Abstract (German)
Ziel der Dissertation ist es als Bestandsaufnahme herauszufinden, wie die aktuelle Herangehensweise der Initialdiagnostik des Unterbauchschmerzes und im Speziellen der Appendizitisdiagnostik am Uniklinikum Regensburg ist, das als Maximalversorger über alle Ressourcen verfügt, ohne, dass ein expliziter SOP existiert. Ausgewertet wurden 532 Patientenakten im Zeitraum von 2015-2017 mithilfe von SPSS in der 24. Version. Dazu zählten Daten der allgemeinen Anamnese, klinischen Untersuchung (Appendizitis-Parameter), Labordiagnostik, Bildgebung (Sonographie, CT, MRT), Vergleich von Verdachts- und Entlassdiagnosen und Daten zur operativen Versorgung. Eine klinische Untersuchung wurde bei 421 Patienten durchgeführt, wobei am häufigsten der McBurney-Punkt pathologisch auftrat. Alle Parameter waren im Bezug zur Appendizitis signifikant. Bei der Standardlaboruntersuchung zeigten sich die Werte von CRP und Leukozyten im Zusammenhang mit Appendizitis signifikant erhöht. Eine Sonographie wurde bei 425 Patienten dokumentiert und erreichte für die Appendizitisdiagnostik eine Sensitivität von 70,8 % und Spezifität von 94,6 %. Eine CT bekamen 121 Patienten mit einem pathologischen Befund von 87,6 % und einer Sensitivität von 87,5 % und Spezifität von 98,1 % für die Appendizitisdiagnostik. Eine MRT wurde bei 56 Patienten durchgeführt, wobei sich bei 62,5 % eine Pathologie zeigte und die Sensitivität bzw. Spezifität für die Appendizitisdiagnostik 33, 3 % bzw. 100 % betrug. Operativ versorgt wurden 113 Patienten. Retrospektiv betrachtet hatten 78 Patienten eine Appendizitis. 89,7 % der Patienten wurden rein laparoskopisch appendektomiert und in 10,3 % war eine Konversation zum offenen Vorgehen nötig. Insgesamt wurden 248 Patienten stationär aufgenommen und die Verdachtsdiagnose bestätigte sich in 85,1 %. Zusammenfassend kann die Empfehlung gegeben werden, dass immer eine klinische Untersuchung inklusive möglichst aller Parameter zur Appendizitisdiagnostik sowie eine Standardlaboruntersuchung durchgeführt werden sollte. Wenn möglich, sollte zusätzlich eine Temperaturmessung, ein Urinstatus und bei Frauen im gebärfähigen Alter ein Schwangerschaftstest durchgeführt werden. Als Bildgebung der ersten Wahl empfiehlt sich die Sonographie. Aufgrund der Strahlenbelastung sollte nur bei Unklarheiten eine CT durchgeführt werden. Eine MRT ist eine mögliche Alternative bei Schwangeren und Kindern, wobei dies in der Notfalldiagnostik einen untergeordneten Stellenwert hat. Die Laparoskopie hat sich mittlerweile als Standardtherapie etabliert. In Ausnahmefällen kann bei unkomplizierter Appendizitis eine Antibiotikatherapie durchgeführt werden.

The aim of the dissertation is to take stock of the current approach to initial diagnosis of lower abdominal pain and, in particular, appendicitis diagnosis at the Regensburg University Hospital, which, as a maximum care hospital, has all the resources at its disposal without an explicit SOP. 532 patient records in the period 2015-2017 were evaluated using SPSS in its 24th version. This included data from general history, clinical examination (appendicitis parameters), laboratory diagnostics, imaging (sonography, CT, MRI), comparison of suspected and discharge diagnoses, and data on surgical care. A clinical examination was performed in 421 patients, with McBurney point pathology being the most common. All parameters were significant in relation to appendicitis. Standard laboratory examination showed significantly elevated levels of CRP and leukocytes in relation to appendicitis. Sonography was documented in 425 patients and achieved a sensitivity of 70.8% and specificity of 94.6% for appendicitis diagnosis. A CT was obtained in 121 patients with a pathological finding of 87.6% and a sensitivity of 87.5% and specificity of 98.1% for appendicitis diagnosis. MRI was performed in 56 patients, showing pathology in 62.5% and sensitivity and specificity for appendicitis diagnosis of 33, 3% and 100%, respectively. Surgical treatment was performed in 113 patients. Retrospectively, 78 patients had appendicitis, with 89.7% undergoing pure laparoscopic appendectomy and 10.3% requiring conversation to the open approach. A total of 248 patients were hospitalized and the suspected diagnosis was confirmed in 85.1%. In conclusion, it can be recommended that a clinical examination including, if possible, all parameters for appendicitis diagnosis as well as a standard laboratory examination should always be performed. If possible, a temperature measurement, a urine status and, in women of childbearing age, a pregnancy test should also be performed. Sonography is recommended as the imaging of first choice. Due to the radiation exposure, CT should only be performed if it is unclear. MRI is a possible alternative in pregnant women and children, although this is of secondary importance in emergency diagnostics. Laparoscopy has now become established as standard therapy. In exceptional cases, antibiotic therapy can be performed for uncomplicated appendicitis.
