Abstract (German)
Die AMD ist eine häufige, chronisch-progrediente Erkrankung der zentralen Netzhaut und der häufigste Grund für Erblindung in der westlichen Welt. AMD zählt zu den komplexen Erkrankungen, die individuelle Genetik trägt dabei erheblich zum Erkrankungsrisiko bei. Zu den typischen Veränderungen im Rahmen der Pathogenese gehört eine Anreicherung einer veränderten EZM und die Bildung von lipid- und proteinreichen Drusen basal und apikal vom RPE. Zudem scheinen eine chronische Überaktivierung des
Komplementsystems und oxidativer Stress an der Pathogenese beteiligt zu sein. Die genauen Pathomechanismen, die zu diesen Veränderungen führen, sind bislang jedoch nicht vollständig verstanden.
In dieser Arbeit wurden zwei der Risikofaktoren für die Entstehung von AMD, genetische Prädisposition und metabolisch-oxidativer Stress, in einem in vitro RPE Zellmodell kombiniert. Dazu wurden hiPSC-RPE-Zellen, abgeleitet von an AMD erkrankten Patienten bzw. Kontrollen mit definiertem, genetischem Risiko verwendet. Zudem wurde bei einem Teil der Zellen durch die
Langzeitfütterung mit POS ein physiologischer Stressor implementiert. Um einen weiteren wichtigen Risikofaktor, fortgeschrittenes Alter, adäquat zu berücksichtigen, wurden die Zellen vor der Durchführung der Experimente länger als bisher am Institut üblich kultiviert. Im Rahmen der
Arbeit wurde zunächst bestätigt, dass die Qualität der hiPSC-RPE-Zellen durch die veränderten Kultivierungsbedingungen nicht beeinträchtigt wurde, da die Differenzierung und Intaktheit der Zellen essenziell für die Validität des Modells zur AMD Forschung ist. Des Weiteren wurden geeignete experimentelle Protokolle für die Analyse der durch die Zellen gebildeten EZM und Ablagerungen etabliert. Mithilfe von Laserscanningmikroskopie wurde die Ablagerung von Strukturproteinen der EZM (FN, Laminin, Elastin, Kollagen IV und V) und von typischen Drusenkomponenten (C9, VTN, TIMP3, ApoE, β-Amyloid, Crystallin, Lipide) untersucht. Hier konnten, abhängig vom betrachteten Protein, Einflüsse des genetischen Risikos und des Stressors
POS-Fütterung auf die Menge und Morphologie der Ablagerungen gezeigt werden. Auffällig schien dabei, dass vor allem POS-Fütterung Einfluss nimmt, während das genetische Risiko eher geringe Effekte auf die Bildung von EZM und drusenartigen Ablagerungen hatte. Als wichtigen Faktor für den physiologischen Auf- und Abbau der EZM wurde zudem die Sekretion und Aktivität von MMPs durch die hiPSC-RPE analysiert. Hier war im apikalen Kompartiment wesentlich stärkere Aktivität von MMP2 vorhanden, die durch ein hohes genetisches AMD Risiko und POS-Fütterung weiter gesteigert wurde. In einem zusätzlichen Projekt wurden via qRT-PCR die Expression von
Genen der oxidativen Stressantwort (HMOX1, NQO1), des Komplementsystems (C3, CFH) und der mitochondrialen Aktivität und Integrität (COX4) untersucht. Hier führte ein hohes genetisches AMD Risiko und POS-Fütterung zu einer geringfügig gesteigerten Expression von NQO1 und C3. Inwiefern die in dieser Arbeit gezeigten Veränderungen kausal zusammenhängen oder parallele, voneinander unabhängige, Phänomene sind und inwieweit sie zur AMD-Pathogenese beitragen, bedarf weiterer Untersuchungen.

AMD is a common, degenerative disease of the central retina and the most common cause of blindness in the western world. AMD is a complex disease, with individual genetics contributing significantly to the risk of disease.
Typical changes in the pathogenesis include an accumulation of an altered ECM and the formation of lipid- and protein-rich drusen basal and apical to the RPE. In addition, chronic overactivation of the complement system and oxidative stress appear to be involved in the pathogenesis. However, the exact pathomechanisms leading to these changes are not yet fully understood.
In this study, two of the risk factors for the development of AMD, genetic predisposition and metabolic-oxidative stress, were combined in an in vitro RPE cell model. For this purpose, hiPSC RPE cells derived from AMD patients and controls with defined genetic risk were used. In addition, a physiological stressor was implemented in some of the cells by long-term feeding of POS. In order to adequately take into account another important risk factor, advanced age, the cells were cultivated for a longer time period than in previous investigations at the institute before the experiments were carried out. In the course of the work, it was first confirmed that the quality of the hiPSC-RPE cells was not affected by the changed cultivation conditions, as the integrity of the cells is essential for the validity of the model for AMD research. Furthermore, suitable experimental protocols were established for the analysis of the ECM and deposits formed by the cells. Using laser scanning microscopy, the deposition of structural proteins of the ECM (FN, laminin, elastin, collagen IV and V) and of typical drusen components (C9, VTN, TIMP3, ApoE, β-amyloid, crystallin, lipids) was investigated. Here, depending on the protein examined, influences of the genetic risk and the
stressor POS-feeding on the quantity and morphology of the deposits could be shown. It was striking that POS-feeding in particular had an influence, while genetic risk had only a minor effect on the formation of ECM and drusen-like deposits. Furthermore, the secretion and activity of MMPs
by the hiPSC-RPE was investigated, as MMPs play an important role in the physiological formation and degradation of the ECM. A much stronger activity of MMP2 in the apical compartment could be shown, which was further increased by a high genetic AMD risk and POS-feeding. In an additional project, the expression of genes of the oxidative stress response (HMOX1, NQO1), the complement system (C3, CFH) and mitochondrial activity and health (COX4) were investigated via qRT-PCR. Here, high genetic AMD risk and POS-feeding led to slightly increased expression of NQO1 and C3. The extent to which the changes shown in this work are causally related or are
parallel, independent phenomena and to what extent they contribute to AMD pathogenesis requires further investigation.
