Der Infektionsmarker Procalcitonin wird in der Thoraxchirurgie bisher noch nicht als Routineparameter in der Beurteilung des postoperativen Verlaufs verwendet. Ziel dieser prospektiven Studie war es, PCT als quantitativen Marker für Infektionen, die im Rahmen von anatomischen thoraxchirurgischen Eingriffen auftreten, dem etablierten Infektionsparameter CRP und Leukozyten gegenüberzustellen und seine Rolle in der frühzeitigen Erfassung von postoperativen Infektionen zu evaluieren.
An der Abteilung für Thoraxchirurgie der Universität Regensburg wurden im Zeitraum vom 01/19 bis 12/20, bei 90 konsekutiv behandelten Patienten mit Durchschnittsalter von 65,19 Jahre, 60 (66,6%) männlich und 30 (33,3%) weiblich, klinisch relevante
Entzündungsparameter prä- und postoperativ nach dem Studienprotokoll erhoben
(Procalcitonin, C-reaktives Protein, Leukozyten). Insgesamt gestaltete sich der postoperative Verlauf bei 87 (96,9 %) Eingriffen als komplikationslos. Drei Patienten (3,3 %) hatten eine postoperative Pneumonie in der frühpostoperativen Phase.
Allein das Operationstrauma selbst, kann bei völlig komplikationsfreiem postoperativem Verlauf eine starke Leukozytenzahl- und CrP-Wert-Erhöhung in den ersten Tagen verursachen. Die einzige Ausnahme stellt der PCT Wert mit selteneren und wenn, dann nur geringeren Veränderung dar. Im Gegensatz dazu traten deutlich erhöhte PCT-Konzentrationen bei postoperativen Pneumonien auf. Bei der geringen Patientenanzahl mit Komplikationen konnte kein Cut-Off Wert erstellt werden.
Die Ergebnisse der Studie bestätigen die Kinetik des Procalcitonins und des C-reaktiven Proteins. Ein Anstieg auf das Maximum erfolgt beim PCT bereits 24 Stunden nach dem operativen Eingriff. Beim CrP kann dieser Anstieg bis zum 3. postoperativen Tag andauern.
Ein statistisch signifikanter Zusammenhang zwischen der Operationsdauer, Resektionsart und dem Ausmaß der Parameterveränderungen konnte jedoch nicht nachgewiesen werden
In dieser Arbeit konnte gezeigt werden, dass PCT als klinischer Marker einen hohen Stellenwert in der postoperativen Überwachung von Patienten nach thoraxchirurgischen Eingriffen einnehmen kann. Das PCT scheint somit ein vielversprechender Serum-Marker mit großem Potenzial in der Prädiktion postoperativer septischer Komplikationen insbesondere der Pneumonien zu sein.

Translation of the abstract (English)

The infection marker procalcitonin has not yet been used as a routine parameter in thoracic surgery to assess the postoperative course. The aim of this prospective study was to compare PCT as a quantitative marker for infections that occur during anatomical thoracic surgery with the established infection parameters CRP and leukocytes and to evaluate its role in the early detection of postoperative infections.
At the Department of Thoracic Surgery at the University of Regensburg, in the period from 01/19 to 12/20, in 90 consecutively treated patients with an average age of 65.19 years, 60 (66.6%) were male and 30 (33.3%) were female , clinically relevant
Inflammation parameters pre- and postoperatively collected according to the study protocol
(Procalcitonin, C-reactive protein, leukocytes). Overall, the postoperative course was uncomplicated in 87 (96.9%) interventions. Three patients (3.3%) had postoperative pneumonia in the early postoperative period.
If the postoperative course is completely free of complications, the surgical trauma itself can cause a strong increase in the number of leukocytes and CrP values in the first few days. The only exception is the PCT value with rarer and if so only minor changes. In contrast, significantly increased PCT concentrations occurred in postoperative pneumonia. Given the small number of patients with complications, no cut-off value could be established.
The results of the study confirm the kinetics of procalcitonin and C-reactive protein. An increase to the maximum occurs with PCT already 24 hours after the surgical intervention. In the case of CrP, this increase can last up to the 3rd postoperative day.
However, a statistically significant connection between the duration of the operation, the type of resection and the extent of the parameter changes could not be demonstrated
In this work it could be shown that PCT as a clinical marker can play an important role in the postoperative monitoring of patients after thoracic surgery. The PCT thus seems to be a promising serum marker with great potential in the prediction of postoperative septic complications, especially pneumonia.
