Odontoblasten trennen das Weichgewebe der dentalen Pulpa vom Hartgewebe des Dentins und kleiden als terminal differenzierte Zellen in einem einschichtigen Epithel die Pulpakammer aus. Odontoblasten vereinen drei sehr komplexe und scheinbar unabhängige Funktionen. Als sensorische Zellen rezipieren Odontoblasten thermische, mechanische sowie chemische Reize. Sie exprimieren mechanosensitive Ionenkanäle oder allgemein TRP-Kanäle (transient receptor potential channels) als Basis von Mechanorezeption und Signaltransduktion. In einer vollkommen anderen Funktion detektieren Odontoblasten im verzweigten System der zellulären Immunantwort als Epithelzellen die Invasion pathogener kariogener Keime, verstärken diese Information und geben sie an andere Immunzellen, etwa dendritische Zellen, im Pulpa-Dentin-Komplex weiter. Schließlich ist die Bildung von Dentin die primäre Funktion reifer Odontoblasten.
Allerdings ist es bis heute nicht gelungen, primäre humane Odontoblasten zu isolieren und in vitro zu kultivieren. Eine Kultur humaner Odontoblasten wäre äußerst wünschenswert, weil nur so Prozesse ihrer komplexen Funktionen auf zellulärer Ebene studiert werden könnten. Dieses Wissen wäre dann die Basis für therapeutische Ansätze und Strategien in der restaurativen Zahnheilkunde. Ein erster Ansatz dazu wäre die vertiefte Charakterisierung von Zellen, die kürzlich in unserer Arbeitsgruppe an der Poliklinik für Zahnerhaltung und Parodontologie aus der Pulpa-Dentin-Grenze isoliert wurden. Jedoch ist die Identifizierung dieser Zellen als Odontoblasten ein schwieriges Vorhaben, weil ein spezifischer Marker für Odontoblasten derzeit nicht bekannt ist. Obwohl die aus der Pulpa-Dentin-Grenze isolierten primären Zellen einen Odontoblasten-Phänotyp zu exprimieren scheinen, sollte eine detaillierte Analyse von Mechanismen der unterschiedlichen Funktionen ihre exakte Identifizierung ermöglichen.
Daher sollte in einem ersten Schritt in der vorliegenden Literaturarbeit das derzeit verfügbare Wissen über intrazelluläre Signalwege der Bildung von Dentin und Tertiärdentin als eine der Funktionen von Odontoblasten vergleichend diskutiert werden. Vor allem sollten die über Wnt/β-Catenin, TGF-β/Smad und FGF vermittelten Pathways in Odontoblasten, odontoblasten-ähnlichen Zellen und dentalen Pulpazellen erfasst werden. Momentan ist nicht bekannt, wie spezifisch diese Signalwege für Odontoblasten wirklich sind und wie weit andere alternative Wege für die Funktion der Dentin- und Tertiärdentinbildung erforscht wurden. Zu diesen Fragen wurde eine Literaturrecherche in der Datenbank MEDLINE® über die Plattform OVID durchgeführt. Die von Dubletten bereinigte Suche mit spezifischen Schlüsselwörtern ergab 8992 individuelle Einträge. Als Ergebnis eines Relevanzrankings wurde in erster Priorität eine Trefferzahl von 2761 erzielt, in zweiter Priorität ohne molekulare Terme wurden 6246 Referenzen ermittelt. Diese Quellen wurden in das Literaturverwaltungsprogramm Citavi importiert. Für die vorliegende Arbeit wurden ausschließlich die Referenzen erster Priorität nach „relevant“, „nicht relevant“, „Relevanz unklar“ und „aussortierte Dubletten“ sortiert.
