Abstract (German)
Die thoraxchirurgische Versorgung erfolgt in Deutschland vornehmlich in nicht-univer-itären Zentren mit klinischem Fokus und weniger an den Universitätsklinika. Analog zu anderen klinischen Fachgebieten ist auch in der Thoraxchirurgie die Forschung eine originäre Anforderung, der sich speziell universitäre Kliniken aber auch außeruniversitäre Eirichtungen im Rahmen der unterschiedlichen Zertifizierungsprozesse (OnkoZert - Deutsche Krebsgesellschaft und Doc-Cert - Deutsche Gesellschaft für Thoraxchirurgie) stellen müssen. Zur Erfüllung dieser Aufgaben ist für die Thoraxchirurgie zu un-terstellen, dass die vorhandenen Organisationsstrukturen, wie institutionalisierte (ei-genständige) Kliniken, Sektionen/Departments bzw. Leitungen auf Oberarztebene, unterschiedliche Voraussetzungen zur Leistungserbringung in der Forschung beinhalten. Ziel dieser Untersuchung war es, die Entwicklung der thoraxchirurgischen Publikationsleistung in Deutschland als Indikator für wissenschaftliche Leistungsfähigkeit an Universitätsklinika wie auch innerhalb der zertifizierten Lungenkrebs- (DKG) und Thoraxzentren (DGT) zu evaluieren und im Kontext der vorhandenen Organisationsstrukturen darzustellen. Als ein Maß zur Bestimmung der wissenschaftlichen Leistung galt dabei die Anzahl an Publikationen in international anerkannten (Peer-Review) Publikationsorganen sowie der Stellenwert der Fachzeitschriften als Zitationsorgan (Impact-Faktor). Es erfolgte eine PubMed-Recherche nach ausgewählten Autor*innen (Leiter*in thoraxchirurgischer Einrichtungen in Deutschland) im Zeitraum von 01.01.2012 bis 31.12.2021. Die Publikationsleistung in Quantität (Anzahl der Publikationen) und Qualität (Impact-Faktor: IF) wurde als Maßstab für die wissenschaftliche Aktivität der jeweiligen thoraxchirurgischen Einrichtung definiert und wurde zudem im jährlichen Verlauf analysiert. Der/die Leiter/-in einer thoraxchirurgischen Einrichtung wurde stellvertretend für die gesamte Einrichtung als primäres Suchkriterium festgelegt. Neben dem Vergleich von Universitätsklinika mit den nicht-universitären Zentren wurde eine weitergehende Analyse zur Beurteilung einer eigenständigen Organisati-onsstruktur durchgeführt. Innerhalb der universitären Thoraxchirurgie wurde zudem zwischen universitärer Thoraxchirurgie mit Professur (Berufung W-2/3, C4) vs. universitäre Thoraxchirurgie ohne Professur differenziert. Als Studienhypothese wurde eine positive Entwicklung der wissenschaftlichen Aktivität in der deutschen Thoraxchirurgie, gemessen an der Publikationsleistung in dem zehn-jährigem Untersuchungszeitraum, sowohl in universitären als auch nicht-universitären Einrichtungen, definiert. Zudem wurde insbesondere von den eigenständigen-universitären Einrichtungen eine hohe Publikationsaktivität sowie -qualität erwartet. Als pri-märer Endpunkt wurde die Anzahl an Publikationen im Vergleich von universitären Einrichtungen gegenüber nicht-universitären Zentren festgelegt. Sekundäre End-punkte waren die IF der Publikationsorgane, die Entwicklung in den letzten zehn Jahren und die Auswirkungen unterschiedlicher Organisationsstrukturen auf die Publikati-onsleistungen. Zur Aufarbeitung der Daten wurden unterschiedliche statistische Test-verfahren verwendet, so wurde der Kolmogorov-Smirnov-Tests zur Prüfung auf Abweichungen von der Normalverteilung eingesetzt. Zum Gruppenvergleich auf Differenzen wurden die nicht parametrischen Tests nach Kruskal-Wallis und Mann-Whitney-U so-wie im Weiteren der Eta-Quadrat-Koeffizient als Zusammenhangmaß angewandt. Die Auswertung umfasste 90 Standorte, wovon 36 universitären Einrichtungen und 54 nicht-universitären Lungenkrebs-/Thoraxzentren der DKG/DGT zugeordnet wurden. Alle eigenständigen universitären Einrichtungen waren mit einer Professur für Thoraxchirurgie (n= 9) besetzt, die nicht-eigenständigen universitären Einrichtungen (n= 27) waren mit 10 (37%) Professuren in Leitungsfunktion vertreten. Die insgesamt 126 ärzt-ichen Leiter*innen haben im Erhebungszeitraum in Summe 2414 Publikationen in 583 unterschiedlichen Publikationsorganen veröffentlicht. Für acht thoraxchirurgische Einrichtungen, resp. deren Leitungen, konnten keine Publikationen (8,9%) und für 34 Ein-richtungen maximal fünf Publikationen (37,8%) im Untersuchungszeitraum gefunden werden. Unter Verwendung des 5-Jahres-Journal-IF ergaben sich summarisch 12 137 IF-Punkte. Die Publikationen wurden überwiegend in englischsprachigen Zeitschriften (n= 2107 Publikationen; 87,2%) im Vergleich zu deutschsprachigen Zeitschriften (n= 307 Publikationen; 12,8%) veröffentlicht. Arbeiten mit spezifisch thoraxchirurgischen