Abstract (German)
In der vorliegenden retrospektiven Studie wurden das LPFS und das OS von 73 Patienten mit 105 Hirnmetastasen untersucht, die im Zeitraum vom 01.01.2017 bis 31.12.2021 mit dem Konzept 6 × 5 Gy in der Klinik und Poliklinik für Strahlentherapie des Universitätsklinikums Regensburg bestrahlt wurden. Es wurde der Einfluss verschiedener Variablen untersucht.
Das mittlere LPFS der 105 mit HFSRT bestrahlten Hirnmetastasen betrug 31,8 Monate. Die Raten des LPFS betrugen nach 6, 12, 18 und 24 Monaten 77,2 %, 66,4 %, 61,6 % und 55,7 %. Die lokale Kontrollrate war somit gut, wenn auch tendenziell niedriger als in einigen anderen Studien.
Einen signifikant positiven Einfluss auf das LPFS zeigten in der univariaten Analyse das Alter (< 65 Jahre, p = 0,008), der fehlende Nachweis extrakranieller Metastasen (p = 0,007) und die supratentorielle Lokalisation der Hirnmetastasen (p = 0,005).
Im 1. Kontroll-MRT, das im Median nach 6,9 Wochen durchgeführt wurde, zeigte sich bei 62 der 105 therapierten Hirnmetastasen (59,0 %) eine Volumenabnahme, bei 28 Hirnmetastasen (26,7 %) eine Volumenkonstanz und bei 15 Hirnmetastasen (14,3 %) eine Volumenzunahme.
Das mediane OS der 73 Patienten betrug 17,1 Monate. Im Vergleich zu anderen Studien mit einem medianen OS von 6 – 13 Monaten war es deutlich höher. Die Überlebensraten nach 6, 12, 18 und 24 Monaten betrugen 81,9 %, 58,1 %, 47,2 % und 41,3 %.
Einen signifikanten Einfluss auf das OS hatten in der univariaten Analyse das Alter (p = 0,021), der ECOG (p < 0,001), die RPA-Klasse (p = 0,002), der GPA-Score (p = 0,002), das Vorhandensein extrakranieller Metastasen (p = 0,001), die Systemtherapie (p = 0,014), das GTV (p = 0,004), das PTV (p = 0,003) und das Therapieansprechen im 1. Kontroll-MRT (p = 0,003).
Zusammenfassend scheint die HFSRT von Hirnmetastasen mit dem Fraktionierungskonzept 6 × 5 Gy eine effektive lokale Therapie zu sein. Eine lang anhaltende lokale Kontrolle von Hirnmetastasen ist relevant, da Fortschritte in der Systemtherapie die Prognose von Patienten mit Metastasen weiter verbessern.

In this retrospective study, local progression-free survival (LPFS) and overall survival (OS) were analyzed in 73 patients with 105 brain metastases who were treated between January 1, 2017, and December 31, 2021, using the 6 × 5 Gy hypofractionated stereotactic radiotherapy (HFSRT) protocol at the Department of Radiation Oncology of the University Hospital Regensburg. The influence of various variables was investigated.
The average LPFS of the 105 brain metastases treated with HFSRT was 31.8 months. The LPFS rates at 6, 12, 18, and 24 months were 77.2%, 66.4%, 61.6%, and 55.7%, respectively. Thus, the local control rate was good, although slightly lower than in some other studies.
In univariate analysis, age (< 65 years, p = 0.008), absence of extracranial metastases (p = 0.007), and supratentorial location of the brain metastases (p = 0.005) showed a significantly positive influence on LPFS.
In the first follow-up MRI, performed after a median of 6.9 weeks, 62 out of 105 treated brain metastases (59.0%) showed a volume reduction, 28 metastases (26.7%) showed stable volume, and 15 metastases (14.3%) showed an increase in volume.
The median OS of the 73 patients was 17.1 months. Compared to other studies reporting a median OS of 6–13 months, this was significantly higher. The survival rates at 6, 12, 18, and 24 months were 81.9%, 58.1%, 47.2%, and 41.3%, respectively.
In univariate analysis, significant factors influencing OS included age (p = 0.021), ECOG performance status (p < 0.001), RPA class (p = 0.002), GPA score (p = 0.002), presence of extracranial metastases (p = 0.001), systemic therapy (p = 0.014), gross tumor volume (GTV, p = 0.004), planning target volume (PTV, p = 0.003), and treatment response in the first follow-up MRI (p = 0.003).
In summary, HFSRT of brain metastases using the 6 × 5 Gy fractionation protocol appears to be an effective local treatment. Durable local control of brain metastases is important, as advances in systemic therapy continue to improve the prognosis of patients with metastatic disease.
