Erwachsene Patienten mit transversaler Enge des Oberkiefers, ein- oder beidseitiger Kreuzbisssituation und Frontengstand bilden ein besonderes Kollektiv in der Mund-, Kieferund Gesichtschirurgie. Die chirurgisch unterstützte Gaumennahterweiterung und die LeFort-IZweisegmentosteotomie sind alternative Behandlungsverfahren zur Behebung einer Schmalkiefersituation. In dieser vergleichenden Studie wurde erstmals eine Methodik erarbeitet, um die Reossifikation nach chirurgisch unterstützter Gaumennahterweiterung und LeFort-I-Zweisegmentosteotomie in der Digitalen Volumentomographie zu beurteilten. Ziel war die quantitative Streckenmessung der erfolgten Weitung im Oberkiefer für beide Verfahren sowie die qualitative Beurteilung und Kategorisierung der Reossifikation mindestens sechs Monate postoperativ unter Nutzung der Digitalen Volumentomographie. Der erstellte Datensatz umfasst ein Patientenkollektiv des Universitätsklinikums Regenburg mit 30 Patienten, die aufgrund einer transversalen Enge im Oberkiefer behandelt wurden. Die quantitative Streckenmessung ergab für die Gruppe der chirurgisch unterstützen Gaumennahterweiterung im anterioren Segment eine durchschnittliche Weitung von 4,29 Millimeter (SD=2,28) und im posterioren Segment eine durchschnittliche Weitung von 1.94 Millimeter (SD=1,05). In der Gruppe der LeFort-I-Zweisegmentosteotomie wurde eine posteriore durchschnittliche Weitung von 3,28 Millimeter (SD=1,64) gemessen. Bei 2 Patienten war in der Digitalen Volumentomographie sechs Monate nach chirurgisch unterstützter Gaumennahterweiterung keine Verknöcherung zu beobachten, bei 6 Patienten eine vollständige Verknöcherung. 3 Patienten zeigten eine partielle oder fortgeschritten Verknöcherung. In der Gruppe der LeFort-I-Zweisegmentosteotomie wurde bei lediglich einem Patienten eine vollständige Verknöcherung beobachtet, bei 4 Patienten keine Verknöcherung und bei 14 eine partielle oder fortgeschrittene. 52 Darüber hinaus wurde ein Zusammenhang zwischen geweiteter Strecke und Reossifikationsfortschritt nach LeFort-I-Zweisegmentosteotomie beobachtet, welcher jedoch noch mit größeren Fallzahlen statistisch verifiziert werden muss. Die Digitale Volumentomographie hat sich als reproduzierbares und präzises Instrument zur quantitativen Streckenmessung und der qualitativen Reossifikationsbeurteilung herausgestellt.

Translation of the abstract (English)

Adult patients with transverse maxillary crowding, unilateral or bilateral crossbite and anterior crowding form a special group in oral and maxillofacial surgery. Surgically assisted palatal expansion and the LeFort two-segment osteotomy are alternative treatment methods for correcting a narrow jaw situation. In this comparative study, a methodology was developed for the first time to assess reossification after surgically assisted palatal expansion and LeFort I two-segment osteotomy in digital volume tomography. The aim was to quantitatively measure the distance of widening in the maxilla for both procedures and to qualitatively assess and categorize the reossification at least six months postoperatively using digital volume tomography. The data set comprises a patient collective from the University Hospital Regenburg with 30 patients who were treated for a transversal narrowing of the maxilla. The quantitative distance measurement revealed an average expansion of 4.29 millimeters (SD=2.28) in the anterior segment and an average expansion of 1.94 millimeters (SD=1.05) in the posterior segment for the surgically assisted palatal expansion group. In the LeFort I two-segment osteotomy group, an average posterior dilation of 3.28 millimeters (SD=1.64) was measured. Digital volume tomography showed no ossification in 2 patients six months after surgically assisted palatal expansion and complete ossification in 6 patients. 3 patients showed partial or advanced ossification. In the LeFort I two-segment osteotomy group, complete ossification was observed in only one patient, no ossification in 4 patients, and partial or advanced ossification in 14 patients. 52 In addition, a correlation was observed between the widened segment and reossification progress after LeFort I two-segment osteotomy, but this still needs to be statistically verified with larger numbers of cases. Digital volume tomography has proven to be a reproducible and precise instrument for quantitative distance measurement and qualitative reossification assessment.
