Abstract (German)
Hintergrund: Bei der Rheumatoiden Arthritis (RA) handelt es sich um eine chronisch-entzündliche Systemerkrankung, die durch Synovialitis zu Arthritis führt. Die Erkrankung geht dementsprechend mit Gelenkschmerzen, Schwellung und Steifigkeit einher. Unbehandelt führt der schubweise progrediente Verlauf zu Knorpelschäden, Knochenerosionen, Gelenkdestruktion und Invalidität. Die Prävalenz der RA liegt bei Erwachsenen zwischen 0,5% und 1 % und steigt mit dem Lebensalter an. Generell sind Frauen von der RA dreimal häufiger betroffen als Männer. Die höchste Prävalenz zeigen Frauen im Alter von über 65 Jahren.
Da bisher keine Heilung der RA möglich ist, besteht die Therapie aus Allgemeinmaßnahmen, der Verabreichung von Glucocorticoiden, Basistherapeutika (DMARDs) und Biologika sowie operativen Verfahren als ultima ratio.
Bei IL-3 handelt es sich um ein Zytokin, welches eine Rolle bei der hämatopoetischen Reaktion auf infektiöse, immunologische und inflammatorische Stimuli spielt. Überwiegend wird dieses Zytokin von aktivierten CD4+ T-Lymphozyten gebildet, aber auch aktivierte Mastzellen und Basophile Granulozyten sind zur IL-3 Produktion fähig. Seine Wirkung entfaltet IL-3 jeweils durch die Bindung an den aus zwei Untereinheiten (α- und βc-Untereinheit) bestehenden IL-3 Rezeptor.
Ziel und Methodik: Ziel der Arbeit war es zu untersuchen, ob Patienten mit Rheumatoider Arthritis im Vergleich zu gesunden Probanden unterschiedliche IL-3 Konzentrationen im peripheren Blut aufweisen sowie gegebenenfalls dessen Effekte nachzuweisen. Begleitend wurde die Korrelation der IL-3 Rezeptor-Expression auf verschiedenen Zelltypen mit der Stärke der Krankheitsaktivität untersucht. IL-3 sollte als möglicher Angriffspunkt neuer therapeutischer Mittel dargestellt werden.
In die Studie aufgenommen wurden Patienten, bei denen die Diagnose einer RA bereits gestellt wurde. Ausschlusskriterien aus der Studie waren andere zusätzliche chronisch-entzündliche Erkrankungen. Parallel wurden gesunde Probanden rekrutiert. Den Probanden wurde peripheres Vollblut entnommen, welches noch am selben Tag in das Forschungslabor transportiert und dort verarbeitet wurde. Die Krankheitsaktivität der RA Patienten wurde anhand des Disease Activity Score 28 (DAS28) ermittelt. Anhand der Patientengruppen mit unterschiedlich stark ausgeprägter Aktivität der RA und der gesunden Kontrollgruppe wurde die IL-3 Produktion, die Expression des IL-3 Rezeptors sowie die Zellzahlen unterschiedlicher Zellpopulationen ausgewertet.
Ergebnisse: In der Studie befinden sich insgesamt 154 RA Patienten, davon 112 Frauen und 42 Männer im Alter zwischen 21,8 und 83,8 Jahren. Das durchschnittliche Alter der Patienten beträgt 56,1 Jahre. Insgesamt beinhaltet die Studie zudem 44 gesunde Probanden, darunter 31 Frauen und 13 Männer im Alter von 19,6 bis 78,3 Jahren. Das durchschnittliche Alter der Probanden beträgt hier 47,8 Jahre.
Die Patienten mit niedriger, moderater oder hoher Aktivität der RA litten verglichen mit den in Remission befindlichen Patienten an mehr druckschmerzhaften und geschwollenen Gelenken. Parallel zu den verstärkten Gelenkbeschwerden zeigte sich laborchemisch ein kontinuierlicher Anstieg der Entzündungsparameter. Bezüglich der täglichen Therapie mit Steroiden zeigte sich in dieser Arbeit eine Dosissteigerung mit zunehmender Krankheitsaktivität. Hier wurden auch signifikante Unterschiede zwischen den Patienten im Stadium der Remission und denen mit unterschiedlich starken Aktivitäten der RA verzeichnet.
Die Arbeit zeigt einen kontinuierlichen Anstieg der IL-3 Konzentration in Zellkultur-Überständen in Zusammenhang mit steigender Krankheitsaktivität. Überraschenderweise konnte im Gegensatz zur erwarteten IL-3 Produktion durch CD4+ T-Zellen eine verstärkte IL-3 Produktion durch CD8+ T-Zellen gezeigt werden. Die Anzahl der T-Zellen im peripheren Blut zeigte allerdings keine signifikanten Unterschiede zwischen den Patienten mit unterschiedlicher Aktivität der RA.
