Subtrochantäre Femurfrakturen treten vor allem in höherem Alter nach häuslichem Sturz oder bei jüngeren Patienten im Rahmen eines Hochenergietraumas auf. Im Zuge des demografischen Wandels wird die Inzidenz solcher Femurfrakturen in den nächsten Jahren zunehmen.
Die Versorgung subtrochantärer Femurfrakturen ist aufgrund der massiven Kräfte, die auf diese Region wirken, sehr anspruchsvoll und zieht eine hohe Komplikationsrate nach sich. Insbesondere Pseudarthrosenbildung und Implantatversagen sind als typische Komplikationen subtrochantärer Femurfrakturen zu nennen. Proportional zur Inzidenz wird auch die Anzahl komplikationsreicher Verläufe und die daraus resultierende Notwendigkeit für Revisionseingriffe weiterhin steigen.
Um die Komplikationsrate in Zukunft zu verringern, war das Ziel der vorliegenden Studie, die sekundäre Dislokation subtrochantärer Femurfrakturen im Heilungsverlauf zu untersuchen, Risikofaktoren für eine verzögerte oder ausbleibende Frakturheilung zu ermitteln und Methoden zur Elimination oder Reduzierung dieser zu entwickeln.
Im Rahmen dieser retrospektiven Studie wurden 76 subtrochantäre Femurfrakturen von 75 Patienten, die im Zeitraum von 2007 bis 2017 im Universitätsklinikum Regensburg versorgt wurden, untersucht. Die Datenerhebung erfolgte anhand der Patientendaten sowie der präoperativ, postoperativ und im Heilungsverlauf aufgenommenen Röntgenbilder.
Die Frakturklassifikation und der Singh-Index wurden auf den präoperativen Röntgenbildern bestimmt. Auf den postoperativen Röntgenbildern wurden die Dislokation der Fragmente nach medial, lateral, ventral und dorsal, der CCD-Winkel, die Position der Klinge im Femurkopf und die Nachsinterung nach distal gemessen sowie die Qualität der Reposition beurteilt. Die Dislokation der Fragmente, der CCD-Winkel und die Nachsinterung nach distal wurden auch auf den radiologischen Verlaufsaufnahmen ermittelt und mit den postoperativen Werten verglichen. Mithilfe des RUSH-Scores wurde der Grad der Frakturheilung bewertet.
Anschließend wurden die Frakturen in zwei Gruppen eingeteilt (innerhalb von 6 Monaten verheilt versus verzögert/nicht verheilt) und auf Unterschiede hinsichtlich der erhobenen Variablen geprüft. Weiterhin wurden die verschiedenen Variablen auf ihren Zusammenhang mit der Frakturheilungsdauer untersucht.
Von 76 subtrochantären Femurfrakturen verheilten 30 Frakturen (42,3 %) innerhalb von 6 Monaten nach der Primärversorgung, insgesamt verheilten 45 Frakturen (59,2 %) ohne Revisionseingriff bei einer mittleren Heilungsdauer von 181,83 Tagen.
Die sekundäre Dislokation stellte sich als signifikantes varisches Abkippen des CCD-Winkels um durchschnittlich 4,52° dar (p < .001). Darüber hinaus konnte eine signifikante distale Nachsinterung in Form einer Beinlängenverkürzung um durchschnittlich 4,81 mm festgestellt werden (p < .001).
Der postoperative CCD-Winkel am verletzten Femur war in der Gruppe der verheilten Frakturen signifikant größer als in der Gruppe der verzögert/nicht verheilten Frakturen (127,42° vs. 123,44°, p = .014). Weiterhin unterschied sich die Gruppe der verheilten Frakturen von der anderen Gruppe durch eine signifikant bessere Qualität der Reposition (p = .011). Sowohl der CCD-Winkel als auch die Qualität der Reposition hatten auch einen signifikanten Einfluss auf die Dauer der Frakturheilung (p = .028 und p = .034).
Zusammenfassend bleibt festzuhalten, dass die anatomische Reposition und die Vermeidung eines varischen CCD-Winkels und somit eine gute Qualität der Reposition der Schlüssel zum Erfolg in der Versorgung subtrochantärer Femurfrakturen sind.

Subtrochanteric femur fractures occur mainly at older age after domestic falls or in younger patients as part of a high energy trauma. Due to demographic transition the incidence of such femur fractures will increase in the next years.
The treatment of subtrochanteric femur fractures is extremely challenging because of the massive forces exerted on this body part and entails a high complication rate. In particular, pseudarthroses and hardware related complications are characteristic for complications of subtrochanteric femur fractures. The number of courses with high complications that require surgical revision will continue to increase proportional to the incidence.
To decrease the complication rate in the future, the aim of this study was to examine the secondary dislocation of subtrochanteric femur fractures during the healing process, to identify risk factors for delayed or non-union, and to develop methods to eliminate or reduce them.
In this retrospective study, 76 subtrochanteric femur fractures of 75 patients treated at the University Hospital Regensburg between 2007 and 2017 were examined. Data collection was based on patient data and radiographs taken preoperatively, postoperatively, and during the healing process.
Fracture classification and Singh-Index were determined on the preoperative radiographs. Postoperative radiographs were used to measure the dislocation of the fragments to the medial, lateral, ventral and dorsal side, the neck angle, the lag screw placement in the femoral head, and leg length shortening due to compression within the fracture and to evaluate the quality of reduction. The dislocation of the fragments, the neck angle and the leg length shortening were additionally determined on the radiographs taken during the healing process and compared with the postoperative values. The fracture healing was evaluated by means of the RUSH-Score.
The cases were then divided into two groups (healed within 6 months versus delayed or non-union) and checked for differences with respect to the collected variables. Furthermore, the different variables were tested for their correlation with the fracture healing time.
30 of 76 subtrochanteric fractures (42.3 %) healed within 6 months after the primary surgery. In total 45 fractures (59.2 %) healed without surgical revision with an average healing time of 181.83 days.
The secondary dislocation constitutes a significant varus deformity of the neck angle by an average of 4,52° (p < .001). In addition, a significant leg length shortening by an average of 4.81 mm was detected.
The postoperative neck angle of the injured femur was in the group of healed fractures significantly bigger than in the group of delayed or non-unions (127.42° vs. 123.44°, p = .014). Furthermore, the group of healed fractures differed from the other group by a significantly higher quality of reduction (p = .011). Both the neck angle and the quality of reduction significantly impacted the fracture healing time (p = .028 und p = .034).
In summary, it can be said that anatomic reduction and avoidance of a varus neck angle, and thus a good quality of reduction, are the keys to success in the treatment of subtrochanteric fractures.
