Hintergrund:
Untersuchungen von Rezeptortyrosinkinasen der humanen epidermalen Wachstumsfaktor-Rezeptor (HER)-Familie stehen im Fokus der modernen Tumorforschung. Die prognostische Bedeutung der HER-Rezeptoren in HNSCC (head and neck squamous cell carcinoma) wurde bislang kontrovers diskutiert.

Methoden:
In einem Kollektiv von insgesamt 242 HNSCC verschiedener Lokalisationen wurden alle vier Rezeptoren der HER-Familie (HER1 - HER4) sowohl auf DNA-Ebene mittels Fluoreszenz-in-situ-Hybridisierung (FISH) als auch auf Proteinebene mittels Immunhistochemie (IHC) retrospektiv hinsichtlich ihrer prognostischen Wertigkeit untersucht.
Dabei wurden die oralen Plattenepithelkarzinome (oral squamous cell carcinomas; n = 23; OSCC-Kollektiv) getrennt von den Karzinomen der übrigen Lokalisationen (Larnyx und Pharynx; n = 219; HNSCC-Kollektiv) betrachtet. Die immunhistochemischen und zytogenetischen Auffälligkeiten wurden im Anschluss vergleichend dargestellt und auf Unterschiede untersucht.

Ergebnisse:
HNSCC-Kollektiv: Univariate Überlebensanalysen zeigten einen signifikant negativen Prognoseeinfluss für HER2-Rezeptorüberexpression (p < 0,01), hohe proliferative Aktivität (gemessen über die Ki-67-Expression; p = 0,02) und chromosomale Instabilität (definiert als Trisomie oder hohe Polysomie in mindestens drei der vier untersuchten Chromosomen; p = 0,03). Hingegen erwiesen sich eine HER1-Rezeptorüberexpression und die damit signifikant assoziierte HER1-Genamplifikation (p = 0,01) nicht als prognostisch bedeutsam. Die multivariate Analyse bestätigte den HER2-Status auf Proteinebene als starken negativen Prognosemarker (HR = 3,405; p < 0,01).
OSCC-Kollektiv: In der univariaten Überlebensanalyse waren sowohl die HER1-Rezeptorüberexpression als auch die damit signifikant assoziierte HER1-Genamplifikation (p = 0,02) signifikant mit einem kürzeren Gesamtüberleben assoziiert (p jeweils < 0,01). In der multivariaten Analyse erwies sich die HER1-Amplifikation als prognostisch bedeutsamer und einziger signifikanter Parameter (HR = 10,809; p = 0,02).
Vergleich der beiden Kollektive: Ein Vergleich der immunhistochemischen und zytogenetischen Daten ergab gewisse Unterschiede in Expressions- und Genstatus der Wachstumfaktorrezeptoren zwischen den beiden Kollektiven. Es zeigten sich jedoch keine signifikanten lokalisationstypischen Assoziationen.

Schlussfolgerungen:
- In HNSCC stellen die HER2-Überexpression und die chromosomale Instabilität relevante, von den meisten gängigen Prognosemarkern unabhängige Prognosefaktoren dar.
- In OSCC spielt der HER1-Status eine Rolle als unabhängiger Prognosemarker. Dabei sind sowohl die Rezeptorüberexpression auf Proteinebene als auch die Amplifikation auf DNA-Ebene von prognostischer Bedeutung.
- Die OSCC unterscheiden sich in Expressions- und Genstatus der Wachstumsfaktorrezeptoren von HNSCC anderer Lokalisation, jedoch ohne statistische Signifikanz.

Translation of the abstract (English)

Background:
The HER (human epidermal growth factor receptor related) family has received considerable interest in modern cancer research. The prognostic value of HER-receptors in HNSCC (head and neck squamous cell carcinoma) has been discussed controversially.

Methods:
Immunohistochemistry (IHC) and fluorescence in situ hybridization (FISH) for HER1-4 were performed in 242 cases of head and neck squamous cell carcinomas in order to retrospectively evaluate the prognostic impact of gene status and protein expression of receptor tyrosine kinases of the HER-family in relation to established clinicopathologic parameters in squamous cell carcinomas of the upper respiratory tract.
Tumors of the larynx and pharynx (HNSCC; n=219) were analyzed separately from oral squamous cell carcinomas (OSCC; n=23), followed by a comparison of the two groups.

Results:
HNSCC: Univariate survival analysis revealed a significant negative prognostic impact for overexpression of HER2 (p < 0,01), high proliferative activity (measured by Ki-67-expression; p = 0,02) and high chromosomal instability (defined as trisomy or high polysomy in at least three out of four chromosomes; p = 0,03) whereas neither HER1-overexpression nor HER1-gene amplification were significant prognostic markers. Multivariate survival analysis confirmed HER2-overexpression as an important negative prognostic marker (HR = 3,405; p < 0,01).
OSCC: In univariate survival analysis both gene amplification and overexpression of HER1 were significant negative prognostic markers (p < 0,01, respectively). In multivariate analysis only HER1-gene amplification emerged as significant parameter of prognostic value (HR = 10,809; p = 0,02).
Comparison: Comparison of descriptive IHC- and FISH-data showed certain but not statistically significant differences between the two groups concerning receptor protein expression or gene status of the HER family-members.

Conclusions:
- Overexpression of HER2 and presence of chromosomal instability harbor an additional prognostic impact on disease-specific survival and prove to be independent negative prognostic factors in HNSCC.
- In OSCC, the HER1-receptor (overexpression and gene amplification) is an independent prognosticator of worse outcome.
- OSCC show certain but not statistically significant differences in receptor protein expression and gene status of the HER receptors compared to other locations of HNSCC.