Inhalten überwogen (62%), wobei jedoch der Anteil mit nicht-thoraxchirurgischen Inhalten relevant hoch erschien (n= 913; 38%). Der überwiegende Beitrag der unter-suchten Autoren bestand in der Mitwirkung als Co-Autor*in bei 1624 Veröffentlichungen. Die Publikationsleistungen der nicht-universitären Zentren lag bei 661 (n=599 mit thoraxchirurgischem Inhalt), entspricht durchschnittlich 12 (11)/Zentrum und lag damit signifikant unter derjenigen der Universitätsklinika mit 1753 (n=902 mit thoraxchirurgischem Inhalt); 49 (25)/Klinik; p≤0,001. Signifikant höhere Publikationsleistungen bestätigten sich bei eigenständiger Thoraxchirurgie: n=586 (n=560 mit thoraxchirurgi-chem Inhalt);65 (62)/Klinik im Vergleich zu nicht- eigenständiger universitärer Thoraxchirurgie: n=1167 (n=342 mit thoraxchirurgischen Inhalt); 43 (13)/Klinik; p= 0,003. Es wurde ein Zehnjahrestrend mit einer nahezu Verdoppelung der Publikationsleistung von n=105 (universitär: n=63) auf n= 203 (universitär: n=124) Publikationen/Jahr nach-gewiesen. Innerhalb der universitären Thoraxchirurgie beeinflusste zudem das Vorhandensein einer Professur (Berufung) die Publikationsleistung. Kumulativ ergaben sich 2845 IF-Punkte (52,7 IF/Klinik) für nicht-universitäre Zentren, 6361 IF-Punkte (235,6 IF/Klinik) für die nicht-eigenständige und 2931 IF-Punkte (325,7 IF/Klinik) für die eigenständige universitäre Thoraxchirurgie. Der errechnete Quotient der Publikationsleistung über sämtliche Themeninhalte (Anzahl Gesamtpublikationen) und der Dauer des Studieneinschlusses wurde als Kennzahl für den Vergleich zwischen den unterschiedlichen Organisationsstrukturen verwendet. Hier ergab sich ein statistischer Zusammenhang (Eta-Koeffizient) zwischen der Organisationsstruktur und Publikationsanzahl (Quotient) mit 0,43 (p<0,001) sowie der Anzahl IF-Punkte (Quotient) mit 0,363 (p=0,002; Eta > 0,3). Für die Anzahl an Publikationen (Quotient) mit thoraxchirurgischer Fragestellung ergab sich ein Wert von 0,63 (p<0,0019) und für die Anzahl der Arbeiten mit onkologischer Fragestellung (Quotient) ein Wert von 0,37 (p=0,001).
Hinsichtlich der DKG-/DGT-Zertifizierung lassen die Zahlen keinen Zusammenhang anhand der Gesamtanzahl an Publikationen mit Eta-Koeffizient von 0,11 (p: n. s.) und 0,22 (p: n. s.) ermitteln. Ebenso fand sich kein signifikanter Zusammenhang zwischen einem Wechsel in der Leitungsfunktion während des Untersuchungszeitraumes und der Anzahl Publikationen (Quotient) mit einem Eta=0,045. Insgesamt bestätigte sich eine positive Entwicklung der Publikationsleistung als Parameter für die wissenschaftliche Aktivität in der deutschen Thoraxchirurgie, wobei der wesentliche Anteil der Publikationen und auch die Höhe der IF-Punkte im betrachteten Untersuchungszeitraum durch die universitäre Thoraxchirurgie zu verzeichnen waren. Insbesondere die eigenständige universitäre Thoraxchirurgie mit einer Professur leistete einen großen Anteil zur wissenschaftlichen Aktivität. Zur erfolgreichen Umsetzung von Forschungsprojekten erscheint ein akademisches universitäres Umfeld mittelbar notwendig, wenngleich, besonders in der Versorgungsforschung, auch die außeruniversitären zertifizierten Zentren wertvolle Beiträge leisten.

In Germany, thoracic surgery is primarily provided in non-university medical centers which prioritize a clinical focus as opposed to the more research-oriented approach characteristic of university hospitals.
Research constitutes an original requirement in thoracic surgery, as is the case in other clinical specialties. This requirement must be met by university hospitals, as well as non-university institutions, in order to comply with the various certification processes (OnkoZert - German Cancer Society and Doc-Cert - German Society for Thoracic Surgery).
In order to fulfil these tasks, it is necessary to make the following assumptions regarding thoracic surgery. It is imperative to acknowledge that the prevailing organizational frameworks, including autonomous hospital departments and sections/departments that are structured on the basis of senior physician expertise, inherently entail disparate prerequisites for the execution of research. The objective of this study was to examine the advancement of thoracic surgical publication performance in Germany as an indicator of scientific performance at university hospitals, as well as within the certified lung cancer (DKG) centers and thoracic centers (DGT). The number of publications in internationally recognized (peer-reviewed) journals and the relevance of the journals as citation organs (impact factor) were used to determine scientific performance.