Bei den Patienten kam es sowohl auf den Basophilen Granulozyten als auch auf den plasmazytoiden Dendritischen Zellen zu einer Herunterregulation der IL-3-Rezeptor-α-Kette bei steigender Krankheitsaktivität. Die Herunterregulation der IL-3-Rezeptor-βc-Kette auf Basophilen Granulozyten und plasmazytoiden Dendritischen Zellen konnte im in vitro Versuch unter Zusatz unterschiedlicher IL-3 Konzentrationen nachgewiesen werden. Darüber hinaus zeigte sich im in vitro Versuch eine zunehmende Aktivität der Basophilen Granulozyten unter Zusatz unterschiedlicher IL-3 Konzentrationen.
Unter steigender Krankheitsaktivität konnte im peripheren Blut der RA Patienten ein Anstieg der Anzahl Basophiler Granulozyten/µl festgestellt werden.
Diskussion: Im Rahmen dieser Arbeit zeigte sich in den Überständen von Zellkulturen eine signifikante Zunahme der IL-3 Konzentration mit steigender Aktivität der RA. Hieraus könnte man schließen, dass höhere IL-3 Konzentrationen zu einem schwereren Verlauf der RA führen bzw. die Aktivität der RA darunter steigt.
Die Tatsache, dass in dieser Arbeit CD8+ T-Zellen die wichtigste Quelle der IL-3 Produktion in der RA darzustellen scheinen, wirft die Frage auf, ob RA Patienten mehr IL-3 produzierende CD8+ T-Zellen im peripheren Blut aufweisen oder ob die im peripheren Blut vorhandenen CD8+ T-Zellen mehr IL-3 produzieren. Bei der RA gibt es mehr IL-3 produzierende CD8+ T-Zellen, da der Prozentsatz der CD8+ T-Zellen, die IL-3+ sind, bei den RA Patienten höher ist und die Gesamtzahl der CD8+ T-Zellen gleich ist. Dies korreliert im Übrigen mit den steigenden IL-3 Konzentrationen in den Zellkultur-Überständen.
Gegensätzlich zu den erhöhten IL-3 Konzentrationen im Zellkultur-Überstand sank die Expression der α-Untereinheit des IL-3 Rezeptors sowohl auf Basophilen Granulozyten als auch auf plasmazytoiden Dendritischen Zellen signifikant mit zunehmender Krankheitsaktivität ab. Hieraus könnte man auf eine reaktive Minderung der IL-3 Rezeptor-Expression schließen, um die potentiell proinflammatorische Wirkung von IL-3 zu mindern. Die Expression der IL-3-Rezeptor-βc-Kette im in vitro Versuch ohne Anwesenheit von IL-3 war bei Basophilen Granulozyten und plasmazytoiden Dendritischen Zellen hoch signifikant stärker verglichen mit jener bei verschiedenen IL-3 Konzentrationen. Hieraus könnte man wiederum auf eine reaktive Minderung der IL-3 Rezeptor-Expression schließen, um die potentiell proinflammatorische Wirkung von IL-3 durch Veränderungen bzw. Verminderungen der Signaltransduktion zu senken.
Die Ergebnisse lassen auf eine gesteigerte Aktivität Basophiler Granulozyten bei erhöhten IL-3 Konzentrationen schließen. Da diese Arbeit erhöhte IL-3 Konzentrationen bei zunehmender Krankheitsaktivität der RA zeigt, könnte man von einer vermehrten Aktivität Basophiler Granulozyten bei steigender Krankheitsaktivität ausgehen. Durch das durch IL-3 bedingte Wachstum sowie die Differenzierung von Basophilen Granulozyten könnte man auf einen möglichen Zusammenhang zwischen dem Anstieg der IL-3 Konzentration mit steigender Krankheitsaktivität und der hier beschriebenen Zunahme Basophiler Granulozyten mit erhöhter Aktivität der RA schließen.
Mit dieser Arbeit konnte somit die wichtige Rolle von IL-3 im Krankheitsprozess der RA nachgewiesen werden. Möglicherweise stellt die Hemmung von IL-3 somit einen neuen therapeutischen Ansatz zur zukünftigen Behandlung der RA dar.

