Abstract (German)

Neben Blutungen stellen thrombotische Ablagerungen ein bisher nur unzureichend geklärtes Problem der ECMO-Therapie dar. Diese haben eine Verschlechterung der Gasaustauschleistung der Oxygenatormembran zur Folge. Neben Blutungen stellen thrombotische Ablagerungen ein bisher nur unzureichend geklärtes Problem der ECMO-Therapie dar. Diese haben eine Verschlechterung der Gasaustauschleistung der Oxygenatormembran zur Folge. Mit der Verwendung eines ECMO-Modells besteht die Möglichkeit, Einzelfaktoren zu betrachten und auf diese Weise spezifisch Informationen bezüglich der Klotbildung zu gewinnen. Mithilfe von Flusskammern, welche mit senkrecht zur Flussrichtung stehenden Gasfasern versehen wurden, wurde eine Klot- beziehungsweise von-Willebrand-Fadenbildung in Blutserum bzw. Blutplasma unter kontrollierten Bedingungen mittels Live-Imaging sichtbar gemacht. Vermutlich kommt es im Rahmen der Plasmaherstellung zu einer Freisetzung thrombogener Mikropartikel, welche zu einer Klotbildung führt. Eine Klotbildung trat auch bei Antikoagulation der Plasmaproben und trotz vorangehender Heparinspülung des Perfusionssystems, welche die Heparinbeschichtung realer ECMO-Systeme simulieren sollte, auf. Die histologische Untersuchung eines isolierten Klots ergab eine Zusammensetzung aus von-Willebrand-Faktor, Fibrinogen und Thrombozyten. Bei der Perfusion mit Blutserum erfolgte der Nachweis von vWF-Fäden und dem Einfluss hoher Scherkräfte.

In addition to bleeding, thrombotic deposits represent a problem of ECMO therapy that has not yet been adequately clarified. These result in a deterioration of the gas exchange performance of the oxygenator membrane. With the use of an ECMO model, it is possible to look at individual factors and in this way gain specific information regarding clot formation. Using flow chambers equipped with gas fibers perpendicular to the flow direction, clot or von Willebrand fiber formation in blood serum or blood plasma was visualized under controlled conditions by means of live imaging. Presumably, a release of thrombogenic microparticles occurs during plasma production, which leads to clot formation. Clot formation also occurred despite anticoagulation of the plasma samples and despite previous heparin flushing of the perfusion system, which was intended to simulate the heparin coating of real ECMO systems. Histological examination of an isolated clot revealed a composition of von Willebrand factor, fibrinogen, and platelets. Perfusion with blood serum showed evidence of vWF threads and the influence of high shear forces.