In der vorliegenden Arbeit wurden zunächst noch vor der Analyse spezifischer Signalwege die Struktur des Dentin-Pulpa-Komplexes dargestellt und die Stadien der Entwicklung humaner Zähne analysiert. Anschließen wurde gezeigt, wie die Signalwege, die zur Dentinbildung führen, auch an der Regulation der Zahnentwicklung beginnend mit der Bildung von Plakoden beteiligt sind. Zu diesen Pathways zählen diejenigen über Wnt, TGFβ/BMP oder auch Wachstumsfaktoren wie der Fibroblasten-Wachstumsfaktor (FGF). Wnt ist eines der zentralen Signalmoleküle der Ontogenese und als wachstumsstimulierendes Protein ein Signalmolekül, das Zellproliferation und Zellmigration induziert, die Physiologie von Stammzellen reguliert und wachsendem Gewebe eine Form verleiht. Die Komplexität des Pathways spiegelt sich in der großen Zahl an Liganden und Rezeptoren wider und aus der Aktivierung von Wnt-Rezeptoren in der Zellmembran resultiert bevorzugt über die Aktivierung des ß-Catenin/TCF-Transkriptionskomplexes eine Vielfalt intrazellulärer Reaktionen. Der Wnt-ß-Catenin Signalweg hat eine bedeutende Funktion als Regulator der Dentinogenese und ist ein zentraler Mechanismus der Reparatur von Schäden des Pulpa-Dentin-Komplexes. Dieser Pathway übernimmt auch eine zentrale Funktion in der Bildung von Tertiärdentin durch die Förderung der Proliferation und der Differenzierung dentaler Stammzellen und Zellen der dentalen Papille.
Ein anderer Signalweg zur Dentinbildung wird von transformierenden Wachstumsfaktoren-ß (TGF-ß) gesteuert, die als Zytokine der Kommunikation zwischen Zellen dienen. Die heterogene Familie humaner TGF-ß Proteine umfasst drei Isoproteine von TGF-ß, Aktivine (Inhibine), Nodal (Bmp-16), BMPs (bone morphogenetic proteins) und GDFs (growth and differentiation factors). Vor allem BMP-2 und BMP-4 scheinen an der Bildung des Skelettes und der Differenzierung von Hartgewebe beteiligt zu sein. Funktionell binden die TGF-ß-Liganden zunächst an Rezeptoren in der Zellmembran und aktivieren Smad- und Nicht-Smad-abhängige Signaltransduktionsketten. Smad-Proteine sind die Haupteffektorproteine des TGF-ß-Signalwegs. Isoformen des TGF-ß fungieren auch in der Zahnreparatur. So war TGF-ß1 in Odontoblasten und Pulpazellen kariöser Zähne verstärkt exprimiert und im tertiären Dentin nachweisbar. TGF-ß scheint die Differenzierung dentaler Pulpazellen zu Odontoblasten zu steuern und so die reparative Dentinbildung zu induzieren.
Die Mitglieder der Familie der Fibroblasten-Wachstumsfaktoren (FGF) interagieren mit signalgebenden Tyrosinkinase-FGF-Rezeptoren (FGFRs) in der Zellmembran und vermitteln ein breites Spektrum an Funktionen. FGFs sind in den frühesten Stadien der Embryonalentwicklung und während der Organogenese an Differenzierung und Wachstum beteiligt und regulieren im erwachsenen Gewebe Stoffwechsel-, Reparatur- und Regenerationsprozesse. In der Zahnentwicklung steuern FGFs die charakteristischen epithelialen-mesenchymalen Interaktionen. FGF2 scheint hier auf die Zellen der Odontoblastenlinie stadienspezifisch zu wirken, und FGF-2,-3,-4 und -8 sind an der Entwicklung der Zahnkrone beteiligt. Mitglieder der Familie der FGFs kontrollieren aber auch die reparative Dentinogenese, beispielsweise über die Differenzierung von Vorläuferzellen in Pulpazellkulturen zu funktionellen Odontoblasten.
