Die Oxygenatorthrombose stellt eine wichtige Komplikation der ECMO-Therapie dar. Ein Mechanismus, der wesentlich an der Ausbildung von Thrombosen beteiligt ist, ist die Fähigkeit Neutrophiler Granulozyten, Neutrophil Extracellular Traps auszubilden.
In der vorliegenden Studie wurden erstmals Neutrophil Extracellular Traps bei auf Gasfasermatten von gebrauchten ECMO-Oxygenatoren adhärierten Blutzellen nachgewiesen. Dazu wurde eine Methode zur morphologischen Identifikation von NETs erarbeitet.
Eine Quantifizierung von NETs konnte nur an einzelnen, nicht in Konglomeraten gelagerten Zellkernen erfolgen. NETs wurden dabei in insgesamt sehr niedriger Frequenz auf nahezu allen Präparaten identifiziert, wobei die Verteilung von NETs zwischen verschiedenen Präparaten selber Oxygenatoren sehr heterogen war. Lagen auf der Gastauschermembran Zellkernkonglomerate ("Clots") vor, so enthielten diese meist NETs.
Es ließen sich keine signifikanten Zusammenhänge der verschiedenen quantitativen und qualitativen NET-Anteile zu ECMO-therapiespezifischen Faktoren wie Kanülierungsart, Oxygenatorlaufzeit, laufender Oxygenatornummer am Patienten, SOFA-Score oder Outcome identifizieren. Es zeigte sich eine negative Korrelation der Zellkerndichte zum Auftreten von NETs an Einzelkernen. Im Vergleich dazu sind kernhaltige zelluläre Ablagerungen homogener zwischen verschiedenen Präparaten selber Oxygenatoren verteilt.
Bezüglich der Dichte der auf der Gastauschermembran adhärierten Zellkerne konnten Unterschiede zwischen verschiedenen Oxygenatormodellen identifiziert werden. Die Kerndichte war in Erstoxygenatoren höher als in Ersatzoxygenatoren nach Oxygenatortausch. Zu anderen therapiespezifischen Faktoren fand sich keine Korrelation.
Zellkerne waren hauptsächlich um die Kreuzungspunkte übereinanderliegender Gasfasern adhäriert. Bezüglich des Auftretens von NETs ergaben sich zwischen den Kreuzungspunkten und deren Zwischenräumen keine Unterschiede.
Für eine weitere Untersuchung erscheint eine systematische Probenentnahme und Untersuchung festgelegter Stellen der Gastauschermembran, sowie eine direkte Korrelation zu im Blut gemessenen NET-Markern sinnvoll.

Translation of the abstract (English)

Oxygenator thrombosis is a significant complication of ECMO therapy. One mechanism, that plays a crucial role in the formation of thrombosis, is the ability of neutrophils to generate neutrophil extracellular traps (NETs).
In this study, neutrophil extracellular traps were detected in blood cells adhered to gas fibers derived from used ECMO oxygenators. A method for the morphological identification of NETs was developed for this purpose.
Quantification of NETs was only possible in single adhered nuclei, not in cell nucleus conglomerates. NETs were identified at an overall very low frequency in almost all specimens, with the distribution of NETs being highly heterogeneous among different specimens of the same oxygenators. When clots were present on the gas exchange membrane, they mostly contained NETs.
No significant correlations were found between various quantitative and qualitative NET counts and ECMO therapy-specific factors such as cannulation type, oxygenator runtime, current oxygenator number on the patient, SOFA score, or outcome. A negative correlation was observed between cell nucleus density and the presence of NETs on individual nuclei. In comparison, nucleus-containing cellular deposits were distributed more homogeneously among different specimens of the same oxygenators.
Differences in the density of adhered cell nuclei on the gas exchange membrane were identified between different oxygenator models. Nucleus density was higher in primary oxygenators compared to replacement oxygenators after oxygenator exchange. No correlations were found with other therapy-specific factors.
Cell nuclei were primarily adhered around the intersections of overlapping gas fibers. There were no differences in the occurrence of NETs between the intersections and their gaps.
For further investigation, a systematic sampling and examination of specific locations on the gas exchange membrane, as well as a direct correlation with NET markers measured in the blood, appear to be advisable.
