Die Dissertation untersucht die Häufigkeit von Tinnitus bei Patienten mit craniomandibulären Dysfunktionen (CMD), die Lokalisation der beiden Krankheitsbilder und den Einfluss funktionstherapeutischer Maßnahmen auf die Tinnitusbelastung.
Es wurden 951 Patienten der Neuaufnahme der Zahnärztlichen Prothetik (UKR) nach Tinnitus befragt und auf eine CMD untersucht. 25 der 30 Patienten die von Tinnitus als auch einer CMD betroffen waren, nahmen an der Studie teil. Weiterführende Untersuchungen der CMD erfolgten anhand des Klinischen Funktionsstatus der Deutschen Gesellschaft für Funktionsdiagnostik und -therapie (DGFDT) mit anschließender Auswertung nach den Richtlinien der Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) und des Helkimo-Index. Der Tinnitus wurde mithilfe von Fragebögen (Tinnitus Handicap Inventory (THI)/Tinnitusfragebogen) bewertet und beim Patient wurde eine Audiometrie und Ohrmikroskopie durchgeführt. Die anschließende Behandlung der Patienten erfolgte durch eine drei bis fünfmonatige Schienentherapie (je nach Indikation Distraktionsschiene (n=14) oder Michigan-Schiene (n=11)). Darüber hinaus wurde eine manuelle Therapie bei 16 Patienten durchgeführt und die Schmerzen von 3 Patienten medikamentös (Ibuprofen) behandelt. Die Abschlussevaluierung erfolgte nach drei bis fünf Monaten. Hierzu wurde erneut der Funktionsstatus, der THI als auch der Tinnitusfragebogen erhoben.
Die Prävalenz von Tinnitus bei CMD-Patienten war achtmal so hoch (36,6%) als bei Patienten ohne CMD (4,4%). Alle Patienten mit unilateralem Tinnitus und unilateraler CMD (n=8) wiesen das jeweilige Krankheitsbild auf der ipsilateralen Seite auf. Nach Anwendung funktionstherapeutischer Maßnahmen traten deutliche Erfolge bei Schmerzen im Bereich der Kaumuskulatur und Kiefergelenke (82%), bei Knacken im Kiefergelenk (69%) und bei Mundöffnungseinschränkungen (50%) ein. Außerdem zeigte die Endauswertung der durchgeführten Funktionstherapie, kombiniert mit Physiotherapie bei 11 von 25 Patienten (44%) eine Verbesserung oder totale Remission der Tinnitusbelastung.
Die Ergebnisse der Studie zeigen eine signifikante Korrelation zwischen Tinnitus und CMD. Bei unilateralem Tinnitus und unilateraler CMD ist die ipsilaterale Seite betroffen. Funktionstherapeutische Maßnahmen erweisen sich als erfolgsversprechend bei CMD-Patienten und scheinen bei etwa der Hälfte der Patienten die Tinnitusbelastung zu verringern.

Translation of the abstract (English)

The aim of this dissertation is to investigate the prevalence of tinnitus in patients with temporomandibular disorders (TMD), the localisation of both diseases and the effect of TMD treatment on tinnitus.
A screening of 951 patients for TMD has been conducted at the outpatient clinic for new patients (Department of Prosthetic Dentistry, University Hospital Regensburg). Additionally, they were asked if they were suffering from tinnitus. Out of 30 TMD-related tinnitus patients, 25 participated in the study. A clinical functional status according to the guidelines defined by the Deutsche Gesellschaft für Funktionsdiagnostik und -therapie (DGFDT) was obtained for further examinations of the TMD and evaluated following the guidelines of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) and the Helkimo index. Tinnitus was rated using standardized questionnaires (Tinnitus Handicap Inventory (THI)/Questionnaire for tinnitus). The otorhinolaryngologist conducted further examinations in form of audiometry and ear microscopy. The subsequent treatment of the patients included intraocclusal stabilization appliances throughout a duration of three to five months (occlusal distraction splint (n=14)/ Michigan splint (n=11)). Moreover 16 patients went through physiotherapy and 3 patients were treated with medication (ibuprofen). The final evaluation took place after three to five months, again consisting of collecting the clinical functional status, the THI and the Questionnaire for tinnitus.
The findings exposed a prevalence of tinnitus in TMD patients that was eight times as high (36,6%) as in patients without TMD (4,4%). Furthermore, the symptoms of all patients with unilateral tinnitus and unilateral TMD (n=8) manifested on the ipsilateral side. After TMD treatment myofascial pain and pain in the joints (82%), sounds of the joint (69%), limitation of mouth opening (50%) were found to be considerably improved. Moreover, the final evaluation of the TMD treatment combined with physiotherapy showed an improvement or total remission of tinnitus (11 out of 25 patients, 44%).
The results of this study show a significant correlation between tinnitus and TMD. In case of unilateral tinnitus and unilateral TMD the ipsilateral side is affected. TMD treatment proves to be promising for TMD patients and seem to reduce the strain of tinnitus for half of the patients.