Neben diesen klassischen Signalwegen scheinen auch besondere Enzymaktivitäten wie die der Sphingomyelinasen (SMases) oder der Histonacetyltransferasen (HAT) die Bildung von Dentin und Tertiärdentin zu fördern. SMases katalysieren die Hydrolyse von Sphingolipiden wie Sphingomyelin (SM) zu Ceramid, die ihrerseits als Signalmoleküle zellulären Stress, Zellteilung oder Zelldifferenzierung steuern. Die Notwendigkeit der Expression von Smpd3 (Sphingomyelin-Phosphodiesterase 3) in Odontoblasten als Regulator der Mineralisation der extrazellulären Matrix der Zähne wurde kürzlich nachgewiesen. Die reversible Acteylierung von Histonen durch Histonacetyltransferasen (HAT) wiederum ist ein wichtiger epigenetischer Mechanismus der Regulation des Gleichgewichts zwischen Knochenbildung und -abbau. Es gibt inzwischen auch Untersuchungen zur Funktion von Histondeacetylasen (HDACs) in der Dentinbildung und von Möglichkeiten, durch die Verwendung von HDAC-Inhibitoren etwa die Differenzierung von Pulpazellen und die Bildung von Tertiärdentin zu beeinflussen.
Trotz dieser teilweise sehr detaillierten Kenntnisse von Signaltransduktionswegen ist das aktuelle Wissen über molekulare Mechanismen im Prozess der Tertiärdentinbildung weiterhin sehr gering. Dennoch sind die hier erzielten Resultate eine Grundlage für experimentelle Arbeiten, Funktionen verschiedener Pathways für die Bildung von Tertiärdentin zu untersuchen. Man darf erwarten, dass die spezifische Bedeutung einzelner Signalwege durch die Inhibition mit chemischen Substanzen oder knockdown-Experimenten und andererseits mittels Überexpression zentraler Regulatorproteine identifiziert werden kann.

Odontoblasts separate the soft tissue of the dental pulp from the hard tissue of the dentin and line the pulp chamber as terminally differentiated cells in a single-layer epithelium. Odontoblasts combine three very complex and apparently independent functions. As sensory cells, odontoblasts receive thermal, mechanical and chemical stimuli. They express mechanosensitive ion channels or generally TRP channels (transient receptor potential channels) as the basis of mechanoreception and signal transduction. In a completely different function, odontoblasts in the branched system of the cellular immune response as epithelial cells detect the invasion of pathogenic cariogenic germs, amplify this information and pass it on to other immune cells, such as dendritic cells, in the pulpa-dentine complex. Finally, the formation of dentin is the primary function of mature odontoblasts.
However, to date it has not been possible to isolate primary human odontoblasts and cultivate them in vitro. A culture of human odontoblasts would be extremely desirable because only in this way could processes of their complex functions be studied at the cellular level. This knowledge would then be the basis for therapeutic approaches and strategies in restorative dentistry. A first approach would be the in-depth characterization of cells that were recently isolated from the pulpa-dentin boundary in our working group at the Polyclinic for Tooth Conservation and Periodontology. However, identifying these cells as odontoblasts is a difficult proposition because a specific marker for odontoblasts is not known at present. Although the primary cells isolated from the pulp-dentin junction appear to express an odontoblastic phenotype, a detailed analysis of mechanisms of the different functions should allow their exact identification.
Therefore, in a first step, the currently available knowledge about intracellular signaling pathways of the formation of dentine and tertiary dentine as one of the functions of odontoblasts should be discussed comparatively in the present literature work. In particular, the pathways mediated via Wnt/β-catenin, TGF-β/Smad and FGF in odontoblasts, odontoblast-like cells and dental pulp cells should be recorded. It is currently unknown how specific these signaling pathways really are for odontoblasts and how far other alternative pathways for the function of dentin and tertiary dentin formation have been explored.
A literature search was carried out on these questions in the MEDLINE® database via the OVID platform. The de-duplicated search using specific keywords returned 8992 unique entries. As a result of a relevance ranking, a number of hits of 2761 was achieved in the first priority, 6246 references were determined in the second priority without molecular terms. These sources were incorporated into the
Imported reference management program Citavi. For the present work, only the first-priority references were sorted according to "relevant", "not relevant", "relevance unclear" and "sorted out duplicates".
