Sternumfrakturen bei thorakalen Wirbelsäulentraumata (Vierte-Säule-Frakturen, 4SF) sind Indikatoren hochenergetischer Unfallmechanismen und hoher klinischer Komplexität. Sie treten häufig zusammen mit AO-Spine-Typ-C-Verletzungen, thorakoabdominellen Begleitverletzungen und erhöhtem intensivmedizinischem Behandlungsbedarf auf; ältere Patienten zeigen eine deutlich erhöhte Mortalität.
Epidemiologisch dominieren männliche Patienten nach Verkehrsunfällen oder Stürzen aus großer Höhe. Frakturen im oberen und mittleren Brustwirbelsäulenbereich (Th1–Th7) sind signifikant häufiger mit 4SF assoziiert als Frakturen der unteren Abschnitte, was die biomechanische Vier-Säulen-Theorie des Thorax stützt. Eine AO-Spine-Typ-C-Morphologie geht häufig mit 4SF einher und kann als klinischer Hinweis auf massive Krafteinwirkung dienen.
4SF korrelieren eng mit thorakoabdominellen Verletzungen sowie Rippen- und Klavikulafrakturen, während eine inverse Beziehung zu instabilen Beckenringfrakturen auf unterschiedliche Unfallmechanismen hindeutet. Betroffene benötigen längere Intubationszeiten und Intensivaufenthalte und wurden signifikant häufiger operativ behandelt, insbesondere durch Wirbelsäulenstabilisierungen, mit verlängerter postoperativer Beatmungsdauer – bei insgesamt nicht signifikant verlängerter Gesamtaufenthaltsdauer.
Die Mortalität wurde vor allem durch höheres Lebensalter, thorakoabdominelle Begleitverletzungen und verlängerte postoperative Intubation beeinflusst. Multivariate Analysen bestätigten Alter ≥ 65 Jahre und einen erhöhten ISS als unabhängige Mortalitätsprädiktoren; das Vorliegen einer 4SF selbst zeigte keinen unabhängigen Effekt, steht aber für ein komplexes, potenziell lebensbedrohliches Traumamuster.
Die Ergebnisse unterstreichen, dass 4SF Kennzeichen schwerer Thoraxtraumata sind und eine sorgfältige Diagnostik, engmaschige respiratorische Überwachung sowie interdisziplinäres Management erfordern – insbesondere bei älteren Polytraumapatienten. Weitere multizentrische Studien sind nötig, um prognostische Faktoren zu präzisieren.

Sternal fractures in thoracic spinal trauma (fourth-column fractures, 4SF) are indicators of high-energy trauma mechanisms and considerable clinical complexity. They frequently occur in conjunction with AO Spine type C injuries, thoracoabdominal trauma, and an increased need for intensive care, with elderly patients in particular showing markedly higher mortality.
Epidemiologically, male patients predominate, most commonly following traffic accidents or falls from height. Fractures of the upper and mid thoracic spine (Th1–Th7) are significantly more often associated with 4SF than fractures of the lower segments, supporting the biomechanical four-column theory of the thorax. AO Spine type C fracture morphology frequently coincides with 4SF and may serve as a clinical indicator of substantial force transmission.
Analysis of concomitant injuries reveals a close correlation between 4SF and thoracoabdominal trauma as well as rib and clavicle fractures, while an inverse relationship with unstable pelvic ring fractures points to distinct injury mechanisms. Patients with 4SF require longer intubation times and ICU stays, reflecting greater injury severity, even though overall hospital length of stay is not significantly prolonged. They also undergo surgical intervention - particularly spinal stabilization - significantly more often and show prolonged postoperative ventilation, underscoring the increased complexity of these injuries.
Mortality was primarily influenced by older age, the presence of thoracoabdominal injuries, and prolonged postoperative intubation. Multivariate analysis confirmed age ≥ 65 years and a higher ISS as independent predictors of mortality; 4SF itself showed no independent effect but represents a complex, potentially life-threatening injury pattern.
These findings underline that 4SF are a hallmark of severe thoracic trauma and require careful diagnostic work-up, close respiratory monitoring, and interdisciplinary management - particularly in elderly polytrauma patients. Further multicenter studies are needed to refine prognostic factors
