Das thorakoabdominale Aortenaneurysma ist eine seltene potenziell
lebensbedrohliche Erkrankung mit einer Inzidenz von 5,9 Neuerkrankungen pro
100.000 Personen pro Jahr (94). Seit dem Ende der 80-er Jahren steht neben der offen-chirurgischen Therapie in der Behandlung von Aortenaneurysmen auch eine endovaskuläre Therapie (EVAR) zu Verfügung. Vom Einsatz eines minimal-invasiven EVAR-Verfahrens profitieren in erster Linie symptomatische und multimorbide Patienten (96). Deshalb war eine Standardisierung des Behandlungsverfahrens von besonderer Wichtigkeit (72, 84). Standardverzweigte Stentgrafts als Alternative zu custom made Aortenstentgrafts kommen an der Universität Regensburg seit 2008 zum
Einsatz.
Diese Studie vergleicht zwei Therapieoptionen zur Versorgung von
thorakoabdominalen Aortenaneurysmen. Um diese Aufgabe zu erfüllen, wurden 155 Patienten, die von Januar 2008 bis Dezember 2016 am Universitätsklinikum Regensburg wegen eines TAAA behandelt wurden, ausselektiert. 71 Patienten erhielten eine Standard- und 84 Patienten eine maßgeschneiderte Endostentprothese.
Von den 155 Patienten waren 17 % (n=27) symptomatisch, 4 % (n=6) wurden wegen eines rupturierten Aortenaneurysma behandelt.
Der Beobachtungszeitraum betrug in der std-BEVAR-Gruppe 27 Monate und war damit signifikant kürzer als die 40 Monate in der cmd-BEVAR-Gruppe. Der technische Erfolg lag bei 97,2 % in der std-BEVAR- vs. 98,8 % in der cmd-BEVAR-Gruppe. Die 30-Tage Mortalität in der std-BEVAR-Gruppe lag bei 6 % und in der cmd-BEVARGruppe bei 8 %. In beiden Gruppen sind postoperative neurologische Komplikationen (SCI) (14 %, (n=10) in der std-BEVAR vs. 8 % (n=7) in der cmd-BEVAR-Gruppe) aufgetreten. Bei 4 % (n=3) in der std-BEVAR- und 7 % (n=6) in der cmd-BEVARGruppe kam es postoperativ zu einer Darmischämie und als Folge zu einer Teilresektion des Darmes. Infolgedessen sind alle neun Patienten im selben Krankenhausaufenthalt verstorben. Bei 20 % (n=14) der Patienten in der std-BEVARund 17 % (n=14) in der cmd-BEVAR-Gruppe kam es postoperativ zu einem akuten Nierenversagen. Dies führte in 13 % (n=9) vs. 11 % (n=9) der Fälle zu einer bleibenden Hämodialyse. In der std-BEVAR-Gruppe sind 14 von 267 (5 %) vs. 15 von 263 (6 %) in der cmd-BEVAR-Gruppe Stentgraft-Komplikationen aufgetreten. Die meisten davon waren BSG-Stenosen (n=10 (4 %) in der std-BEVAR- und n=11 (4 %) 84 in der cmd-BEVAR-Gruppe). Reinterventionsrate in der std-BEVAR-Gruppe lag bei 24
% (n=17) und in der cmd-BEVAR-Gruppe bei 42 % (n=35). Eine statistische Vergleichsanalyse der std-BEVAR vs. cmd-BEVAR hat gezeigt, dass
die Standardprothesen mit den Spezialprothesen vergleichbar sind. Signifikante
Unterschiede zwischen den beiden Gruppen hat es bei der Indikationsstellung für die TAAA-Versorgung gegeben. Sprich alle Patienten (n=6) mit rupturierten- und 21 von 27 mit den symptomatischen TAAA haben eine rasch verfügbare Standardprothese bekommen. Ein weiterer signifikanter Unterschied zwischen den beiden Gruppen wurde in Bezug auf die Aneurysmadurchmesser festgestellt. Die Patienten mit größerem Aneurysmaumfang haben eine sofort verfügbare Standartdprothese erhalten. Die standard-gebranchten Endostentprothesen können ohne lange Wartezeiten sowohl in Notfällen als auch bei elektiven Eingriffen eingesetzt werden.
