Neurinome bzw. Schwannome sind die häufigsten primären Wirbelsäulentumore und machen etwa ein Drittel aller primären spinalen Neubildungen aus. Die in der Regel benignen, von Schwann-Zellen ausgehenden Tumore können sowohl Schmerzen als auch sensible und motorische Defizite verursachen. Die Magnetresonanztomographie ist der Goldstandard für die Diagnosestellung und eignet sich optimal für Nachsorgeuntersuchungen. Die chirurgische Tumorresektion stellt die einzige effektive Behandlungsmethode dar. Die operative Entfernung der spinalen Neurinome hat bei einem Großteil der Patienten einen positiven Einfluss auf den Verlauf der Schmerzsymptomatik, die Funktionalität sowie die Lebensqualität. Dies konnte auch in der vorliegenden Studie bestätigt werden. So konnten sowohl eine Schmerzminderung als auch ein Rückgang der fokalneurologischen Defizite bei den meisten Patienten des Studienkollektivs beobachtet werden. Die Entscheidung, ob den Schwannomen zugehörige Nervenwurzeln erhalten oder abgesetzt werden sollen bzw. können, kann durch intraoperatives elektrophysiologisches Monitoring (IOM) erleichtert werden. Der Nutzen des Monitorings wird im Bereich von extramedullären Neubildungen bisher kontrovers diskutiert. Das Patientenkollektiv der vorliegenden Studie konnte vom Gebrauch des intraoperativen Monitorings profitieren. Die mithilfe des IOM operierten Personen hatten einen signifikanten Vorteil gegenüber den ohne IOM operierten Patienten bezüglich neuer postoperativer Defizite.

Translation of the abstract (English)

Neurinomas or schwannomas are the most common primary spinal tumours and account for about one third of all primary spinal neoplasms. The tumours, which are usually benign and originate from Schwann cells, can cause pain as well as sensory and motor deficits. Magnetic resonance imaging is the gold standard for diagnosis and is ideal for follow-up examinations. Surgical tumour resection is the only effective treatment method. The surgical removal of spinal neurinomas has a positive influence on the course of pain symptoms, functionality and quality of life in the majority of patients. This was also confirmed in the present study. A reduction in pain as well as a decrease in focal neurological deficits could be observed in most patients of the study collective. The decision as to whether nerve roots associated with schwannomas should or can be retained or removed can be facilitated by intraoperative electrophysiological monitoring (IOM). The benefit of monitoring in the field of extramedullary neoplasms has been controversially discussed so far. The patient population of the present study could benefit from the use of intraoperative monitoring. Those operated on with the aid of IOM had a significant advantage over patients operated on without IOM with regard to new postoperative deficits.
