Diese Arbeit wurde im Rahmen einer internationalen Studie zur Aktualisierung des Fragebogenmodul EORTC QLQ-LC13, dem Standardinstrument zur Erfassung der Lebensqualität von Patienten mit Lungenkarzinom, durchgeführt. Abstract (German)
Diese Arbeit wurde im Rahmen einer internationalen Studie zur Aktualisierung des Fragebogenmodul EORTC QLQ-LC13, dem Standardinstrument zur Erfassung der Lebensqualität von Patienten mit Lungenkarzinom, durchgeführt. In dieser dritten Phase wurde das bis dahin entstandene provisorische Fragebogenmodul hinsichtlich Akzeptanz und Verständlichkeit bei den Patienten untersucht. Entstanden ist hierdurch der EORTC QLQ-LC29. Im Rahmen der Bearbeitung der Fragebögen zusätzlich gemachte Äußerungen der Patienten wurden erfasst und waren Gegenstand weiterer Fragestellungen.
Das Interview mit den Patienten läuft nach einem standardisierten Vorgehen ab: Zunächst beantworten die Patient selbstständig den Kernfragebogen EORTC QLQ-C30 und den krankheitsspezifischen Fragebogen EORTC-LC13 mit 35 neue Fragen. Anschließend werden unter Anleitung der Untersucherin alle Fragen des aktualisierten Moduls hinsichtlich Relevanz, Verständlichkeit und Aufdringlichkeit durch den Patienten bewertet. Es folgt eine Befragung des Patienten nach alltäglichen Lebensumständen und der Möglichkeit zur Selbstversorgung. Spontane Äußerungen des Patienten werden erfasst. Nach der Patientenbefragung folgt eine Bewertung der globalen Lebensqualität des Patienten aus Arztsicht.
Insgesamt machten 45 von 50 Patienten verwertbare Aussagen. Es konnten 105 abgrenzbare Aussagen gefunden werden. Die Aussagen werden folgenden sechs Kategorien zugeordnet: „Bewertung des eigenen Gesundheitszustandes“, „Schwerpunktsetzung auf einen Sachverhalt“, „Auffälligkeiten im Umgang mit der eigenen Vergänglichkeit“, „Durch Mimik ausgedrückte Stimmungslage“, „Krankheit im sozialen Miteinander“ und „Zukunftsängste“.
Bei einigen dieser Kategorien finden sich inhaltliche Überschneidungen mit Fragen, die auch im Fragebogen vorkommen. Dies trifft auf die Kategorien „Bewertung des eigenen Gesundheitszustandes“, „Durch Mimik ausgedrückte Stimmungslage“, „Krankheit im sozialen Miteinander“ und „Zukunftsängste“ zu. Es bestehen Überschneidungen zu den Fragebögen in Frage nach Schwierigkeiten durch Symptome, der Emotionalen und Sozialen Funktion, Zukunftsängsten und der Globalen Lebensqualität/Gesundheit.
Einige Kategorien kommen im Bogen gar nicht vor: „Schwerpunktsetzung auf einen Sachverhalt“ und „Auffälligkeiten im Umgang mit der eigenen Vergänglichkeit“. Durch den Fragebogen nicht erfassbare Inhalte sind die nonverbale Kommunikation durch Mimik und zwischenmenschlichen Umgangsformen, der für die Lebensqualität des Patienten wichtigste Faktor, sowie das Ausmaß der Akzeptanz und der Umgang mit der Erkrankung und der möglichen Konsequenz des eigenen Todes. Im Interview haben Patienten die Möglichkeit positive Aspekte der Lebensqualität zu nennen.
Im Fragebogen, aber nicht im Interview abgedeckte Inhalte sind die Rollenfunktion und Finanzielle Schwierigkeiten. Eine Bewertung der Lebensqualität insgesamt ist Bestandteil der Fragebogenskala Globale Lebensqualität/Gesundheit und kommt im Kategoriensystem so nicht vor.