Die Fünf-Jahres-Überlebensrate lag in der std-BEVAR- bei 47 % vs. 52 % in der cmd-BEVAR-Gruppe ohne signifikante Unterschiede zwischen den beiden Gruppen. Die erhöhte Sterblichkeit nach Versorgung eines thorakoabdominalen Aortenaneurysma ist auf die erhöhte präoperative Morbidität zurückzuführen (p=0,002). Der Einsatz von standardverzweigten Aortenprothesen in der Versorgung von komplexen thorakoabdominallen Aortenaneurysmen etabliert sich zunehmend als Therapie der ersten Wahl. Sowohl die standardgebranchte als auch individuell angefertigte gebranchte Stentgrafts befinden sich im Weiterentwicklungsstadium, sodass künftig auch anspruchsvolle anatomische Verhältnisse der Zugangsgefäße, der Aorta selbst und den renoviszerallen Gefäßen behandelt werden können. Eine wissenschaftliche Bewertung der Gesamteffektivität von off-the-shelf vs. custom made Aortenprothesen kann durch Langzeitstudien mit größeren Patientenkollektiven erreicht werden.

Thorakoabdominal aortic aneurysm (TAAA) is a rare, potentially fatal condition with an incidence rate of 5.9 new cases per 100,000 people per year (94). Since the 1980s, an endovascular therapy (EVAR) has been made available in addition to open surgery for the treatment of aortenaneurysms. Patients who are symptomatic and multimorbid stand to gain the most from using a EVAR procedure (96). Therefore, standardizing the treatment process was extremely important (72, 84). Since 2008, Regensburg University has begun using off-the-self endografts as an alternative to custom made aortenstentgrafts.
This study compares two treatment options for treating thorakoabdominal aortic aneurysms. For this purpose, 155 patients who received a off-the-shelf multibranched endograft (n=71) or custom made (n=84) endograft from the University Clinic Regensburg's department of facial surgery between January 2008 and December 2016 were included. 17% (n=27) of the 155 patients had symptoms, and 4% (n=6) needed treatment for an aortenaneurysm that had ruptured.
The follow up was 27 months in the std-BEVAR group and significantly shorter than the 40 months in the cmd-BEVAR group. The technical success rate was 97.2% in the std-BEVAR group against 98.8% in the cmd-BEVAR group. The std-BEVAR group's 30-day mortality was 6%, while the cmd-BEVAR group's was 8%. Postoperative neurologic complications (SCI) have been reported in both groups, with rates of 14% (n=10) in the std-BEVAR group and 8% (n=7) in the cmd-BEVAR group. Intestinal ischemia occurred postoperatively in 4% (n=3) of the std-BEVAR group and 7% (n=6) of the cmd-BEVAR group, leading to a partial resection of the intestine. As a result, all nine patients passed away in the same hospitalization. Postoperatively, there was an acute kidney injury in 20% (n=14) of patients in the std-BEVAR group and 17% (n=14) of patients in the cmd-BEVAR group. In 13% (n=9) as opposed to 11% (n=9) of the cases, this resulted in a persistent hemodialysis. Stentgraft complications were reported in 14 of 267 (5%) in the std-BEVAR group, compared to 15 of 263 (6%) in the cmd-BEVAR group. The majority of these were stenosis of bridging stent graft (n=10 (4%) in the std-BEVAR group and n=11 (4%) in the cmd-BEVAR group). Reintervention rates were 24% (n=17) in the std-BEVAR group and 42% (n=35) in the cmd-BEVAR group.
The statistical comparison of std-BEVAR vs. cmd-BEVAR has shown that the off-the-self multibranched aortic stent graft and custom made aortic stent graft are comparable. There were significant differences between the two groups regarding the indication for surgery on TAAA. All patients with ruptured TAAA (n = 6) and 21 of 27 patients with symptomatic TAAA received a off-the-self multibranched endograft. There were significant differences between the two groups in aortic aneurysm diameter. The patients with a larger aneurysm diameter received a off-the-self stent graft. This off-the-self graft can be used immediately in emergency situations and for elective surgery. The five-year survival rate was 47% in the standard BEVAR group and 52% in the cmd-BEVAR group, with no significant differences between the two groups. The increased preoperative morbidity is the cause of the increased postoperative mortality after treatment of a TAAA (p=0.002).
Standard stent graft in the treatment of complex TAAA is increasingly used as first-line therapy. Both the standard branched and custom-made branched stent grafts are in the development phase so that complex anatomical conditions of the access arteries and the aorta can be treated in the future. Through long-term studies including larger patient populations, it is possible to arrive at a scientific assessment of the overall effectiveness of commercially available versus custom-made aortenprostheses.
