Abstract (German)
Die Expression von Chemokinrezeptoren wird zur Charakterisierung von Th Zell-Subpopulationen herangezogen. Es konnte gezeigt werden, dass humane Th17 Zellen, die maßgeblich bei der Entstehung von Autoimmunerkrankungen beteiligt sind, durch die Expression von CCR6 und CCR4 identifiziert werden können. Um die Rolle von CCR6 auf Th Zellen während der physiologischen Immunantwort zu klären, analysierten wir zunächst den Phänotyp und die Differenzierung von CCR6-positiven Zellen in einem Immunisierungsmodell.
Wir konnten zum ersten mal zeigen, dass CCR6 während der Immunreaktion ein hinreichender Marker zur Identifikation von funktionellen Th17 Zellen in immun-kompetenten C57BL/6 Mäusen ist, da sämtliche IL-17 produzierenden Th Zellen innerhalb der CCR6-positiven Th Zellfraktion lagen. CCR6-positive Th17 Zellen zeigten außerdem in naiven Mäusen einen aktivierten Phänotyp, ohne jede äußere Stimulation. Die Th17 Zellen befinden sich somit in ständiger �Alarmbereitschaft� und können daher schnell auf ein eindringendes Pathogen reagieren. Die Funktion von Th17 Zellen in der frühen Phase der Immunantwort konnten wir durch die Interaktion von Th17 Zellen mit Zellen aus dem Knochenmark nachweisen. In Kokultur Experimenten konnten wir zeigen, dass Th17 Zellen im Zusammenspiel mit Knochenmarkzellen ein proinflammatorisches Milieu generierten, was die Rekrutierung von Immunzellen zur Folge haben kann.
Im Lauf einer fortschreitenden Immunreaktion in unseren Immunisierungsmodellen verlieren Th17 Zellen an Bedeutung. Dies zeigte sich in einer erniedrigten Anzahl an Th17 Zellen und den geringen Mengen an IL-17 Protein nach der Impfung. Hier, in der akuten Phase der Immunantwort, sind IFN-γ produzierende Th1 Zellen die dominante Th Zell-Subpopulation. IFN-γ hemmt somit auch in vivo die Entwicklung und Expansion von Th17 Zellen im Zuge einer Erst-Exposition mit Antigen. Ungeachtet dessen entstanden nach Vakzinierung antigenspezifische CCR6-positive Th17 Zellen. In vivo aktivierte Th17 Zellen reagierten nach Stimulation mit spezifischem Antigen mit Zytokinproduktion und Proliferation.
Die Funktionalität von CCR6 als Marker für Th17 Zellen demonstrierten wir in der EAE. In diesem Mausmodell der Multiplen Sklerose werden myelin-spezifische, autoreaktive Th Zellen durch Vakzinierung mit PLP erzeugt, die zur Entwicklung einer progredienten Lähmung in den Tieren führt. Der Transfer von Th Zellen, die CD4+CCR6+ depletiert waren, führte zu einer deutlich abgeschwächten EAE. Die Erkrankung konnte allerdings nicht vollständig verhindert werden. Der Grund dafür mag in der Plastizität der Th17 Zellen begründet sein, die wir in Zelltransfer- Versuchen nachweisen konnten. CCR6-positive Th17 Zellen waren in vivo in der Lage, zu Th1 Zellen und Th1 Zellen zu Th17 Zellen zu differenzieren.
Weiterhin konnten wir einen neuen Mechanismus etablieren, der die pro-inflammatorische Wirkung von Th17 Zellen in chronisch-entzündlichen Erkrankungen erklärt. Th17 Zellen, nicht aber Th1 Zellen, wiesen unter chronischem Stress eine Glucocorticoidresistenz auf. Bis heute ist die Heilung einer Vielzahl von chronisch-entzündlichen Erkrankungen durch die Verabreichung von Glucocorticoiden nicht möglich. Die Dezimierung der Th1 Zell-vermittelten zellulären Immunantwort durch die Glucocorticoid-Gabe kann die akute Aufhebung der Gewebszerstörung zur Folge haben, doch bleiben, vor allem unter chronisch-entzündlichen Bedingungen, autoreaktive Th17 Zellen zurück, die jederzeit wieder eine neue Erkrankung auslösen können.
In dieser Arbeit konnte die eindeutige Identifikation von funktionellen und antigen¬spezifischen Th17 Zellen durch die CCR6 Expression gezeigt werden. Aufgrund unserer Befunde kann die selektive Neutralisierung von CCR6-positiven Th Zellen eine neue, vielversprechende Therapiemöglichkeit zur Bekämpfung von Autoimmunerkrankungen darstellen.

The Th cell subpopulations are characterised by the expression profile of chemokine receptors. It has been demonstrated that human Th17 cells which are mainly involved in the development of autoimmune diseases are characterised by the expression of CCR6 and CCR4. To study the role of CCR6 on the surface of Th cells we first analysed the phenotype and the differentiation of CCR6 positive cells in an immunisation model.
For the first time, we showed that the expression of CCR6 on Th cells is an adequate marker to identify functional Th17 cells in immune competent C57BL/6 mice. All IL-17 producing Th cells expressed CCR6. Furthermore, these cells already showed an activated phenotype in naïve mice. Therefore, Th17 cells are on alert and able to response rapidly to infiltrating pathogenic germs. We demonstrated the function of Th17 cells in the early phase of the immune response in co-culture experiments. Th17 cells and bone marrow derived cells together generated a pro-inflammatory milieu which could lead to the recruitment of additional cells of the immune system.
During the progressive immune reaction in our immunisation models Th17 cells became less important. After vaccination, the number of Th17 cells and the amount of produced IL-17 protein declined. At the peak phase of the immune response Th1 cells which produce IFN-γ represented the major Th cell subpopulation. Therefore, IFN-γ also inhibits the development and expansion of Th17 cells in vivo in the course of first-exposition with antigen. Albeit, we detected antigen specific CCR6-positive Th17 cells after vaccination. In vivo activated Th17 cells responded to antigen specific stimulation with cytokine production and prolilferation.
We demonstrated the functionality of CCR6 as a marker for Th17 cells using the EAE, the mouse model of multiple sclerosis. After vaccination with PLP, myelin-specific autoreactive Th cells were generated. This caused a progressive palsy in the animals. The transfer of CD4+CCR6+ depleted Th cells attenuated the course of the disease manifestly. However, we were not able to prevent the onset of the disease completely. The reason for this might be the plasticity of Th17 cells. In cell transfer experiments we detected that Th17 cells were able to convert to Th1 cells and Th1 cells developed to Th17 cells in vivo.
Furthermore, we established a new mechanism which illustrates the pro-inflammatory properties of Th17 cells in chronic inflammatory diseases. The application of chronic stress lead to the development of glucocorticoid resistant Th17 cells but not Th1 cells. To date, the healing of many chronic inflammatory diseases is not possible on the basis of the application of glucocorticoids. The administration of glucocorticoids induced a decimation of the Th1 mediated cellular immune response. This results in the acute neutralisation of tissue damage. But remaining autoreactive Th17 cells, especially under the terms of chronic inflammation, have the ability to initiate a new onset of the disease.
This study shows that CCR6 is a sufficient marker for functional and antigen specific Th17 cells. Based on our results, the selective neutralisation of CCR6 positive Th cells opens up a new promising therapy for autoimmune diseases.
