Hematopoietic stem cells (HSCs) form the basis of the hematopoietic system. HSCs balance self-renewal and differentiation into lineage-committed progenitors which further mature into all types of functional blood cells. However, when somatically acquired mutations disrupt this homeostasis, HSCs undergo clonal expansion, which can initiate in leukemic transformation resulting in malignancies such as acute myeloid leukemia (AML). Several of these mutations disrupt the function of epigenetic regulators which play a crucial role in gene regulation. An important layer of epigenetics is the three-dimensional chromatin structure which includes DNA loops connecting regulatory genomic elements. The cohesin protein ring complex, consisting of SMC1A, SMC3, RAD21 and the variable subunit STAG1 or STAG2, is vital for establishing and maintaining these chromatin contacts. Interestingly, cohesin members are frequently mutated in AML, with mutations primarily affecting the X-chromosomal STAG2 gene. In this thesis, I present the first study comprehensively analyzing transcriptional and epigenetic implications of cohesin-STAG2 mutations directly in primary human AML samples. Our mutational screening of over 350 AML samples revealed STAG2 mutations in 5.9% of cases, while the paralogue STAG1 was never mutated. Comparative analysis of co-occurring mutations revealed a distinct pattern for STAG2-mutant AMLs. Using chromatin immunoprecipitation coupled with sequencing (ChIP-seq) targeting STAG1, STAG2, RAD21, the insulator CTCF and the active enhancer/promoter histone mark H3K27ac, we explored cohesin-associated epigenetic landscapes. We found that STAG2 mutations consistently led to complete loss of STAG2 protein, compensated to some extent by increased STAG1 binding. We identified loci with significantly altered cohesin occupancy and chromatin activation in STAG2-mutant AMLs compared to cohesin-wildtype AMLs. Especially regulatory loci without CTCF, enriched for ETS-transcription factor motifs, which preferentially associate with STAG2-cohesin in cohesin wildtype cells, were insufficiently bound by STAG1-cohesin. Conversely, we also identified positions with STAG1 overcompensation and increased cohesin occupancy. These changes were associated with significant up- or downregulation of corresponding transcripts, as detected by RNA sequencing. To investigate the impact of cohesin-dependent epigenetic alterations on chromatin topology, we performed high-throughput chromosome conformation capture (Hi-C). We observed a significant weakening of looping interactions in STAG2-mutant AMLs, particularly for short-range contacts (<500 Mb), while longer range contacts were often strengthened. Many of the weakened loops were found to be involved in regulatory contacts, where we observed reduced cohesin and H3K27ac occupancy correlating with dysregulated gene expression. To validate our findings in AML samples and explore the leukemogenic potential of STAG2 loss, we established models of STAG2-depletion in primary human hematopoietic stem and progenitor cell (HSPC) cultures derived from cord blood. We observed transcriptomic and epigenetic effects in HSPCs with STAG2 knockdown similar to the changes seen in AMLs, which were not observed upon equivalent depletion of the paralogue STAG1. Especially, downregulated genes, cohesin sites with reduced occupancy and weakened chromatin looping identified in STAG2-mutant AMLs were frequently equivalently altered in STAG2-depleted HSPCs. Notably, the downregulated tumor suppressor gene DACT1 and ITGA9, a regulator of HSC proliferation and differentiation, were among the genes showing cohesin-dependent epigenetic changes in both. Furthermore, we found that depletion of STAG2, but not STAG1, interfered with the differentiation capacity of HSPCs, particularly affecting erythroid colony formation. Compared to control cells, STAG2-deficient HSPCs were shifted towards an immature HSC-like transcriptome while differentiation-associated genes of multiple lineages were downregulated.
In summary, our findings highlight the critical role of STAG2 as a key regulator of cohesin-associated chromatin architecture and gene regulation in the hematopoietic system and establish the gene as a bona fide tumor suppressor of AML. The loss of STAG2 and subsequent switch to STAG1 result in locus-specific changes in cohesin occupancy, enhancer activity, and chromatin looping, which contribute to transcriptomic alterations associated with leukemogenesis. These findings expand our understanding of the molecular mechanisms underlying development of cohesin-mutated malignancies and provide insights into potential therapeutic targets.

Hämatopoetische Stammzellen (HSCs) bilden das Rückgrat des hämatopoetischen Systems. HSCs halten die Balance zwischen Selbsterneuerung und Differenzierung zu Vorläuferzellen, die sich auf eine hämatopoetische Linie festlegen und zu reifen Blutzellen aller Art weiterentwickeln. Wenn jedoch dieses Gleichgewicht durch somatische Mutationen gestört wird, kann es zu klonaler Expansion und letztendlich leukämischer Transformation der HSCs kommen, was sich zum Beispiel in Form einer akuten myeloischen Leukämie (AML) klinisch manifestiert. Viele dieser Mutationen betreffen epigenetische Regulatoren, die zugleich wichtige Rollen in der Genregulation innehaben. Ein wichtiger Teil der Epigenetik ist die dreidimensionale Chromatinstruktur, welche über DNA-Schlaufen den räumlichen Kontakt regulatorischer Genomelemente sicherstellt. Der Cohesin Proteinringkomplex, bestehend aus SMC1A, SMC3, RAD21 und der variablen Untereinheit STAG1 oder STAG2, ist essenziell für die Etablierung und Erhaltung solcher Chromatinkontakte. Interessanterweise sind die Mitglieder dieses Komplexes, allen voran das X-chromosomale STAG2 Gen, häufig von Mutationen in AML betroffen.
