Abstract (German)
Mit der Entscheidung zu einer Mammaaugmentation entwickeln sich bei der Patientin genaue Vorstellung bezüglich des postoperativen Ergebnisses, dessen Verwirklichung das oberste Ziel des Chirurgen sein sollte. Die Wahl der Implantatform („rund“ oder „anatomisch“) ist dabei eine der wichtigsten, aber auch umstrittensten Entscheidungen bei der präoperativen Planung einer ästhetischen Mammaaugmentation. In dieser Arbeit wurde sich erstmalig mittels objektiver Berechnungen anhand intraoperativ gewonnener Daten der Frage gewidmet, ob und inwiefern die Wahl des Implantats das operative Ergebnis beeinflusst.
Es wurden zehn Probandinnen mit ästhetischer Augmentation oder Augmentation mit nachfolgender Mastopexie ohne vorherige Operationen im Bereich der Brust, Epilepsie oder eine Ptose der Brust größer gleich Grad III in die Studie aufgenommen. Intraoperativ wurden nach Präparation der Implantatlager bei den Patientinnen jeweils sechs verschiedene Brustimplantate, die sich in Form und Größe unterscheiden (3x „rund“ und 3x „anatomisch“ zu je „kleinem“, „mittlerem“ und großem“ Volumen), eingesetzt. Noch vor Setzung des ersten Schnittes und nach jedem Einbringen der Implantatpaare und temporärem Wundverschluss mittels Klammern wurde jeweils ein intraoperatives 3D-Modell generiert. Der Scan fand in sitzender und mit um 45° aufgerichteten Oberkörper statt. Zum Einsatz kam hierbei der 3D-Scanners Typ Artec EVA der Firma Artec (Luxemburg), dessen Messgenauigkeit bewiesen werden konnte [64]. Es erfolgte eine Analyse der 65 3D-Scans (130 nach Separierung dieser in rechte und linke Brust) mit Hilfe der Software Artec Studio 12 Professional. Dabei wurden zum einen lineare Messwerte ausgehend von anatomischen Landmarken bestimmt. Die Strecken runder und anatomischer Implantate wurden durch Bildung der Differenz verglichen, wobei sich keine wesentliche Diskrepanz zwischen den linearen Messwerten finden ließ. Die Werte nähern sich der Null an, was darauf schließen lässt, dass die Form keinen entscheidenden Einfluss bei der Streckenvermessung hat.
Zum anderen ist das „Mess“-Tool der Software in der Lage, automatisch den mittleren Abstand zwischen zwei überlagerte Scans in Millimetern (mm) zu berechnen.
Dieser mittlere Abstandsmittelwert zwischen dem „nativen“ Scan und dem „implantierten“ Scan gibt den Einfluss der Implantate auf die Topographie der Brust in mm an und wurde in der Thesis als „topographic shift“ (TS) bezeichnet. Wird die Topographie der gesamten Mammae ohne Unterteilungen betrachtet, so kann kein signifikanter Unterschied zwischen den Implantatformen „rund“ und „anatomisch“ (p = 0,900) jedoch zwischen den Implantatgrößen (p < 0,001) gefunden werden. Mit steigender Projektion steigt der „topographic shift“ und der Einfluss auf die äußerliche Erscheinung. Um den Volumenzuwachs in den weiteren Berechnungen besser lokalisieren zu können, wurde die Brust in vier Quadranten unterteilt und für jeden von ihnen der „topographic shift“ generiert. Auch hierbei lässt sich innerhalb des jeweiligen Quadranten kein signifikanter Unterschied der Form (p = 0,88) verzeichnen. Bei Betrachtung aller Scans zeichnet sich ein signifikanter Unterschied zwischen dem „topographic shift“ (p < 0,001) der jeweiligen vier Quadranten ab. Durch die Brustvergrößerung gewinnen die Patientinnen vor allem im oberen Brustpol an Volumen. Kleine Volumina wirken sich zuerst auf die Quadranten I und II aus. Mit steigendem Implantatvolumen wird Einfluss auf den Quadrant III genommen. Quadrant IV wird durch die Augmentation kaum beeinflusst und korreliert nicht mit der Implantatgröße. Zuletzt wurde untersucht, inwiefern sich runde und anatomische Implantate auf die Lage der Mamille auswirken. Es konnte in 48 von 54 Fällen eine Übereinstimmung anatomischer und runder Implantate bezüglich der Mamillenlage in den Quadranten [2x Quadrant I, 46 x Quadrant II, 0 x Quadrant III] festgestellt werden. In sechs Fällen wurde die Mamille durch das Einbringen der Implantate bei unterschiedlichen Formen in einen anderen Quadranten verschoben.
