Hintergrund: Die Multiple Symmetrische Lipomatose (MSL) ist eine Fettverteilungsstörung unbekannter Ätiologie, die durch das disproportionale, symmetrische Wachstum lipomatösen Gewebes im subkutanen Fettgewebe charakterisiert ist. Hintergrund: Die Multiple Symmetrische Lipomatose (MSL) ist eine Fettverteilungsstörung unbekannter Ätiologie, die durch das disproportionale, symmetrische Wachstum lipomatösen Gewebes im subkutanen Fettgewebe charakterisiert ist. In Vorversuchen dieser Arbeitsgruppe konnte eine Regulation verschiedener Enzyme entlang des Phopsphoinositid-3-Kinase(PI3-Kinase)-Akt-mammalian target of rapamycin (mTOR)-Signalwegs nachgewiesen werden. Material und Methoden: Um herauszufinden, ob das Wachstum von lipomatösem Fettgewebe bei der Multiplen Symmetrischen Lipomatose auf eine Dysregulation des PI3K-Akt-mTOR-Signalwegs bei adipogenen Stammzellen (ASCs) zurückzuführen ist, wurden ASCs aus betroffenem und unbetroffenem, subkutanem Fettgewebe von an MSL-Erkrankten isoliert und kultiviert. Die ASCs aus betroffenem und unbetroffenem Gewebe wurden anhand ihrer Vitalität, der Genexpression verschiedener Apoptose-regulierender Gene und der adipogenen Differenzierbarkeit untersucht sowie der Einfluss von Buparlisib auf die Zellen analysiert. Schließlich wurde die Gen- und Proteinexpression von Enzymen des PI3-Kinase-Signalwegs detektiert. Ergebnisse: Es konnte gezeigt werden, dass ASCs aus betroffenem Gewebe eine höhere Vitalität aufweisen und im Vergleich schneller proliferieren als ASCs aus unbetroffenem Gewebe, sich jedoch schlechter adipogen differenzieren. Die Untersuchungen zur Genexpression Apoptose-regulierender Gene ergaben eine signifikante Induktion antiapoptotisch wirksamer Gene bei ASCs aus betroffenem Gewebe, was als Gegenregulation zu einem hyperproliferativen Stimulus bei ASCs aus betroffenem Gewebe gedeutet werden kann. Unter Buparlisib konnte sowohl die Proliferation von ASCs aus betroffenem Gewebe als auch deren Expression antiapoptotisch wirksamer Gene auf ein gesundes Maß gesenkt werden. Als mögliche Ursache für die Hyperproliferation bei ASCs aus betroffenem Gewebe konnte eine Induktion des PI3-Kinase-Signalwegs nachgewiesen werden, was sich in der erhöhten Proteinexpression von RPS6K1 und p-RPS6K1 zeigt. Unter allen untersuchten Genen des PI3-Kinase-Signalwegs zeigte PIK3CA bei ASCs aus betroffenem Gewebe die stärkste Induktion der Genexpression. Des Weiteren bleib die Gen- und Proteinexpression von PIK3CA bei ASCs aus betroffenem Gewebe unter Einfluss von Buparlisib unverändert, was auf eine autonome Signalwegstimulation bei ASCs aus betroffenem Gewebe hinweist. Darüber hinaus kann ein Missverhältnis der PTEN- und PIK3CA-Expression als Ursache der MSL diskutiert werden, da die Genexpression von PTEN im Vergleich zur Expression von PIK3CA nicht adäquat gesteigert war. Schlussfolgerung: Bei der Multiplen Symmetrischen Lipomatose kommt es zu einer Induktion des PI3-Kinase-Akt-mTOR-Signalwegs auf Stammzellebene.

Background: Multiple Symmetric Lipomatosis (MSL) is a fat distribution disorder of unknown etiology, characterized by disproportionate, symmetric growth of lipomatous tissue in the subcutaneous fat. Preliminary experiments of this working group demonstrated regulation of various enzymes along the phosphoinositide-3-kinase (PI3K)-Akt-mammalian target of rapamycin (mTOR) signaling pathway. Material and Methods: To investigate whether the growth of lipomatous fat tissue in MSL is due to dysregulation of the PI3K-Akt-mTOR signaling pathway in adipogenic stem cells (ASCs), ASCs were isolated and cultured from affected and unaffected subcutaneous fat tissue of MSL patients. ASCs from affected and unaffected tissue were examined with respect to their viability, gene expression of various apoptosis-regulating genes, and their adipogenic differentiation potential. In addition, the influence of buparlisib on the cells was analyzed. Finally, gene and protein expression of enzymes of the PI3K signaling pathway were detected. Results: ASCs from affected tissue showed higher viability and proliferated faster than ASCs from unaffected tissue but exhibited impaired adipogenic differentiation. Analysis of gene expression of apoptosis-regulating genes revealed a significant induction of anti-apoptotic genes in ASCs from affected tissue, which can be interpreted as a counter-regulation to a hyperproliferative stimulus in ASCs from affected tissue. Under buparlisib treatment, both proliferation of ASCs from affected tissue and the expression of anti-apoptotic genes were reduced to a healthy level. As a possible cause of hyperproliferation in ASCs from affected tissue, induction of the PI3K signaling pathway was demonstrated, reflected by increased protein expression of RPS6K1 and p-RPS6K1. Among all investigated genes of the PI3K pathway, PIK3CA showed the strongest induction of gene expression in ASCs from affected tissue. Furthermore, gene and protein expression of PIK3CA in ASCs from affected tissue remained unchanged under buparlisib treatment, suggesting autonomous pathway stimulation in ASCs from affected tissue. In addition, an imbalance between PTEN and PIK3CA expression can be discussed as a possible cause of MSL, since gene expression of PTEN was not adequately increased in relation to the expression of PIK3CA. Conclusion: In Multiple Symmetric Lipomatosis, induction of the PI3K-Akt-mTOR signaling pathway occurs at the stem cell level.
