Patienten mit psychopathischer Persönlichkeitsstörung zeigen klinisch auffällige Deviationen ihrer emotionalen Verarbeitung, deren neurobiologische Hintergründe weitgehend ungeklärt sind. Patienten mit psychopathischer Persönlichkeitsstörung zeigen klinisch auffällige Deviationen ihrer emotionalen Verarbeitung, deren neurobiologische Hintergründe weitgehend ungeklärt sind. Die vorliegende fMRT-Studie prüfte, ob eine beeinträchtigte Fähigkeit zur Theory of Mind für die schwerwiegenden Defizite bei emotionalen Verarbeitungsprozessen psychopathischer Patienten mitverantwortlich gemacht werden kann.
Die Versuchspersonengruppen zeigten während der Bearbeitung einer Aufgabe zur Emotionsattribution auf Verhaltensebene keine Unterschiede. Auf neuronaler Ebene konnten jedoch signifikante Unterschiede in Form von Mehraktivierungen bei Psychopathen im Vergleich zu gesunden Probanden in parietalen (BA 40, BA 7) und temporalen Arealen (BA 22/39) nachgewiesen werden. Verglichen mit forensischen Kontrollprobanden zeigten Psychopathen Mehraktivierungen im fusiformen Areal (BA 18) sowie im medialen Präfrontalcortex (BA 10). Die Ergebnisse sprechen dafür, dass der Emotionsattribution bei psychopathischen Patienten im Vergleich zu forensischen und gesunden Kontrollprobanden distinkte kognitive Prozesse zugrunde liegen. Dieser innovative Befund an psychisch hochauffälligen Patienten zeigt, dass eine zielorientierte Bearbeitung kognitiv-emotionsgeleiteter Aufgaben von diesen zwar erfüllt, dies aber nur unter Zuhilfenahme komplexerer neuronaler Strukturen erreicht werden kann.

Psychopathy is a complex personality disorder of unknown etiology. Central to the disorder are difficulties in emotion processing as well as animpaired emotional responsiveness. Psychopathy is a complex personality disorder of unknown etiology. Central to the disorder are difficulties in emotion processing as well as animpaired emotional responsiveness.
Functional magnetic resonance imaging was used to consider whether a dysfunctional Theory of Mind can be held responsible for those severe difficulties.
Compared with non-psychopathic patients and healthy controls, psychopathic patients showed no differences regarding behavioral data during an emotional attribution task. They did show differences concerning neuronal correlates in terms of an overactivation in several brain areas such as the fusiform (BA 18) and prefrontal medial cortex (BA 10).
These results suggest that patients with psychopathic personality disorder are able to deal with cognitve tasks with emotional content. However it seems that this only works by recruiting additional, maybe more extensive, neuronal areas and networks.