Schlussfolgerung:
Final lässt sich sagen, dass weniger die Wahl der Implantatform als dessen Größe und Projektion das postoperative Ergebnis beeinflusst. Persönliche Erfahrung und die Betrachtung des Brustvergrößerung als komplexer Prozess tragen entscheidend zum Erfolg der Operation bei. Bei der präoperativen Planung muss insbesondere die gewünschte Größe genaustens abgeklärt werden. Auch wenn die gewonnen Daten durch äußere Gegebenheiten und der erstmaligen Anwendung der Methodik limitiert sind, so können die gewonnenen Aufschlüsse über die Volumenverteilung in den einzelnen Quadranten bei steigender Implantatgröße bei der präoperativen behilflich sein und dazu beitragen, nach Möglichkeit ein OP-Resultat zu erzielen, das der Vorstellung des Patienten vollständig entspricht.

With the decision to have a breast augmentation, the patient develops a precise idea of the postoperative result, the realization of which should be the primary goal of the surgeon. The choice of the implant shape (“round” or “anatomical”) is one of the most important, but also one of the most controversial decisions in the preoperative planning of an aesthetic breast augmentation. In this work, the question of whether and to what extent the choice of the implant influences the surgical result was addressed for the first time by means of objective calculations based on data obtained intraoperatively. Ten subjects with aesthetic augmentation or augmentation followed by mastopexy without prior breast surgery, epilepsy, or greater than or equal to grade III breast ptosis were included in the study. After the preparation of the implant site, six different breast implants, which differ in shape and size (3x "round" and 3x "anatomical", each with "small", "medium" and large" volume) were inserted intraoperatively in the patients. An intraoperative 3D model was generated before the first incision and after each insertion of the implant pairs and temporary wound closure using staples. The scan was performed with the patient seated and the upper body raised by 45°. The 3D scanner type Artec EVA from the company Artec (Luxembourg) was used here, which measurement accuracy could be proven [64]. The 65 3D scans (130 after separating them into right and left breasts) were analyzed using Artec Studio 12 Professional software. On the one hand, linear measured values were determined based on anatomical landmarks. The distances of round and anatomical implants were compared by taking the difference, with no significant discrepancy between the linear measurements being found. Values are approaching zero, suggesting that shape is not a critical factor in route measurement. On the other hand, the software's "measurement" tool is able to automatically calculate the mean distance between two superimposed scans in millimeters (mm).
This mean distance mean value between the "native" scan and the "implanted" scan indicates the influence of the implants on the topography of the breast in mm and was referred to as "topographic shift" (TS) in the thesis. If the topography of the entire breast is considered without subdivisions, no significant difference can be found between the implant shapes "round" and "anatomical" (p = 0.900) but between the implant sizes (p < 0.001). With increasing projection, the "topographic shift" and the influence on the external appearance increases. In order to be able to better localize the increase in volume in the further calculations, the breast was divided into four quadrants and the "topographic shift" was generated for each of them. Here also no significant difference in shape (p = 0.88) can be recorded within the respective quadrant. When considering all scans, there is a significant difference between the "topographic shift" (p < 0.001) of the four quadrants. With breast augmentation, patients gain volume, especially in the upper pole of the breast. Small volumes affect quadrants I and II first. Quadrant III is influenced as the implant volume increases. Quadrant IV is hardly influenced by the augmentation and does not correlate with the implant size. Finally, the extent to which round and anatomical implants affect the position of the nipple was examined. In 48 of 54 cases, anatomical and round implants corresponded with regard to the nipple position in the quadrants [2x quadrant I, 46x quadrant II, 0x quadrant III]. In six cases, the nipple was shifted to another quadrant due to the insertion of the implants with different shapes.
Conclusion:
In conclusion, it can be said that the choice of implant shape affects the postoperative result less than its size and projection. Personal experience and considering breast augmentation as a complex process are key to the success of the surgery. In the preoperative planning, the desired size in particular must be clarified in detail. Even if the data obtained are limited by external conditions and the first application of the method, the information obtained about the volume distribution in the individual quadrants with increasing implant size can be helpful in the preoperative and contribute to achieving a surgical result if possible that fully corresponds to the patient's expectations.
