Abstract (German)
LI-RADS ist ein Klassifikationssystem, das 2011 vom American College of Radiology entwickelt wurde, um die radiologische Berichterstattung, und Interpretation von CT- und MR-Untersuchungsergebnissen der Leber bei Patienten mit Risiko für ein hepatozelluläres Karzinom (HCC) zu standardisieren. Eine weiter verbreitete Annahme von LI-RADS könnte helfen die Variabilität radiologischer Befunde, die das HCC betreffen, zu reduzieren. Hierdurch würde eine klarere Kommunikation zwischen Radiologen, Hepatologen, Viszeralchirurgen und Onkologen ermöglicht werden, was letztendlich das Patientenmanagement optimieren könnte. Um diesen Zielen näher zu kommen, entschlossen wir uns zu einer Statuserhebung, die untersucht, wie bekannt LI-RADS aktuell unter Fachleuten in Deutschland ist, und wie häufig es von diesen angewandt wird. Um dies repräsentativ zu evaluieren, haben wir einen Fragebogen erstellt, durch den der aktuelle Status quo hinsichtlich der Bekanntheit und der klinischen Anwendung des Klassifikationssystems LI-RADS unter Internisten/Gastroenterologen, Chirurgen und Radiologen in Deutschland erhoben wird. Mit Hilfe von www.surveymonkey.com, einem Online-Tool zur Erstellung von Online-Umfragen, wurde ein aus insgesamt 17 Fragen bestehender Fragebogen zu LI-RADS verfasst. Durch Verwendung der Website www.deutsches-krankenhausverzeichnis.de erstellten wir eine Liste von Krankenhäusern, die die Kriterien Innere Medizin, Gastroenterologie, Allgemein- und Viszeralchirurgie, und Radiologie erfüllten. Hiervon wurden 102 Krankenhäusern zufällig ausgewählt, und anschließend die Leitungen der entsprechenden Fachabteilungen gebeten, uns einen Assistenzarzt und einen Oberarzt zu benennen, um an unserer Umfrage teilzunehmen. Hierdurch erhielten wir die Kontaktdaten von 177 potenziellen Kandidaten, an welche unsere Umfrage per E-Mail versendet wurde. Unsere Zielpersonen wurden bis zu dreimal kontaktiert, insgesamt nahmen 87 Personen an unserer Umfrage teil, wir erreichten hierdurch eine Rücklaufquote von 49,15 %. Anschließend wurden die Daten mittels statistisch deskriptiver Methoden ausgewertet. Zusammenfassend lässt sich sagen, dass mit 73,17 % der Großteil der Befragten zumindest schon einmal von LI-RADS gehört hat. Die meisten Teilnehmer, die gut über LI-RADS Bescheid wussten, waren das Fachgebiet betreffend eher Radiologen, die berufliche Position betreffend eher Oberärzte und leitende Oberärzte als Assistenzärzte, und arbeiteten am ehesten an Universitätskliniken. Bemerkenswert ist auch, dass der Großteil der an unserer Umfrage teilnehmenden Ärzte eine strukturierte Befundung und Terminologie in der Radiologie prinzipiell für wünschenswert hielt: Denn 98,7 % der Befragten gaben an, sich entweder „etwas mehr strukturierte Befunde“, oder „mehr strukturierte Befunde bei onkologischen Fragestellungen“, oder „generell mehr strukturierte Befunde“ zu wünschen. Um eine solche Strukturierung und Standardisierung umsetzen zu können, stimmte die Mehrheit (59,5 %) unserer Umfragen-Teilnehmer der Annahme voll oder eher zu, dass durch LI-RADS im klinischen Alltag Beurteilungskriterien standardisiert werden können und der Interpretationsspielraum verringert wird. Dieses überwiegend positive Meinungsbild ist allerdings deutlich diskrepant zu der tatsächlichen Nutzung der LI-RADS-Klassifikation im klinischen Alltag. 74,03 % haben die LI-RADS-Klassifikation selbst noch nicht angewandt und bei der Mehrheit (80,77 %) wird LI-RADS auch im Tumorboard bzw. in der Abteilung nicht verwendet. Es zeigte sich deutlich, dass insofern LI-RADS eingesetzt wurde, dies unter den Teilnehmern an unserer Umfrage bisher vor allem an größeren Krankenhäusern und Universitätskliniken geschah. 32,00 % aller Befragten gaben an, sich (mehr) Anwendung von LI-RADS bei allen Raumforderungen in einer zirrhotischen Leber zu wünschen. Unter den Teilnehmern, die LI-RADS selbst schon angewandt haben, wünschen sich dies sogar 72,22 %. Dies lässt eine allgemeine Bereitschaft zur bzw. einen generellen Wunsch nach einer weiter verbreiteten Anwendung von LI-RADS vermuten. Deswegen ist es wichtig LI-RADS in der Institutions- bzw. Abteilungskultur der Kliniken fest zu verankern, um das Klassifikationssystem noch stärker in den Arbeitsalltag von Medizinern, die mit HCC-Risikopatienten arbeiten, zu integrieren, um hierdurch eine weitere Standardisierung zu ermöglichen.