Background: Rheumatoid Arthritis (RA) is a chronic inflammatory and systemic disease characterized by synovial inflammation leading to joint tenderness, swelling and stiffness, eventually causing cartilage damage, bone erosions and joint damage. The prevalence rate is approximately 0,5% to 1% increasing with age and showing to be the highest in women over the age of 65 years. The disease is three times more common in women than in men.
Since there is no cure for RA yet, the therapy contains physical therapy, glucocorticoids, disease-modifying antirheumatic drugs (DMARDs), biologic agents and surgery.
IL-3 is a cytokine involved in the hematopoietic response to infectious, immunologic and inflammatory stimuli. It is produced mainly by CD4+ T cells but also by activated mast cells and basophil granulocytes. IL-3 shows its effects after binding to the IL-3 receptor, consisting of an α- and a βc-receptor subunit.
Aims and methods: The purpose of this study was to find out if the peripheral blood of RA patients contains different concentrations of IL-3 compared to healthy controls. The expression of the IL-3 receptor on the surface of different cell types and the correlation with disease activity was also investigated. IL-3 should be presented as a potential target in the future therapy of RA.
This study contains patients with diagnosed RA. Patients with any other chronic inflammatory diseases were excluded. At the same time there were healthy controls recruited. We took blood samples of the participants which were processed the same day in the research laboratory. The patients were categorized into several groups of different disease activities by using the Disease Activity Score 28 (DAS28). Compared to the healthy controls we investigated the IL-3 production, the expression of the IL-3 receptor and the cell numbers of different cell populations.
Results: We included 154 RA patients with 112 women and 42 men at the age from 21,8 to 83,8 years. The mean age was 56,1 years. Additional there were 44 healthy controls with 31 women and 13 men included at the age from 19,6 to 78,3 years. The mean age was 47,8 years.
RA patients with low, moderate or high disease activity were suffering from more tender and swollen joints compared to those in remission. We could also show a rise of the inflammation parameters at higher disease activity. The patients with higher disease activity received a higher dose of steroids compared to RA patients in remission.
We could show an increase in IL-3 production in the supernatant of cell culture in patients with higher disease activity. Unexpectedly CD8+ T cells seemed to be the main source of IL-3, not CD4+ T cells. Indeed there was no difference in the cell number of T cells in the peripheral blood.
The α-chain of the IL-3 receptor was downregulated on basophil granulocytes and plasmacytoid dendritic cells of the RA patients with higher disease activity. The downregulation of the IL-3-receptor-βc-chain on basophil granulocytes and plasmacytoid dendritic cells was shown in in vitro conditions with different concentrations of IL-3. Furthermore in the in vitro conditions we measured higher acitivity of basophil granulocytes after adding different concentrations of IL-3.
With increasing disease activity of the RA the number of basophil granulocytes/µl in the peripheral blood rised.
Discussion: As we could show an significant increase of the IL-3 concentration in the supernatant in cell culture in patients with higher disease activity, this could explain a contribution of IL-3 to a more severe progression of the arthritis.
As in this study CD8+ T cells were found to be the main source of IL-3 in RA patients, we investigated whether there are more IL-3 producing CD8+ T cells circulating in the peripheral blood or the CD8+ T cells in the peripheral blood produce more IL-3. We figured out that there must be more IL-3 producing CD8+ T cells because the percentage of CD8+ T cells being IL-3+ was higher and the total number of CD8+ T cells did not differ. After all this correlates with the increasing IL-3 concentrations measured in the cell culture supernatant.
Contrary to the increased concentrations of IL-3 in cell culture supernatant the expression of the IL-3-receptor-α-chain decreased on the surface of basophil granulocytes and plasmacytoid dendritic cells in patients with increasing disease acitivity. This could be interpreted as a reaction in order to reduce the potential proinflammatory effects of IL-3. As the absence of IL-3 in in vitro conditions showed a significantly higher expression of the IL-3-receptor-βc-chain on basophil granulocytes and plasmacytoid dendritic cells compared to different concentrations of IL-3, this could also be interpreted as a reaction to decrease the negative effects of IL-3.
As the results show higher activity of basophil granulocytes in the presence of higher concentrations of IL-3 and we measured higher IL-3 concentrations with increasing disease activity, there may be an augmented activity of basophil granulocytes at higher disease activity of the RA. The contribution of IL-3 to the growth and differentation of basophil granulocytes may correlate with the increasing concentration of IL-3 in higher disease activity and the previously described increase of the cell number of basophil granulocytes with rising disease activity of the RA.
So this study could demonstrate the important role of IL-3 in the disease process of RA. Possibly the inhibition of IL-3 can represent a new target in the future treatment of RA.
