Kinder mit operativ korrigierten Anorektalen Malformationen (ARM) leiden häufig unter fäkaler Inkontinenz. Es ist wesentlich für die Kontinenzentwicklung der Kinder, dass der Neoanus optimal im vorhandenen Muskelgewebe platziert wird. Um dies zu optimieren, wurde in der Klinik St. Hedwig (Regensburg) die Beckenbodenmuskulatur der Patienten im Rahmen der posterioren sagittalen Anorektoplastik (PSARP) mittels sakraler Nervenstimulation (SNS) beurteilt.
Ziel der Studie war die Erfassung und Evaluation des Outcomes von Patienten mit ARM, die mittels PSARP unter Einsatz von SNS operiert wurden. Das Outcome der Patienten wurde mit dem Outcome nach PSARP ohne SNS in Literaturangaben verglichen.
Es wurden eine retrospektive Datenanalyse und ein prospektives Follow up durchgeführt. Im Rahmen des Follow-ups wurden der Bowel-Function-Score (BFS) nach Rintala, das Outcome nach Krickenbeck und die Lebensqualität mittels Hirschsprung's disease/ anorectal malformation quality-of-life questionnaire (HAQL) erhoben.
Für das Follow-up wurden 21 Patienten eingeschlossen. Es erfolgte im Mittel 5.98 ± 2.49 Jahre postoperativ. Der mediane BFS nach Rintala lag bei 18 Punkten. 83 % der Patienten hatten Stuhlkontrolle, 24 % hatten Stuhlschmieren und 52 % waren obstipiert. Eine komplette Kontinenz war bei 50 % der Patienten zu verzeichnen. Der erreichte mittlere HAQL Score war für alle Subskalen ≥ 83. Bezüglich des BFS nach Rintala zeigte sich ein besseres Ergebnis als in Literaturangaben. Die Ergebnisse des Outcomes nach Krickenbeck ordneten sich im Literaturvergleich im oberen (Stuhlkontrolle, Stuhlschmieren, komplette Kontinenz) respektive mittleren (Obstipation) Bereich ein. Die Lebensqualität nach HAQL unterschied sich nicht wesentlich von den Angaben in der Literatur, allerdings zeigten sich geringfügig höhere Mittelwerte für nahezu alle Subskalen.
Zusammenfassend kann von einem überdurchschnittlich guten Outcome gesprochen werden. Die Ergebnisse liefern erste Hinweise auf eine mögliche Verbesserung des Outcomes von ARM-Patienten durch den Einsatz von SNS im Rahmen von PSARP Operationen. Allerdings können auch weitere Aspekte der Patientenversorgung eine Rolle gespielt haben. Um eine etwaige Kausalität nachzuweisen, sollten weitere Studien in randomisiert kontrolliertem Studiendesign folgen.
Die Studie war durch das retrospektive Studiendesign mit Follow-up, die geringe Fallzahl und das Fehlen einer Kontrollgruppe limitiert.

Translation of the abstract (English)

Children with surgically corrected anorectal malformations (ARM) often suffer from fecal incontinence. It is essential for the development of continence in these children that the neoanus is placed well in the existing muscle tissue. In order to optimize this, the pelvic floor muscles of the patients were assessed during posterior sagittal anorectoplasty (PSARP) using sacral nerve stimulation (SNS) at clinic St. Hedwig (Regensburg, Germany).
The aim of the study was to record and evaluate the outcome of patients with ARM who received PSARP with SNS. The outcome of the patients was compared with the outcome after PSARP without SNS in literature references.
A retrospective data analysis and a prospective follow-up were carried out. As part of the follow-up, the Bowel Function Score (BFS) according to Rintala, the outcome according to Krickenbeck and the quality of life according to Hirschsprung's disease/ anorectal malformation quality-of-life questionnaire (HAQL) were collected.
For the follow-up, 21 patients were included. It was carried out on average 5.98 ± 2.49 years postoperatively. The median BFS was 18 points. 83% of patients had voluntary bowel movements, 24% had soiling and 52% were constipated. Complete continence was recorded in 50% of patients. The average HAQL score achieved was ≥ 83 for all subscales.
Regarding the BFS, a better result was found than in literature references. The results of the outcome according to Krickenbeck were classified in the upper (voluntary bowel movements, soiling, complete continence) or middle (constipation) range compared to the literature references. The quality of life according to HAQL did not differ significantly from the data in the literature.
In summary, one can speak of an above-average outcome. The results provide initial indications of a possible improvement in the outcome of ARM patients through the use of SNS during PSARP. However, other aspects of patient care may also have had an impact on the results. In order to prove any causality, further studies in randomized controlled trial design should follow.
The study was limited by the retrospective study design with follow-up, the low number of cases and the absence of a control group.