LI-RADS is a classification system developed by the American College of Radiology in 2011 to standardize the radiological reporting and interpretation of CT and MRI scan results of liver lesions in patients at risk for hepatocellular carcinoma (HCC). A widespread application of LI-RADS could help to reduce the variability of radiological results pertaining to HCC, and in addition improve the communication among radiologists, hepatologists, hepatic surgeons and oncologists, which could ultimately optimize the patient management. In order to get closer to these goals, we decided to conduct a nationwide online survey to assess the current status regarding the awareness and application of LI-RADS among experts in Germany. With the help of www.surveymonkey.com, an online tool for the creation of online surveys, we drew up an online-questionnaire about LI-RADS, consisting of a total of 17 questions. Using the website www.deutsches-krankenhausverzeichnis.de a list of hospitals was generated meeting the criteria internal medicine, gastroenterology, general and visceral surgery, and radiology. 102 of these hospitals were randomly selected, and then the directors of the relevant departments were asked to name one consultant and one resident from their department in order to participate in our survey. 177 potential participants were invited to fill out the questionnaire. Our target persons were contacted up to three times, altogether 87 people took part in our survey, which corresponds to a response rate of 49.15%. The results of the survey were analysed by means of descriptive statistics. In summary, with 73,17%, the majority of the respondents has at least heard about LI-RADS. Most of the participants, who had a good knowledge about LI-RADS, were particularly radiologists, were more likely senior physicians than assistant physicians, and were most likely to work at university hospitals. It is also noteworthy that most of the doctors participating in our survey considered a structured diagnosis and terminology in radiology to be desirable: 98.7% of the respondents stated that they wish either “slightly more structured radiological reports” or “more structured radiological reports for oncological questions”, or “generally more structured radiological reports”. To be able to establish such structuring and standardization, the majority (59.5%) of our survey participants fully or somewhat agreed with the assumption that LI-RADS can standardize assessment criteria in everyday clinical practice and furthermore reduce the scope for interpretation. The results of our survey demonstrate that LI-RADS is relatively known in Germany. This and the predominantly positive opinion about LI-RADS and structured reporting is, however, clearly discrepant with the actual use of the classification in everyday clinical practice, which is quite sparse: 74.03% have not used the LI-RADS classification themselves yet, and the majority (80.77%) has not used LI-RADS in the tumour board or in the department. So, it seems that LI-RADS is not yet broadly implemented in clinical routine in Germany. It shows up clearly, that so far LI-RADS is being used, currently this happens especially at larger hospitals and university hospitals among the participants in our survey. 32% of all respondents wanted (more) use of LI-RADS regarding all lesions in a cirrhotic liver. Among the participants who have already used LI-RADS themselves, even 72.22% wanted an increased use of the diagnostic algorithm. This shows that there is generally a willingness and a desire for a more widespread use of LI-RADS. Interdisciplinary education may support the propagation and use of the LI-RDAS classification. Moreover, it is important to firmly anchor LI-RADS in the institutional or departmental culture of the hospitals, in order to integrate the classification system into the everyday work of doctors, working with HCC-risk patients, to enable further standardization.
