Abstract (German)
Thema und Forschungsfrage: Diese Dissertation untersucht die Ergebnisse der endoskopischen Therapie (endoskopische Submukosadissektion [ESD] und endoskopische Mukosaresektion [EMR]) bei frühen Neoplasien des oberen Gastrointestinaltrakts. Im Fokus stehen die Faktoren Remission, Rezidivrate, Komplikationen und Überleben bei Neoplasien im Ösophagus (low-grade intraepithelial neoplasia [LGIN], high-grade intraepithelial neoplasia [HGIN], frühe Barrett-Adenokarzinomen, frühe Plattenepithelkarzinome) sowie im Magen (Magenfrühkarzinome).
Methodologie: In dieser retrospektiven Studie wurden die Daten von Patienten aufgenommen, die zwischen 2011 und 2020 in der Klinik für Gastroenterologie der Barmherzigen Brüder in Regensburg interventionell endoskopisch behandelt wurden.
Ergebnisse:
• Remission der Neoplasie: Eine komplette Remission wurde in 98% der Fälle erreicht (97,7% bei Barrett-Neoplasien [BN], 100% bei frühen Magenkarzinomen [EGC]). In 2% der Fälle konnte keine komplette Remission erzielt werden.
• Rezidivfreies Überleben: Über eine gesamte Follow-up-Zeit von 3284 Monaten wurden 11 Rezidive verzeichnet (6 bei BN, 4 bei EGC). Vier Todesfälle wurden registriert, wobei nur einer tumor-assoziiert war. Das 5-Jahres-rezidivfreie Überleben (RFS) betrug 77,8% für die gesamte Stichprobe, 79,6% für BN und 65,8% für EGC. Höhere Rezidivraten wurden bei Barrett-Karzinomen in höheren T1a-Stadien, horizontalen R1-Resektionen und Piecemeal-Resektionen festgestellt.
• Vergleich der endoskopischen Resektionstechniken (EMR/ESD): EMR wies eine höhere Rate horizontaler R1-Resektionen (39,7%) im Vergleich zu ESD (14,1%) auf. Die En-bloc-Resektionsrate war bei ESD (98,7%) höher als bei EMR (28,2%). Die gesamte Stichprobe zeigte nach ESD eine höhere Rezidivrate als nach EMR. Wenn nur „echte“ Rezidive berücksichtigt wurden, waren die Raten nahezu gleich. Bei Barrett-Neoplasien zeigte sich ein umgekehrter Effekt auf das 5-Jahres-RFS: 80,0% in der EMR-Gruppe und 90,0% in der ESD-Gruppe.
• Komplikationen bei endoskopischer Therapie: Leichte Komplikationen (Stenosen oder leichte Blutungen) traten in 27 Fällen (18%) auf, schwere Komplikationen (Perforationen und starke Blutungen) in 8 Fällen (5,3%). Schwere Komplikationen waren bei ESD häufiger (6,7%) als bei EMR (4,2%).
Schlussfolgerungen: Die Ergebnisse dieser Studie sind vergleichbar mit anderen nationalen und internationalen Studien und bestätigen die Sicherheit und Effektivität der endoskopischen Therapie bei frühen Neoplasien des oberen Gastrointestinaltrakts. Eine hohe Rate an Stenosen nach Ablation der residualen Barrett-Schleimhaut nach endoskopischer Resektion wurde beobachtet, was die Entwicklung von Strategien zur Vermeidung notwendig macht. ESD erwies sich in vielen Faktoren der EMR überlegen, insbesondere bei der Rate an R0- und En-bloc-Resektionen. Daher sollte ESD bei größeren Läsionen als bevorzugte Standardprozedur in Betracht gezogen werden, trotz der höheren Rate an schweren Komplikationen. Verbesserte Erfahrungen und der Einsatz modernerer Instrumente könnten die ESD weiter optimieren und sicherer gestalten.

Topic and Research Question: This dissertation examines the outcomes of endoscopic therapy (endoscopic submucosal dissection [ESD] and endoscopic mucosal resection [EMR]) for early neoplasms of the upper gastrointestinal tract. The focus is on the factors of remission, recurrence rate, complications, and survival in neoplasms of the esophagus (low-grade intraepithelial neoplasia [LGIN], high-grade intraepithelial neoplasia [HGIN], early Barrett's adenocarcinomas, early squamous cell carcinomas) and the stomach (early gastric cancers).
Methodology: This retrospective study statistically evaluated the data of patients who were interventionally endoscopically treated at the Department of Gastroenterology of the Barmherzigen Brüder Hospital in Regensburg between 2011 and 2020.
Results:
• Neoplasm Remission: Complete remission was achieved in 98% of cases (97.7% in Barrett's neoplasms [BN], 100% in early gastric cancers [EGC]). In 2% of cases, complete remission was not achieved through endoscopic therapy.
• Recurrence-Free Survival: Over a total follow-up period of 3284 months, 11 recurrences were noted (6 in BN, 4 in EGC). Four deaths were recorded, with only one being tumor-associated. The 5-year recurrence-free survival (RFS) was 77.8% for the entire sample, 79.6% for BN, and 65.8% for EGC. Higher recurrence rates were observed in Barrett's carcinomas at higher T1a stages, horizontal R1 resections, and piecemeal resections.
• Comparison of Endoscopic Resection Techniques (EMR/ESD): EMR had a higher rate of horizontal R1 resections (39.7%) compared to ESD (14.1%). The en-bloc resection rate was higher with ESD (98.7%) compared to EMR (28.2%). The entire sample showed a higher recurrence rate after ESD than after EMR. When only "true" recurrences were considered, the rates were nearly equal. For Barrett's neoplasms, there was a reversed effect on the 5-year RFS: 80.0% in the EMR group and 90.0% in the ESD group.
• Complications in Endoscopic Therapy: Minor complications (stenosis or minor bleeding) occurred in 27 cases (18%), and severe complications (perforations and significant bleeding) occurred in 8 cases (5.3%). Severe complications were more common with ESD (6.7%) than with EMR (4.2%).
Conclusions: The results of this study are comparable to international studies and confirm the safety and effectiveness of endoscopic therapy for early neoplasms of the upper gastrointestinal tract. A high rate of stenosis after ablation of the residual Barrett's mucosa following endoscopic resection was observed, necessitating the development of strategies for prevention. ESD was superior to EMR in many factors, particularly in the rate of R0 and en-bloc resections. Therefore, ESD should be considered the preferred standard procedure for larger lesions, despite the higher rate of severe complications. Improved experience and the use of more modern instruments could further optimize and make ESD safer.