In the present work, the structure of the dentin-pulp complex was presented and the stages of development of human teeth were analyzed before the analysis of specific signaling pathways. It was then shown how the signaling pathways that lead to dentine formation are also involved in the regulation of tooth development, starting with the formation of placodes. These pathways include those via Wnt, TGFβ/BMP or growth factors such as fibroblast growth factor (FGF).
Wnt is one of the central signaling molecules of ontogenesis and as a growth-stimulating protein, a signaling molecule that induces cell proliferation and cell migration, the physiology
regulated by stem cells and gives shape to growing tissue. The complexity of the pathway is reflected in the large number of ligands and receptors, and the activation of Wnt receptors in the cell membrane, preferably via the activation of the ß-catenin/TCF transcription complex, results in a variety of intracellular reactions. The Wnt-ß-catenin signaling pathway has an important function as a regulator of dentinogenesis and is a central mechanism for repairing damage to the pulp-dentin complex. This pathway also plays a central role in the formation of tertiary dentine by promoting the proliferation and differentiation of dental stem cells and dental papilla cells.
Another signaling pathway for dentin formation is controlled by transforming growth factor-ß (TGF-ß), which serve as cytokines of communication between cells. The heterogeneous family of human TGF-ß proteins includes three isoproteins of TGF-ß, activins (inhibins), Nodal (Bmp-16), BMPs (bone morphogenetic proteins) and GDFs (growth and differentiation factors).
In particular, BMP-2 and BMP-4 appear to be involved in skeletal formation and hard tissue differentiation. Functionally, the TGF-ß ligands first bind to receptors in the cell membrane and activate Smad and non-Smad dependent signal transduction chains. Smad proteins are the main effector proteins of the TGF-ß signaling pathway. Isoforms of TGF-ß also function in tooth repair. Thus, TGF-ß1 was increased in odontoblasts and pulp cells of carious teeth
expressed and detectable in tertiary dentin. TGF-ß appears to control the differentiation of dental pulp cells into odontoblasts and thus induce reparative dentin formation.
Members of the fibroblast growth factor (FGF) family interact with signaling tyrosine kinase FGF receptors (FGFRs) in the cell membrane and mediate a wide range of functions. FGFs are involved in differentiation and growth in the earliest stages of embryonic development and during organogenesis, and regulate metabolic, repair, and regenerative processes in adult tissues. In tooth development, FGFs control the characteristic epithelial-mesenchymal interactions. Here, FGF2 seems to act in a stage-specific manner on the cells of the odontoblast lineage, and FGF-2,-3,-4 and -8 are involved in the development of the tooth crown. However, members of the FGF family also control reparative dentinogenesis, for example via the differentiation of progenitor cells in pulp cell cultures into functional odontoblasts.
In addition to these classic signaling pathways, special enzyme activities such as sphingomyelinases (SMases) or histone acetyltransferases (HAT) also seem to promote the formation of dentine and tertiary dentine. SMases catalyze the hydrolysis of sphingolipids such as sphingomyelin (SM) to ceramide, which in turn control cellular stress, cell division or cell differentiation as signaling molecules. The need for the expression of Smpd3 (sphingomyelin phosphodiesterase 3) in odontoblasts as a regulator of mineralization of the extracellular matrix of the teeth has recently been demonstrated. The reversible acetylation of histones by histone acetyltransferases (HAT), in turn, is an important epigenetic mechanism regulating the balance between bone formation and breakdown. There are now also investigations into the function of histone deacetylases (HDACs) in dentine formation and the possibilities of influencing the differentiation of pulp cells and the formation of tertiary dentine through the use of HDAC inhibitors.
Despite this sometimes very detailed knowledge of signal transduction pathways, the current knowledge about molecular mechanisms in the process of tertiary dentin formation is still very limited. Nevertheless, the results obtained here are a basis for experimental work to investigate the functions of different pathways for the formation of tertiary dentine. One can expect that the specific importance of individual signaling pathways can be identified by inhibition with chemical substances or knockdown experiments and, on the other hand, by overexpression of central regulatory proteins.
