In dieser Studie wurde untersucht, ob man anhand einer 18F-Flourdesoxyglucose-Positronen-Emissions-Tomographie/Computertomographie (F-18-FDG-PET/CT)-Aufnahme zeitnah vor Beginn der Immuncheckpoint-Inhibitor (ICI)-Therapie prognostische Aussagen zum Auftreten von autoimmunvermittelter Hepatitis, Colitis und Thyreoiditis treffen kann. Dazu wurden die Standardised Uptake Ratio (SUR)-Werte als Maß für die 18F-Flourdesoxyglucose (F-18-FDG)-Aufnahme und somit den Glukosemetabolismus der einzelnen Organe und Gewebe bei der letzten F-18-FDG-PET/CT-Bildgebung vor Beginn der ICI-Therapie gemessen und auf signifikante Zusammenhänge mit dem Auftreten der genannten autoimmunvermittelten Nebenwirkungen überprüft. Es besteht ein signifikanter Zusammenhang zwischen einer erhöhten F-18-FDG-Anreicherung im Knochenmark und dem Auftreten einer Colitis. Es konnten keine weiteren prognostischen Marker identifiziert werden, mit denen man Hepatitis und Thyreoiditis vorhersagen könnte. Dieselbe Analyse wurde anhand des metabolischen Tumorgesamtvolumen (total Metabolic Tumor Volume, tMTV) vorgenommen, ohne einen signifikanten Zusammenhang zu finden.
Beim Vergleich mit Befunden aus dem in-vitro-Immunmonitoring der Patienten konnte weiterhin noch ein Zusammenhang zwischen dem F-18-FDG-Uptake des Knochenmarks und dem Monozytenanteil und dem Anteil an CD14-positiven Monozyten an der gesamten Monozytenpopulation gefunden werden.
Zusätzlich wurden die Patienten, die eine Pseudoprogression beziehungsweise eine echte Progression zeigten, hinsichtlich der SUR-Werte und des tMTV bei der ersten Aufnahme nach Beginn der ICI-Therapie miteinander verglichen. Dabei konnte allerdings kein signifikanter Zusammenhang beobachtet werden.
Es wurde weiterhin überprüft, ob sich anhand der vor Beginn der ICI-Therapie gemessenen SUR-Werte Aussagen zum Gesamtüberleben treffen lassen. Es konnte dabei aber kein signifikanter Zusammenhang gefunden werden. Ebenfalls wurde untersucht, ob sich anhand des zu verschiedenen Zeitpunkten während des Therapieverlaufs bestimmten tMTV das Gesamtüberleben vorhersagen lässt, ohne einen signifikanten Zusammenhang zu finden.
Weitere Studien sind nötig, um die Ergebnisse dieser Studie zu validieren und um noch offene Fragen zu klären.

Translation of the abstract (English)

This study investigates whether 18F-Flourodesoxyglucose-Positron-Emission-Tomography/Computer Tomography (F-18-FDG-PET/CT)-images captured before the beginning of immune-checkpoint-inhibitor (ICI)-therapy allows to predict autoimmune-related hepatitis, colitis and thyroiditis. To do so the Standardised-Uptake-Ratio (SUR)-values as a measure for 18F-Flourodesoxyglucose (F-18-FDG)-uptake and thus glucose metabolism of the individual organs and tissues captured at the last F-18-FDG-PET/CT-imaging before the beginning of ICI-therapy was measured and compared with the occurrence of the autoimmune-related adverse events mentioned above. A significant connection between raised F-18-FDG-uptake in bone marrow and the occurrence of colitis could be found. However, no further prognostic markers could be identified, which would allow to predict hepatitis and thyroiditis. The same analysis was carried out for the total Metabolic Tumor Volume (tMTV), without finding any significant connection.
Furthermore, when comparing the results obtained from the in-vitro-immunmonitoring of the patients, a connection between the F-18-FDG-Uptake of bone marrow and the share of monocytes, as well as the percentage of CD-14-positive-monocytes of the total monocyte population could be identified.
In addition, patients, that showed pseudoprogression or genuine progression were compared to each other with regards to their SUR-values and their tMTV measured in the first imaging after the beginning of ICI-therapy, without finding any significant connections.
It was also studied whether SUR-values captured before the beginning of ICI-therapy allow to predict overall-survival. However, no significant results could be obtained. We also checked whether tMTV measured at different points of time before or during therapy allows to predict overall-survival, again without finding any significant connections.
Further studies are needed to validate the results of this study and to answer open questions.
