Abstract (German)

Die hämatopoetische Stammzelltransplantation ist oft die einzig kurative Therapieoption bei hämatologischen Erkrankungen. Sie hat dabei aber ein hohes Mortalitätsrisiko. Ein Faktor dafür ist das erhöhte Risiko an Infektionen zu versterben bei fehlender Immunkompetenz.
Im Rahmen dieser Arbeit konnte gezeigt werden, dass die aGvHD einen signifikanten Effekt auf die B-Zell-Rekonstitution hat. So kommt es nach dem Auftreten einer aGvHD zu einem Reifungsarrest auf Höhe der Pro-B-Zellen im Knochenmark, der sich verzögert bis ins periphere Blut auswirkt und folglich zu niedrigeren B-Zellen bei Patienten nach aGvHD führt.
In der Analyse der B-Zellen und ihrer Vorläuferzellen konnten kaum signifikante Unterschiede zwischen Patienten ohne GvHD und Patienten mit de novo cGvHD gezeigt werden. Auch zwischen Patienten mit nur aGvHD und quiescent onset cGvHD lagen für diese Zellen kaum signifikante Unterschiede vor. Die cGvHD scheint sich daher kaum auf die B-Zell-Rekonstitution auszuwirken. Signifikante Unterschiede zwischen den einzelnen Gruppen resultierten daher vor allem aus dem Effekt der aGvHD.
Während bei der de novo cGvHD offensichtlich die Produktion von B-Zellen nicht beeinträchtigt ist, konnten Veränderungen im Memory-B-Zell-Kompartiment mit dem Auftreten von CD21- Memory B-Zellen im peripheren Blut wiederholt mit der cGvHD assoziiert werden, wobei es sich dabei um einen Knochenmark-unabhängigen Prozess handelt.

Frequently the haematopoietic stem cell transplantation remains the only curative therapeutic option in haematological diseases. There is a high risk of mortality. One factor is the increased risk to die because of an infection due to lacking immunological competence.
In this study we showed that the aGvHD has an singificant impact on the B cell reconstitution. There is a stop of the maturation at the stage of the pro B cell in the bone marrow after aGvHD. This effect you can see delayed in the peripheral blood what leads to a decreased B cells for patients after aGvHD.
In the analysis of the B cells and their precursor cells we couldn`t find significant between patients without GvHD and the novo cGvHD. Also between patients with aGvHD and quiescent onset cGvHD there have been marginally significant differences. It seems that the cGvHD has not a big effect on the B cell reconstitution. Significant differences between the groups are especially due to the effect of the aGvHD.
There is no problem of the B cell production during the de novo cGvHD. Changes in the memory compartment with the arise of CD21- memory B cells which is associated to the cGvHD seems to be a bone marrow independant process.