Beim Abgleich der durch die Fragebögen generierten Lebensqualität mit den Äußerungen im Gespräch fallen bei sich zu den Kategorien „Bewertung des eigenen Gesundheitszustandes“ und „Krankheit im sozialen Miteinander“ äußernden Patienten bei sich inhaltlich überschneidenden Fragebogenskalen viele Patienten mit niedriger Lebensqualität auf. Es zeigen sich Diskrepanzen zwischen den Antworten im Fragebogen und den Inhalten der spontanen Äußerungen.
Ein begleitendes Interview eignet sich vor allem für den Einsatz des Bogens im klinischen Alltag. Dabei können die Lebensqualitätsfragebögen als Trigger für Äußerungen durch den Patienten dienen. Eine bei der Fragbogenbearbeitung stattfindende Betreuung des Patienten scheint im Sinne eines Informationsgewinnes gerechtfertigt. Es können individuelle, patientenbezogene Informationen gewonnen werden, die im Sinne der personalisierten Medizin für eine verbesserte Patientenversorgung genutzt werden können.
Durch ein Interview entsteht ein personeller Mehraufwand, mit zusätzlichen Kosten. Eine verbesserte Patientenversorgung kann aber auch durchaus zur Kostensenkung im Gesundheitssektor beitragen.

Translation of the abstract (English)
This work was carried out as part of an international study to update the EORTC QLQ-LC13 questionnaire module, the standard instrument for assessing the quality of life of patients with lung cancer. In this third phase, the previously developed provisional questionnaire module was examined in regard to acceptance and comprehensibility. The result is the EORTC QLQ-LC29. Additional comments from patients during the answering process of the questionnaires were recorded and were the subject of further questions.
The interviews with the patients follow a standardized procedure: First, the patients independently answer the questionnaire for all cancer patients, the EORTC QLQ-C30, as well as the disease-specific questionnaire EORTC-LC13 with 35 new questions. Afterwards, under the guidance of the examiner, all questions of the updated module are assessed by the patient in regard to relevance, comprehensibility and intrusiveness. Followed by a questioning of the patient according to everyday life circumstances and the possibility of self-sufficiency, spontaneous statements of the patient were recorded. After the patient survey, the procedure ends with an evaluation of the patient's overall quality of life from a physician's perspective.
In total, 45 out of 50 patients made additional statements. 105 definable statements could be found. The statements are categorized into the following six categories: "Assessment of the own state of health", "Focusing on one state of affairs", "Conspicuity in dealing with the own transience", "Mimic-expressed mood", "Sickness in social interaction" and "Future fears".
For some of these categories the content overlaps with questions that appear in the questionnaires. This applies to the categories "Assessment of the own state of health", "Mimic-expressed mood", "Sickness in social interaction" and "Future fears". The Categories overlap with the questionnaire-scales in troubles of symptoms, emotional and social function, fears about the future and global quality of life / health.
Some categories do not even exist: "Focusing on one state of affairs " and "Conspicuity in dealing with the own transience". Content not covered by the questionnaire is “non-verbal communication through facial expressions” and “interpersonal interactions”, the most important factor in the patient's quality of life, as well as the degree of acceptance and treatment of the disease and the possible consequences of the own death. In the interview, patients have the opportunity to name positive aspects of quality of life.
Content covered in the questionnaire, but not in the interview are “role function” and “financial difficulties”. An overall quality of life assessment is part o the ”Global Quality of Life / Health”- questionnaire and does not appear in the category system.
Comparing the quality of life generated by the questionnaires with the utterances in conversation, many patients with lower quality of life have more comments on the categories "Assessment of the own state of health " and "Sickness in social interaction". There are discrepancies between the answers in the questionnaire and the contents of the spontaneous statements.
An accompanying interview is particularly suitable for the use of the questionnaire in everyday clinical practice. The quality of life questionnaires can serve as triggers for patient comments. Taking care of the patient during the questionnaire procedure seems justified in terms of additional information gain. Individual, patient-related information can be obtained, which can be used in the sense of personalized medicine for improved patient care.
An interview creates additional staff costs, but an improvement in patient care can also help to reduce costs in the healthcare sector.
