Abstract (German)
Das Karpaltunnelsyndrom ist das häufigste Kompressionssyndrom der peripheren Nerven. Ein Missverhältnis zwischen Breite und Inhalt des Karpaltunnels verursacht eine Druckschädigung des Nervus medianus. Mit der offenen, minimal invasiven chirurgischen Technik und mit Medialschnitt, unterhalb der Palmaraponeurose, gab es manchmal Muskelgewebe, das vor dem Erreichen des Retinaculum flexorum durchtrennt werden musste. Der Zweck der vorliegenden Studie ist es, festzustellen, wie oft dieses Muskelgewebe vorhanden ist und ob eine Korrelation zur Entstehung des KTS, sowie auf die prä-und postoperative Ergebnissen von klinischer und chirurgischer Seite bestand.
Methoden. In unserer Autopsien bestätigten wir die Existenz von Muskelgewebe zwischen Thenar und Hypothenar als Ursprungs Variation der Thenar-Hypothenar Muskulatur (Variationen Typ IIA und Typ IIB nach Singer und Ashworth, 2001). In 100 konsekutiven Patienten mit Karpaltunnelsyndrom, untersuchten wir die Häufigkeit von Muskel-Ursprungs-Variation (MUV). Die Nachuntersuchungen der Patienten, erfolgten durch Befragung nach einer Woche und drei Monaten. Wir verglichen die Gruppen KTS mit Variation (KTS+MUV) versus Karpaltunnelsyndrom ohne Variation (KTS-MUV), sowie KTS Patienten versus Kontrollgruppe (Leichen). Die statistische Auswertung erfolgte mit dem SPSS-Software über den Kruskal-Wallis-Test und Exakter Fisher-Test für unabhängige-Proben. Eine statistische Signifikanz wurde bei p <0,05 angenommen.
Ergebnisse. In 19% der Patienten bestätigten wir eine Muskel-Ursprungs-Variation. Die Prävalenz einer Muskel-Ursprungs-Variation bei KTS-Patienten war nicht höher als in der Kontrollgruppe (13%). Wir fanden keine signifikanten Unterschiede bei Alters- und Handverteilung. Die präoperativen Symptome wurden von allen Patienten gleich empfunden. Im Hinblick auf neurologische Parameter zeigten die Patienten mit Muskel-Ursprungs-Variation deutlich häufiger Parese (p=0,044) und Atrophie (p=0,005). Postoperative Untersuchungen ergaben eine höhere Inzidenz von Symptomverbesserung und Symptomfreiheit bei KTS-Patienten mit Muskel-Ursprungs-Variation nach einer Woche und nach drei Monaten (p=0,001 und p=0.001).
Diskussion. Zum ersten Mal berichten wir über die Rolle der Muskel-Ursprungs-Variation beim Karpaltunnelsyndrom Patienten. Wir haben die bereits beschriebenen Variationen Typ IIA und B von Singer und Ashworth, 2001, als eine einzige Variation, die Muskel-Ursprungs-Variation, beschrieben. Wir fanden keinen Anhalt, dass die Muskel-Ursprungs-Variation einen Einfluss auf eine häufigere und frühere KTS-Entwicklung hat. Wir glauben jedoch, dass MUV eine Druckerhöhung am Karpaltunnel bewirkt, jedoch nicht ausreicht, um KTS zu verursachen. Wir fanden keine klinischen Hinweise für eine Muskel-Ursprungs-Variation. Die Ergebnisse bei Patienten mit Muskel-Ursprungs-Variation waren bei der postoperativen Kontrolle statistisch signifikant besser. Aufgrund dessen können wir schlussfolgern, dass die MUV innerhalb dieser Zeit eine klinische Relevanz im Outcome hat. Eine langfristige Kontrolle wurde nicht durchgeführt. Wir vermuten jedoch, dass die Ergebnisse bei Patienten mit und ohne Muskel-Ursprungs-Variation gleich sind. Um Komplikationen zu vermeiden und einen optimalen Behandlungserfolg zu erzielen, ist eine genaue Kenntnis der Anatomie und deren Variationen des Karpaltunnels notwendig.

Carpal tunnel syndrome (CTS) is the most frecuent entrapment syndrome of peripheral nerves. A mismatch between the width and contents of the carpal tunnel causes a pressure damage of the median nerve. With open, minimal invasive surgery and medline incision, beneath the palmar fascia there is sometimes muscle tissue, that has to be transsected before reaching the flexor retinaculum. The purpose of the present study is to determine how often this muscle tissue is present and whether there exists any correlation to development of CTS, as well as a correlation to pre-and postoperative findings.
Methods. In a previous autopsy series, we confirmed the existence of muscle tissue between the thenar and hypothenar than the thenar-hypothenar origin variation muscle (MUV) (variations type IIA and type IIB by Singer and Ashworth, 2001). In 100 consecutive patients diagnosed with CTS and operated on, we examined incidence of muscle origin variations. (MUV). We followed the patients clinically and neurologically after 1 week and 3 months. We compared the groups CTS-Patients with variations (CTS+V) versus CTS-Patients without variations (CTS-V), as well as versus control group (bodies). Statistical analysis was performed with SPSS software applying a Kruskal-Wallis test and Fisher-Exact test for independent samples. Statistical significance was accepted at p <0.05.
Results. In 19% of patients, we confirmed a muscle origin variation (MUV). The prevalence MUV in CTS-Patients was not higher than in control group (13%). We found no significant differences in age and distribution of hand. The preoperative symptoms were suffered by all patients equally. With regard to neurological parameters, CTS+V had more often paralysis (p = 0.044) and atrophy (p = 0.005). Postoperative assessment revealed higher incidence of symptom improvement and symptom relief in CTS+V group at week and three months (p = 0.001 and p=0.001 respectively).
Discussion. For the first time we report about the role of the muscle origin variation in carpal tunnel syndrome patients. The already described variations type IIA and B by Singer and Ashworth, 2001, we have defined as one variation, the muscle origin variation. We found no evidence of more frequent and earlier development of CTS in CTS+V. However, we believe that MUV will increase the pressure on carpal tunnel, but insufficient to cause CTS. We found no clinical evidence for MUV. The postoperative findings in CTS+V were statistically significantly better. We can conclude that the MUV has within that time has a clinical relevance in the outcome. A long-term monitoring was not performed. We suspect, however, that the results are the same in CTS+V and CTS-V.. A detailed knowledge of anatomy and variations of the carpal tunnel is necessary to reduce complications and to achieve optimal treatment success.
