten mit internistischen Komplikationen waren im Schnitt älter, hatten einen höheren Anteil an Männern sowie den Indikationen Spinalkanalstenose und Spondylolisthese, mehr Vorerkrankungen und längere Operationszeiten als die Gruppe der Patienten, welche chirurgische Komplikationen entwickelten.
Diskussion: Insgesamt lagen die in der vorliegenden Studie ermittelten Komplikationsinzidenzen im Literaturvergleich in einem sehr niedrigen Bereich. Dies kann an den Ein- und Ausschlusskriterien für das gewählte Patientenkollektiv liegen, spricht aber auch für die Qualität der untersuchten Eingriffe, zumal schwerwiegende oder lebensbedrohliche Komplikationen sehr selten auftraten.
Ein besonderes Augenmerk sollte auch in Zukunft auf Patienten mit hohem Alter, mit multisegmentalen Eingriffen sowie mit Komorbiditäten, insbesondere mit kardialen Vorerkrankungen, gelegt werden.

Introduction: The thesis examines the incidence and potential risk factors of internal and surgical complications after elective neurosurgical interventions on the lumbar spine.
Methods: For this purpose, a patient collective consisting of 1,226 patients was considered who were electively operated for the first time in the period from January 2009 to October 2014 in the Department of Neurosurgery at the Regensburg University Hospital because of a herniated disc, spinal stenosis or instability of the lumbar spine. The reference group was the data of all patients operated on in the period from February 5th, 2014 to October 16th, 2014 with the above-mentioned indications, who did not show any complications. The investigated predictors of age, gender, previous illnesses, indication and location as well as the type and course of postoperative complications could be taken from ward files and discharge reports. The surgical and anesthetic protocols were used to collect intraoperative data such as the duration of the procedure. In order to generate statistically usable sample sizes, both pre-existing illnesses and complications were combined in groups.
Results: The mean age of the patients in the study group was 67.4 (SD 12.2) years and significantly higher than in the control group (M = 58.6, SD = 13.9, p <0.001). There was no statistical significance with regard to the gender distribution. The diagnosis of a herniated disc was the most common indication in the control group with 59.2%, whereas the diagnosis of instability predominated in the study group with 48.4% (p <0.001). The L4 / 5 segment was operated most frequently. Multisegmental interventions were performed on 24.7% of the patients in the study group and 11.7% in the control group (p = 0.017). One or more pre-existing conditions were documented with 37.6% of the patients in the study group; in the control group, 22.3% of the patients had comorbidities (p = 0.019). Pre-existing cardiac diseases were the most common comorbidity with 23.7% in the study group and 10.7% in the control group, followed by diabetes mellitus.
In the univariate analysis, the general risk factors for the occurrence of internal or surgical complications were high patient age (p = 0.001), long duration of surgery (p <0.001), multisegmental interventions (p = 0.017), the indication of instability (p <0.001) and the presence of one or more pre-existing conditions in general (p = 0.019) and cardiac pre-existing conditions (p = 0.015) in particular. These results are consistent with the existing literature. According to multivariate analysis, age and the indication instability were identified as risk factors. Specific complications that were significantly related to the patient's age were blood transfusions, secondary bleeding, wound healing disorders, cardiac complications and internal infections. Regarding gender, only female patients had an increased risk of intra- or postoperative blood transfusions. The duration of the operation was statistically significant for the occurrence of material deficiencies, injuries to the dura, pneumonia and urinary tract infections and there was also an increased need for blood transfusions. Multisegmental interventions correlated with blood transfusions, rebleeding, wound healing disorders and internal infections. The indication instability was significantly associated with wound healing disorders, internal infections and nephrological complications.
With regard to specific pre-existing conditions, some statistically significant results were found. Cardiac complications were found to be strongly related to cardiac comorbidities and diabetes mellitus. Infections and nephrological complications occurred more frequently in patients with previous nephrological diseases. A relationship between pre-existing malignant diseases and thromboembolism could also be demonstrated in the present study. Due to the low number of samples, no statements could be made for the pre-existing condition of coagulation disorder.
Patients with internal complications were older on average and more often had the indications of spinal stenosis and spondylolisthesis, more pre-existing conditions and longer surgery times than the group of patients who developed surgical complications.
Discussion: Overall, the complication incidences determined in the present study appeared at a very low rate as compared to the literature. This may be due to the inclusion and exclusion criteria for the selected group of patients, but it also indicates the quality of the examined interventions, especially since serious or life-threatening complications occurred very rarely.
In the future, particular attention should be paid to patients of old age, with multisegmental interventions and comorbidities, especially those with previous cardiac diseases.
