Thromboembolische Ereignisse sind eine häufige Nebenwirkung von V-V ECMO. Die Interaktion zwischen Leukozyten und Thrombozyten in PNA und PMA spielen eine Rolle in der Entstehung von Thrombosen. Die Thrombozytenaktivierungsmarker PSelektin-Expression und Fibrinogenbindung könnten neben PNA und PMA als Indikatoren für die Entstehung von Thrombosen innerhalb der ECMO verwendet werden. Um diese Marker unter ECMO nachzuweisen, wurden Blutproben von 10 ECMO-Patienten vor und während der Therapie und von elf gesunden Probanden untersucht. Durch septische Krankheitsbilder zeigten sich PNA und PMA vor Beginn
der ECMO erhöht, es zeigte sich eine erhöhte P-Selektin-Expression, eine reduzierte Bindung von Fibrinogen und eine reduzierte Aktivierungsfähigkeit von Thrombozyten.
Innerhalb von zwei Stunden nach Beginn der Therapie zeigte sich ein Abfall der
PNA- und PMA-Level, zu einer weiteren Änderung unter ECMO kam es nicht. Die basale Bindung von Fibrinogen und P-Selektin-Expression veränderte sich nicht unter ECMO. Unter ECMO zeigten die Thrombozyten eine geringere
Aktivierungsfähigkeit, innerhalb von fünf Tagen kam es zum Abfall der P-SelektinExpression nach Stimulation. Die Untersuchungsparameter unterschieden sich vor Beginn der Therapie nicht zwischen Patienten, die einen Wechsel der ECMO erhielten, verglichen mit denen, die keinen Wechsel erhielten. In der vorliegenden Untersuchung konnte keine Eignung von PNA, PMA, P-Selektin-Expression und Fibrinogenbindung als Marker für das Auftreten einer Thrombose während einer Therapie mit ECMO festgestellt werden.

Thromboembolic events are a common side effect of veno-venous extracorporeal membrane oxygenation (VV ECMO). The interaction between leukocytes and platelets in PNA and PMA plays a role in the formation of thrombosis. Platelet activation markers, P-selectin expression, and fibrinogen binding, in addition to PNA and PMA, could be used as indicators of thrombosis formation within ECMO. To detect these markers under ECMO, blood samples from 10 ECMO patients were analyzed before and during therapy, and compared to samples from 11 healthy subjects. Because of sepsis there were increased PNA and PMA levels before the onset of ECMO, with elevated P-selectin expression, reduced fibrinogen binding, and reduced platelet activation capacity. Within two hours of therapy initiation, a decrease in PNA and PMA levels was observed, with no further changes during ECMO. Basal fibrinogen binding and P-selectin expression remained unchanged under ECMO. Platelets showed reduced activation capacity under ECMO, with a decrease in stimulated P-selectin expression within five days. The investigation parameters did not differ before therapy onset between patients who underwent ECMO exchange and those who did not. This study did not find PNA, PMA, P-selectin expression, and fibrinogen binding suitable as markers for the occurrence of thrombosis during ECMO therapy.
