Im Zuge des demografischen Wandels steigt die Zahl älterer, multimorbider Patienten, die sich thoraxchirurgischen Eingriffen unterziehen. Ziel dieser Studie war es, den prädiktiven Wert des ISAR-Scores (Identification of Seniors at Risk) auf das postoperative Outcome bei Patienten ≥75 Jahren mit Pleuratumoren zu untersuchen.

In die retrospektive Analyse wurden 91 Patienten eingeschlossen, die zwischen 2015 und 2022 im Thoraxzentrum Ruhrgebiet operiert und im deutschen Pleuratumorregister erfasst wurden. Die Auswertung erfolgte anhand zweier Cut-off-Werte des ISAR-Scores (≥2 Punkte und ≥3 Punkte). Es zeigte sich, dass Patienten mit einem höheren ISAR-Score tendenziell länger hospitalisiert waren. Ein Cut-off von ≥3 Punkten war signifikant mit einer höheren 30-Tage-Letalität sowie einer erhöhten postoperativen Komplikationsrate assoziiert. Besonders Patienten mit präoperativ erfasster Hilfebedürftigkeit zeigten ein deutlich erhöhtes Risiko.

Der ISAR-Score erwies sich als geeignetes Instrument zur präoperativen Risikostratifikation bei geriatrischen Patienten mit Pleuratumoren. Ein Cut-off von ≥3 Punkten bietet dabei eine gute Vorhersagekraft für ein ungünstiges postoperatives Outcome.

In the context of demographic change, the number of elderly, multimorbid patients undergoing thoracic surgery is steadily increasing. The aim of this study was to evaluate the predictive value of the Identification of Seniors at Risk (ISAR) score on postoperative outcomes in patients aged ≥75 years with pleural tumors.

This retrospective analysis included 91 patients who underwent surgery between 2015 and 2022 at the Thoraxzentrum Ruhrgebiet and were documented in the German Pleural Tumor Registry. The analysis was performed using two different ISAR score cut-off values (≥2 points and ≥3 points). Patients with higher ISAR scores tended to have longer hospital stays. A cut-off of ≥3 points was significantly associated with higher 30-day mortality and an increased postoperative complication rate. Preoperatively documented need for assistance was particularly predictive of a higher postoperative risk.

The ISAR score proved to be a useful tool for preoperative risk stratification in geriatric patients with pleural tumors. A cut-off of ≥3 points provided strong predictive value for adverse postoperative outcomes.
