Die Weisheitszahnentfernung gehört mittlerweile zu den häufigsten operativen Eingriffen in der Zahnmedizin. Dennoch bleiben die postoperativen Komplikationen nach der Weisheitszahnentfernung vor allem im Unterkiefer ein Problem. Die Wundheilungsstörungen und die damit verbundenen postoperativen Komplikationen wie Schmerzen, Schwellung und Nachblutungen, treten vorrangig dann ein, wenn die Koagulum-Bildung ausbleibt oder ein gebildetes Koagulum durch Fibrinolyse zerstört wird. Um dem Zerfall vorzubeugen, entwickelte die Coltène/Whaledent GmbH das Produkt Gelatamp®. Der kolloides Silber enthaltende Gelatineschwamm dient u.a. der Prophylaxe von Nachblutungen nach Zahn-Extraktionen. Auch wird er präventiv bei Wundinfektionen verwendet. Die klinische Studie stellte eine prospektive, multizentrische, doppel-blinde, randomisierte, kontrollierte Studie dar und wurde durch zwei Patienten-Fragebögen erweitert. Untersucht werden die Behandlungen und postoperativen Analysen von 16 Patienten. Zusammengefasst konnte bei der Anwendung von Gelatamp®, im Gegensatz zu Gelita Spon® eine Reduzierung des Erythems und der Dehiszenz erzielt werden. Begründet werden kann die Abnahme des Erythems durch den Zusatz des kolloidalen Silbers, da die Silberionen eine Gewebsneubildung anregen und sich positiv auf Wundoberflächen und –ränder auswirken. Die Einarbeitung von kolloidalem Silber in die Gelatinestruktur bietet zwei Vorteile. Zum einen verteilt es sich gleichmäßig in der Alveole und zum anderen verhindert es das Auswaschen des kolloidalen Silbers aus dem Schwämmchen. Der antimikrobielle Effekt kann der Grund für die Reduzierung der Dehiszenz sein, welche eine verzögerte oder sekundäre Heilung beschreibt. Der größte Vorteil von dem kolloidalen Silber in Gelatamp® ist wohlmöglich, dass die Mikroorganismen keine Resistenz bilden. So wirkt es zwar wie ein Breitspektrum-Antibiotikum, jedoch ohne bekannte schädliche Nebenwirkung und ohne bekannte Resistenzbildung. Auch hat es eine leichte Handhabung und kann sowohl in Zahnarztpraxen als auch in Kliniken angewendet werden.. Um wissenschaftlich fundiert belegen zu können, dass Gelatamp® als Gelatineschwämmchen sowohl hämostatisch als auch antibakteriell wirkt, sind weitere Untersuchungen mit einem größerem Patientenkollektiv notwendig, da ein größerer Stichprobenumfang u.a. mit einer höheren statistischen Signifikanz einhergeht und somit aussagekräftigere Erkenntnisse erzielt werden können. Diese Tendenz äußert sich primär in dem Minimieren postoperativer Komplikationen.

Wisdom tooth removal is now one of the most common surgical procedures in dentistry. Nevertheless, postoperative complications after wisdom tooth removal remain a problem, especially in the mandible. Wound healing disorders and the associated postoperative complications, such as pain, swelling and secondary bleeding, occur primarily when coagulum formation fails or when a formed coagulum is destroyed by fibrinolysis. To prevent this disintegration, Coltène/Whaledent GmbH developed the product Gelatamp®. The gelatine sponge containing colloidal silver is used, among other things, for the prophylaxis of post-operative bleeding after tooth extractions. It is also used preventively for wound infections. The clinical study was a prospective, multicenter, double-blind, randomized controlled trial and was extended by two patient questionnaires. The treatments and postoperative analyses of 16 patients were examined. In summary, a reduction of erythema and dehiscence could be achieved with the application of Gelatamp®, in contrast to Gelita Spon®. The reduction in erythema can be explained by the addition of colloidal silver, as the silver ions stimulate new tissue formation and have a positive effect on wound surfaces and edges. The incorporation of colloidal silver into the gelatin structure offers two advantages. First, it distributes evenly in the alveolus and second, it prevents the colloidal silver from washing out of the sponge. The antimicrobial effect may be the reason for the reduction of dehiscence, which describes delayed or secondary healing. The biggest advantage of the colloidal silver in Gelatamp® is probably that the microorganisms do not form resistance. Thus, while it acts like a broad-spectrum antibiotic, it has no known harmful side effects and no known development of resistance. It also has ease of use and can be used in dental offices as well as clinics.... In order to be able to prove scientifically that Gelatamp® as a gelatin sponge has both a hemostatic and antibacterial effect, further studies with a larger patient collective are necessary, since a larger sample size is associated, among other things, with a higher statistical significance and thus more meaningful findings can be obtained. This tendency primarily manifests itself in minimizing postoperative complications.
