Morbus Stargardt ist eine hereditäre Erkrankung des hinteren Augenabschnittes, die sich klinisch mit einem chronisch progressiven Visusverlust präsentiert. Die molekulargenetische Diagnostik dieser Erkrankung wird durch eine hohe genotypische und phänotypische Heterogenität erschwert. Dies liegt unter anderem darin begründet, dass sich die krankheits-verursachenden Mutationen in den intronischen Bereichen des ABCA4 befinden können, welche bei einer routinemäßigen DNA-Diagnostik der kodierenden Gensequenzen nicht erfasst werden. Das Ziel der vorliegenden Arbeit war es eine Methode zu entwickeln, die es erlaubt das ABCA4 Transkript aus den peripher mononukleären Zellen des Blutes von Patienten zu gewinnen, obwohl das Gen dort regulär nicht exprimiert wird. Dies würde es ermöglichen den Einfluss von Mutationen in den nicht kodierenden Bereichen direkt auf Transkriptebene im Blut der Patienten nachzuweisen. Eine vielversprechende Möglichkeit das Ziel der Arbeit umzusetzen, basiert auf dem CRISPR/Cas9-System.

Stargardt disease is a hereditary disease of the posterior segment of the eye that presents clinically with a chronic progressive loss of visual acuity. Stargardt disease is a hereditary disease of the posterior segment of the eye that presents clinically with a chronic progressive loss of visual acuity. The molecular genetic diagnosis of this disease is complicated by a high degree of genotypic and phenotypic heterogeneity. This is partly due to the fact that the disease-causing mutations may be located in the intronic regions of ABCA4, which are not detected by routine DNA diagnostics of the coding gene sequences. The aim of the present work was to develop a method that allows the ABCA4 transcript to be extracted from the peripheral blood mononuclear cells of patients, even though the gene is not regularly expressed there. This would make it possible to detect the influence of mutations in the non-coding regions directly at the transcript level in the patient's blood. The CRISPR/Cas9 system is a promising way of realizing the aim of the work.
