Einführung und Ziele: Eine Makrohämaturie ist einer der Hauptgründe für eine Vorstellung in der urologischen Notaufnahme. Die Anzahl der Patient: innen, die aufgrund einer kardialen Genese einer dualen Thrombozytenaggregationshemmung und/ oder einer orale Antikoagulation unterliegen, steigt. Ziel dieser Arbeit war es, den Antikoagulationsstatus bei Vorstellung mit Makrohämaturie, sowie dessen Einfluss auf weitere Diagnostik und Therapie zu erfassen. Zudem sollte evaluiert werden, ob die Medikation pausiert oder abgesetzt wurde, um eine Therapie zu ermöglichen, bzw. ob und inwiefern eine definitive Therapie durch die Medikation verzögert wurde.
Material und Methoden: Es erfolgte die Anlage einer Datenbank mittels retrospektiver Datenerhebung. Grundlage dessen waren die Daten von Patient: innen, die im Zeitraum von 01.01.2021 bis 31.12.2021 die zentrale urologische Notaufnahme im Caritas Krankenhaus St. Josef Regensburg aufsuchten. Alle Patient: innen, bei denen eine Makrohämaturie diagnostiziert wurde, wurden hinsichtlich ihres Antikoagulations-status, bestehender Vorerkrankungen und dem weiterem Behandlungsverlauf analysiert. Die statistische Analyse wurde mit IBM SPSS® Statistics 29 durchgeführt.
Ergebnisse: Bei 541 Patient: innen wurde eine Makrohämaturie diagnostiziert. Davon nahmen 49,5% eines oder mehrere blutverdünnende Präparate ein. Darunter waren die größten Anteile mit 35,8% Thrombozytenaggregationshemmer und DOAKs mit 37,7%. Als häufigste Ursachen für die Makrohämaturie ließen sich Tumoren (22,2%) und Harnwegsinfektionen feststellen (19,8%). Bei 27,2% der Antikoagulierten wurde ein Tumor ursächlich für die Makrohämaturie diagnostiziert. Es ließen sich statistisch signifikante Unterschiede bezüglich festgelegter diagnostischer und therapeutischer Maßnahmen (wie z.B. Spülpflichtigkeit, das Vorliegen einer Tamponade oder dem Hb-Wert) zwischen der Personengruppe ohne Antikoagulation und derer mit Anti-koagulation feststellen. Ebenso konnte man signifikante Unterschiede zu einer vorhandenen urologischen malignen Vorerkrankung oder einer urologischen Tumor-erkrankung als Ursache nachweisen. Des Weiteren konnten auch zwischen den verschiedenen Wirkstoffgruppen einige Differenzen festgestellt werden. Auch ein höheres Alter und eine vorliegende Antikoagulation begünstigte in allen Fällen eine Makrohämaturie.
Es ließ sich kein Einfluss einer Pausierung der gerinnungshemmenden Medikation auf mögliche thromboembolische Komplikationen im Verlauf feststellen. Weder direkt, noch im Nachbeobachtungszeitraum von drei Monaten wurde eine Komplikation festgestellt.
Zusammenfassung: Die vorliegenden Daten zeigen, dass die Einnahme von blut-verdünnenden Medikamenten einen Einfluss auf das Auftreten und die Ausprägung einer Makrohämaturie hat. Das männliche Geschlecht, sowie höheres Alter und eine vorliegende maligne Vorerkrankung begünstigen das Auftreten einer Makrohämaturie unter Antikoagulation. Es sollte beim Auftreten einer Makrohämaturie immer eine weiterführende Tumordiagnostik stattfinden.
Eine Verzögerung der Therapie durch etwaige Komplikationen, die aus einer Pausierung oder einem Bridging hervorgehen, ließ sich nicht feststellen.

Introduction and aims: Macrohematuria is one of the main reasons for presentation to the urology emergency department. The number of patients on dual antiplatelet therapy and/or oral anticoagulation due to a cardiac etiology is increasing. The aim of this study was to determine the anticoagulation status of patients presenting with macrohematuria and its influence on further diagnostics and therapy. In addition, it was to be evaluated whether the medication was paused or discontinued to enable therapy, or whether and to what extent definitive therapy was delayed by the medication.
Material and methods: A database was created by means of retrospective data collection. This was based on the data of patients who visited the central urological emergency department at Caritas Hospital St. Josef Regensburg in the period from 01.01.2021 to 31.12.2021. All patients diagnosed with macrohematuria were analyzed with regard to their anticoagulation status, existing pre-existing conditions and the further course of treatment. The statistical analysis was performed with IBM SPSS® Statistics 29.
Results: Macrohematuria was diagnosed in 541 patients. Of these, 49.5% were taking one or more blood-thinning medications. Of these, the largest proportions were platelet aggregation inhibitors at 35.8% and DOACs at 37.7%. The most common causes of macrohematuria were tumors (22.2%) and urinary tract infections (19.8%). A tumor was diagnosed as the cause of macrohematuria in 27.2% of anticoagulated patients. Statistically significant differences were found between the group of people without anticoagulation and those with anticoagulation with regard to defined diagnostic and therapeutic measures (such as the need to flush, the presence of a tamponade or the Hb value). Significant differences were also found in the presence of a previous urological malignant disease or a urological tumor as the cause. Furthermore, some differences were also found between the various drug groups. Older age and existing anticoagulation also favored macrohematuria in all cases.
No influence of pausing the anticoagulant medication on possible thromboembolic complications in the course of the disease could be determined. No complications were observed either directly or during the follow-up period of three months.
Summary: The available data show that the use of blood-thinning medication has an influence on the occurrence and severity of macrohematuria. Male gender, older age and a previous malignant disease favor the occurrence of macrohematuria under anticoagulation. Further tumor diagnostics should always be carried out if macrohematuria occurs.
A delay in therapy due to any complications resulting from pausing or bridging could not be determined.
