The hepatorenal syndrome (HRS) is a complication of liver cirrhosis with high morbidity and mortality but it is primary of a functional and reversible genesis. Its diagnosis is based on the exclusion of other reasons for renal failure and the estimation of renal function.
Aim of this study was to compare the diagnostically value of the fractional excretion of urea (Fe(HS)), the Cystatin C and the glomerular filtration rate estimated by MDRD in sensitivity and specifity for detecting a HRS.
The creatinin has a sensitivity of 100% and a specifity of 79.0% at a cut-off value of >1.5mg/dl. For the creatinin-clearance the values are analogical 91.7% and 79.0% at a cut-off of <40ml/min, for the GFR estimated by MDRD 100% and 71.0% at <45ml/min. The best sensitivity (100%) and specifity (63.2%) for the Cystatin C is at a cut-off of >1.67mg/l. For the Fe(HS) the sensitivity is 100% and specifity 37.0% at a cut-off-value of <41.5%.
The conventionally parameter for renal function still have the best selectivity in detecting a HRS. The Sensitivity could be even advanced by the correction of the conventionally parameters with patients demographic date like done in the estimation of GFR by MDRD. The Cystatin C shows an equal sensitivity but with a lower specifity. The Fe(HS) is only useful for the exclusion of a HRS but not its detection.
