The success of CAR T cell therapy towards solid tumours is so far limited due to the immunosuppressive tumour environment. Transforming growth factor-β (TGF-β) is highly abundant and one of the main causes of an unsuccessful CAR T cell response due to its T cell suppressive properties. We have previously identified that CAR T cell signalling through the CD28 costimulatory domain provides resistance to TGF-β by high amounts of IL-2 released by the stimulated T cells whereas 4-1BB does not. However, accumulating IL-2 in the targeted tumour stimulates regulatory T cells, contributing further to immunosuppression. Therefore, we have developed a Switch Receptor (SWR) that is composed of the TGF-β receptor-II (TGF- βRII) binding domain and the intracellular IL-2 receptor-β chain (IL-2Rb) mimicking IL-2 signalling upon TGF-β engagement. The TGF-βRII-IL-2Rβ SWR converted TGF-β resistance to 4-1BB CAR T cells with increased T cell amplification, cytolytic properties and tumour cell cytotoxicity. The SWR also converted resistance to the OX40 CAR and an IL-2 deficient variant CD28ΔLCK CAR, which were all previously sensitive to TGF-β mediated suppression. Moreover, the SWR showed superiority over the currently used dominant-negative (DN) TGF- β receptor in amplification and tumour eradication after multiple rounds of tumour re- challenging. Signalling through IL-2Rβ, without the presence of the IL-2R γ chain, induced pSTAT5 and this was sufficient enough to convert TGF-β resistance. This study shows that CAR T cells equipped with a TGF-βRII-IL2-Rβ SWR are resistant to TGF-β mediated suppression therefore leading to an enhanced therapeutic effect in the fight against solid tumours.

Der Erfolg der CAR-T-Zelltherapie bei soliden Tumoren ist bisher aufgrund der immunsuppressiven Tumorumgebung begrenzt. Der transformierende Wachstumsfaktor-β (TGF-β) ist in großer Menge vorhanden und aufgrund seiner T-Zell-suppressiven Eigenschaften eine der Hauptursachen für eine erfolglose CAR-T-Zell-Antwort. Wir haben bereits festgestellt, dass die CAR-T-Zell-Signalisierung über die kostimulatorische Domäne CD28 eine Resistenz gegen TGF-β durch hohe Mengen an IL-2 bewirkt, die von den stimulierten T-Zellen freigesetzt werden, während 4-1BB dies nicht tut. Die Anhäufung von IL-2 im Zieltumor stimuliert jedoch regulatorische T-Zellen, was zu einer weiteren Immunsuppression beiträgt. Daher haben wir einen Switch Receptor (SWR) entwickelt, der aus der TGF-β-Rezeptor-II (TGF-βRII)-Bindungsdomäne und der intrazellulären IL-2-Rezeptor-β-Kette (IL-2Rb) besteht und die IL-2-Signalgebung bei TGF-β-Bindung imitiert. Die TGF-βRII-IL-2Rβ-SWR wandelte die TGF-β-Resistenz in 4-1BB-CAR-T-Zellen mit erhöhter T-Zell-Amplifikation, zytolytischen Eigenschaften und Tumorzellzytotoxizität um. Das SWR wandelte auch die Resistenz gegen das OX40-CAR und eine IL-2-defiziente Variante des CD28ΔLCK-CAR um, die alle zuvor empfindlich auf TGF-β-vermittelte Unterdrückung reagierten. Darüber hinaus zeigte sich der SWR dem derzeit verwendeten dominant-negativen (DN) TGF-β-Rezeptor bei der Amplifikation und der Tumorausrottung nach mehreren Runden der Tumorreduktion überlegen. Die Signalisierung durch IL-2Rβ, ohne das Vorhandensein der IL-2R γ-Kette, induzierte pSTAT5 und dies reichte aus, um die TGF-β-Resistenz umzuwandeln. Diese Studie zeigt, dass CAR-T-Zellen, die mit einem TGF-βRII-IL2-Rβ-SWR ausgestattet sind, gegen die TGF-β-vermittelte Unterdrückung resistent sind, was zu einer verbesserten therapeutischen Wirkung im Kampf gegen solide Tumore führt.