In dieser Dissertation präsentiere ich die erste Studie, die umfassend transkriptionelle und epigenetische Implikationen von Cohesin-STAG2 Mutationen direkt in primären humanen AML-Proben analysiert. Unser Mutationsreihentestung von über 350 AMLs ergab, dass STAG2 in ca. 5.9% aller Fälle mutiert war, wohingegen das Paralog STAG1 nie mutiert war. Ein Vergleich des Co-Mutationen zeigte ein einzigartiges Muster für STAG2-mutierte AML. Durch Chromatin Immunpräzipitation gefolgt von DNA-Sequenzierung (ChIP-seq), welche STAG1, STAG2, RAD21, das Isolatorprotein CTCF oder die Histonmodifikation H3K27ac, die aktive Verstärker- oder Promotorelemente markiert, zum Ziel hatte, generierten wir Profile der epigenetischen Landschaft. Hierbei entdeckten wir den kompletten Verlust des STAG2 Proteins, der teilweise durch erhöhte Bindung von STAG1 kompensiert wurde. Allerding fanden wir auch viele Loci mit signifikant veränderter Cohesin Beladung im Vergleich zu AML ohne Cohesin Mutationen. Insbesondere regulatorische Regionen ohne CTCF, welche angereichert sind für die DNA-Erkennungssequenzen von ETS-Transkriptionsfaktoren und in Cohesin Wildtypzellen bevorzugt mit Cohesin-STAG2 besetzt sind, wurden ungenügend von der STAG1-Cohesinvariante gebunden und zeigten Anzeichen erniedrigter Promotoraktivierung. Umgekehrt gab es auch loci mit Überkompensation durch STAG1 und erhöhter Cohesin Akkumulation. Diese Veränderungen waren mit signifikanter erhöhter oder erniedrigter Expression von korrespondierenden Transkripten assoziiert, was wir durch RNA-Sequenzierung ermitteln konnten. Um den Einfluss auf die Cohesin-abhängige Chromatintopologie zu untersuchen, wurde die Hochdurchsatz-Chromosomenkonformationserfassung (Hi-C) Methode durchgeführt. Hierbei beobachteten wir die signifikante Abschwächung zahlreichen Kurzdistanzinteraktionen (<500 Mb) in AML mit STAG2 Mutation, wohingegen Kontakte über längere genomische Distanzen teilweise verstärkt waren. Viele der geschwächten Interaktionen waren an regulatorischen Kontakten beteiligt und wiesen reduzierte Cohesin Präsenz sowie verringerte H3K27ac Histonmarkierung auf und korrelierten mit fehlregulierter Genexpression. Um die Befunde in AML zu valideren und das onkogene Potential von STAG2 zu untersuchen, etablierten wir zelluläre Modelle des Verlusts von STAG2 in primären humanen hämatopoetischen Stamm- und Vorläuferzellen (HSPCs) aus dem Nabelschnurblut. HSPCs, in denen STAG2 experimentell depletiert wurde, wiesen ähnliche transkriptomische und epigenetische Aberrationen auf wie STAG2-mutierte AML, was nicht durch eine gleichstarke Depletion von STAG1 ausgelöst wurde. Vor allem herunterregulierte Gene, sowie genomische Regionen mit erniedrigter Cohesin Bindung und geschwächten Chromatininteraktionen, die in den AML mit STAG2 Mutationen identifiziert wurden, zeigten häufig äquivalente Veränderungen in den STAG2 defizienten HSPCs auf. Unter den herunterregulierten Genen, die im Modell und in Patienten Cohesin-abhängige epigenetische Veränderungen aufwiesen, waren das Tumorsuppressorgen DACT1, sowie ITGA9, ein potenziell wichtiger Regulator der HSC-Vermehrung und Differenzierung. Außerdem stellten wir fest, dass die Differenzierungskapazität, insbesondere die erythroide Entwicklung, durch die Depletion von STAG2, nicht aber durch Verlust von STAG1, beschränkt wurde. Im Vergleich zu Kontrollzellen hatten die STAG2-defizienten HSPCs einen unreiferes, HSC-ähnliches Transkriptom, wohingegen differenzierungsassoziierte Gene verschiedener Linien niedriger exprimiert wurden.
Zusammengefasst, heben unsere Ergebnisse die kritische Rolle von STAG2 als Schlüsselregulator der Cohesin-assoziierten Chromatinarchitektur und Genregulation im Hämatopoetischen System hervor und etablieren STAG2 als bona fide Tumorsuppressor der AML. Der Verlust von STAG2- und darauf folgende Wechsel zu STAG1-Cohesin induziert Lokus-spezifische Veränderungen der Cohesin Beladung und Promotoraktivität, was zur Fehlregulation von Genen, die eine Rolle in der AML-Entstehung spielen, beiträgt. Im Allgemeinen erweitern diese Ergebnisse unser Verständnis der molekularen Mechanismen, die Cohesin-mutierten Leukämien zugrunde liegen and ermöglichen auch Ausblicke auf zukünftige therapeutische Optionen.
