Abstract (German)
Die allogene Stammzell- und Knochenmarktransplantation ist weiterhin für eine Vielzahl der hämatologischen Neoplasien die einzig kurative Behandlungsmethode, birgt aber auch erhebliche Risiken, unter anderem das der Entwicklung einer akuten Graft-versus-Host-Disease (GvHD). Entscheidend ist hier pathogenetisch die Aktivierung von Spender T-Zellen, welche sich daraufhin gegen Oberflächenmerkmale der Empfängerzellen richten. Heute ist ebenso bekannt, dass die Entstehung erheblich durch das Mikrobiom, insbesondere des Magen-Darm-Trakts, beeinflusst wird.
Der Einfluss einer Dysbiose des intestinalen Mikrobioms wurde in dieser Arbeit sowohl indirekt durch die Analyse der Gabe einer Breitspektrumantibiose zu verschiedenen Zeitpunkten vor und nach allogener STX, wie auch direkt durch 6 Mikrobiomparameter in Stuhl und Urin untersucht. Hierfür wurden von n = 285 Patienten klinische Daten ausgewertet, sowie Stuhl- und Urinproben einer pro- und einer retrospektiven Kohorte 7 Tage nach STX und bei Onset einer akuten GvHD bzw. an Tag 28 ± 5 bei Patienten ohne akute GvHD gesammelt und mittels qRT-PCR analysiert.
Dabei konnte in dieser Arbeit gezeigt werden, dass eine systemische Antibiotikatherapie mit Beginn mindestens 2 Tage vor STX zu einem Verlust der Diversität des Mikrobioms 7 Tage nach STX und einem schlechteren Gesamtüberleben, wie auch einer erhöhten TRM führt. Bei Beginn einer Antibiose 2 Tage vor Onset einer akuten GvHD Grad 1-4 konnte hingegen kein signifikanter Effekt mehr nachgewiesen werden, was darauf hinweist, dass der Zeitpunkt der Mikrobiomstörung von entscheidender Bedeutung ist.
Hinsichtlich der Suche eines im klinischen Alltag verlässlichen und flächendeckend einsetzbaren Mikrobiomparameters kann die Analyse der 16srRNA-Kopien pro Gramm Stuhl zum Zeitpunkt des Onset einer akuten GvHD Grad 1-4 in dieser Arbeit als am vielversprechendsten hervorgehoben werden. Es zeigte sich bei Onset eine signifikant geringere Kopienzahl im Stuhl bei Patienten mit akuter GvHD Grad 1-4 im Vergleich zu Patienten ohne akute GvHD an d28 ± 5. Allerdings ergab dieser Parameter eine für die klinische Risikobeurteilung unzureichende Spezifität und Sensitität in der ROC-Analyse. Dies liegt unter Umständen daran, dass dieser mittels qRT-PCR erhobene Parameter nicht zwischen potentiell commensalen und pathogenen Keimen unterscheidet und so schädliche Einflüsse nicht ausreichend detailliert benennen kann.
Insgesamt kann festgehalten werden, dass ein Diversitätsverlust des Mikrobioms des Magen-Darm-Trakts immunologisch nach allogener STX negative Auswirkungen hinsichtlich der Entwicklung und Prognose einer akuten GvHD und der therapie-assoziierten Mortalität hat und dies weiterhin ein aussichtsreiches Feld für Forschungen bleibt, um in Zukunft verlässliche neue Parameter zu Diagnostik, Prognoseabschätzung und Therapieplanung einer akuten GvHD zu gewinnen.

Allogeneic stem cell and bone marrow transplantation remains the only curative treatment for a large number of hematological neoplasms, but it also carries considerable risks, including the development of acute graft-versus-host disease (GvHD). The key pathogenetic factor here is the activation of donor T cells, which then target surface markers on the recipient cells. Today, it is also known that the development of GvHD is significantly influenced by the microbiome, especially that of the gastrointestinal tract.
The influence of dysbiosis of the intestinal microbiome was investigated in this study both indirectly by analyzing the administration of broad-spectrum antibiotics at various time points before and after allogeneic STX, and directly by analyzing six microbiome parameters in stool and urine. For this purpose, clinical data from n = 285 patients were evaluated, and stool and urine samples from a prospective and a retrospective cohort were collected 7 days after STX and at the onset of acute GvHD or on day 28 ± 5 in patients without acute GvHD and analyzed by qRT-PCR.
This study showed that systemic antibiotic therapy starting at least 2 days before STX leads to a loss of microbiome diversity 7 days after STX and poorer overall survival, as well as increased TRM. However, when antibiotic therapy was started 2 days before the onset of acute GvHD grade 1-4, no significant effect was observed, which indicates that the timing of microbiome disruption is crucial.
With regard to the search for a microbiome parameter that is reliable and widely applicable in everyday clinical practice, the analysis of 16srRNA copies per gram of stool at the time of onset of acute GvHD grade 1-4 can be highlighted as the most promising in this study. At onset, a significantly lower copy number in stool was observed in patients with acute GvHD grade 1-4 compared to patients without acute GvHD at d28 ± 5. However, this parameter showed insufficient specificity and sensitivity for clinical risk assessment in the ROC analysis. This may be because this parameter, which was determined using qRT-PCR, does not distinguish between potentially commensal and pathogenic bacteria and therefore cannot identify harmful influences in sufficient detail.
Overall, it can be concluded that a loss of diversity in the microbiome of the gastrointestinal tract following allogeneic STX has negative immunological effects on the development and prognosis of acute GvHD and therapy-associated mortality, and that this remains a promising field of research for the future development of reliable new parameters for the diagnosis, prognosis assessment, and therapy planning for acute GvHD.