A PubMed search was conducted for selected authors (heads of thoracic surgical institutions investigated) in the period from January 1, 2012 to December 31, 2021.
The performance of the thoracic surgical institutions in terms of publication was defined in two ways: quantity, defined as the number of publications, and quality, defined as the impact factor (IF). The performance was analyzed on an annual basis. The head of a thoracic surgery institution was defined as the primary search criterion on behalf of the entire institution.
A secondary analysis was conducted to determine whether the independence of department structure was pertinent. This investigation was undertaken in addition to the initial comparison of university hospitals with non-university centers. In the domain of university thoracic surgery, a discerning classification system was employed, distinguishing between academic institutions that housed thoracic surgery departments accompanied by a professorship (designated as appointment W-2/3, C4) and those devoid of such a professorship.
The study hypothesis assumed a positive development of scientific activity in German thoracic surgery, measured by publication output over the ten-year study period, both in university and non-university institutions. In addition, a high level of publication activity and quality was anticipated particularly from the independent departments at university hospitals. The primary endpoint was defined as the number of publications comparing university institutions with non-university centers. Secondary endpoints included the IF of the publication organs (journals), the development over the
last ten years and the effects of different structures by how departments of thoracic surgery were organized.
A range of statistical test procedures were employed to process the data, including the Kolmogorov-Smirnov test to ascertain deviations from the normal distribution. The non-parametric Kruskal-Wallis and Mann-Whitney-U tests were employed to compare groups for differences, and the Eta-squared coefficient was also utilized as a correlation measure.
The evaluation encompassed 90 sites, of which 36 were university institutions and 54 non-university lung cancer/thorax centers of the DKG/DGT.
A total of nine independent university institutions were staffed with a professorship for thoracic surgery, while 10 professorships in leadership roles were represented among the non-independent university institutions. The 126 surgical directors published a total of 2,414 publications in 583 different journals during the survey period. No publications could be found for 8 thoracic surgery facilities (8.9%), and a maximum of 5 publications could be found for 34 facilities (37.8%) during the study period. The application of the 5-year journal IF resulted in a total of 12137 IF points. The publications were predominantly published in English-language journals (n = 2107 publications; 87.2%) compared to German-language journals (n = 307 publications; 12.8%). The majority of the papers (62%) contained content related to specific thoracic surgical procedures, while a significant proportion (n = 913; 38%) addressed non-thoracic surgery-related subjects.
The majority of the authors surveyed contributed to 1624 publications as co-authors. The publication output of the non-university centers was 661 (n=599 with thoracic surgery content), corresponding to an average of 12 (11)/centers. This was significantly lower than that of the university hospitals, which had 1753 (n=902 with thoracic surgery content); 49 (25)/department; p≤0.001. A substantial enhancement in publication performance was observed for independent thoracic surgery departments, with a mean of 65 (62) publications per department, as compared to 43 (13) publications per department for non-independent university thoracic surgery departments. This discrepancy was statistically significant, with a p-value of 0.003. The data set revealed a ten-year trend, characterized by a substantial increase in publication output, with the number of publications increasing almost twofold.
The number of publications increased from 105 (university: 63) to 203 (university: 124) per year.
Within the university-affiliated departments of thoracic surgery, the presence of a professorship (appointment) also influenced publication performance. Cumulatively, there were 2845 IF points (52.7 IF/department) for non-university centers, 6361 IF points (235.6 IF/department) for non-independent centers, and 2931 IF points (325.7 IF/department) for independent organized university departments of thoracic surgery. The calculated quotient of publication performance across all subject content (number of total publications) and the duration of study inclusion was used as a key
figure for the comparison between the different structures for how thoracic surgery were organized. The findings indicated a statistically significant correlation (Eta coefficient) between the structural characteristics and the number of publications (quotient) with a value of 0.43 (p < 0.001), as well as the number of IF points (quotient) with 0.363 (p=0.002; Eta >0.3). The analysis yielded a value of 0.63 (p<0.0019) for the number of publications (quotient) with thoracic surgery content and a value of 0.37 (p=0.001) for the number of papers on content with oncology (quotient). With regard to DKG/DGT certification, the figures show no correlation based on the total number of publications with an Eta-coefficient of 0.11 (p: n. s.) and 0.22 (p: n. s.). Conversely, no substantial correlation was identified between alterations in management function (i.e., changes in department leadership) during the study period and the number of publications (quotient) with an Eta=0.045.
A positive trend in publication performance was confirmed as a parameter for scientific activity in German thoracic surgery. The majority of publications and the amount of IF points were derived from university-affiliated departments of thoracic surgery. In particular, independent departments of university-affiliated thoracic surgery with a professorship made a major contribution to scientific activity. An academic university environment appears to be indirectly necessary for the successful implementation of research projects, although the non-university certified centers also make valuable contributions, particularly in healthcare research.
